Acute monoblastic leukemia with t(10;11)(p12;q23) presenting with pulmonary involvement: a case report and literature review.

A forty-three-year-old Thai man presented with acute fever and dyspnea for one week with bilateral patchy infiltration, pancytopenia with monoblast. Bone marrow study was consistent with acute monoblastic leukemia. Lung lesions rapidly progressed to acute respiratory failure, which required intubation. Bronchoscopy with bronchoalveolar lavage revealed monotonous monoblast infiltration. Induction chemotherapy with 7 + 3 regimen was administered to halt the progression of leukemic pulmonary infiltration. Although there was clinical improvement, the chest radiograph developed crescent formation i

A Rare Case Study of Granular Acute Lymphoblastic Leukemia Combined with Pleural Infiltration.

BACKGROUND: Acute B-lymphoblastic leukemia (B-ALL) is a common hematologic malignancy characterized by blasts with a variable amount of cytoplasm, typically ranging from minimal to moderate, and containing few cytoplasmic granules. However, granular acute lymphoblastic leukemia, as a rare subtype, is distinguished by the presence of abundant coarse, purplishred granules within the cytoplasm of the blasts, which can be confused with other diseases in clinical diagnosis. METHODS: The patient was examined using bone marrow morphological analysis, flow cytometry, genetic screening, and chromosome

Chronic subdural hematoma in a patient with acute myeloid leukemia and dural metastatic infiltration.

Dural metastasis (pachimeningeal carcinomatosis) refers to involvement of dura mater and subdural or epidural spaces in systemic cancer. Although dural metastasis are relatively frequent in malignancies, (8-9% of patients with advanced systemic cancer in autopsy), they are rarely associated with subdural hematoma. We present the case of a 66-year-old male, refereed to the hematological unit for severe anemia of unknown cause and diagnosed as acute myeloid leukemia, developed in the 10th day from admission progressive gait disorders with marked imbalance and mental status change. A CT head scan

[Bilateral papilledema secondary to chronic lymphocytic leukaemia].

CASE REPORT: We present the case of a 65-year-old man who was seen in the casualty department complaining of bilateral visual loss. His past medical history was unremarkable except for chronic lymphocytic leukaemia diagnosed two years previously: he was no longer receiving treatment for this disorder. His visual acuity was 20/200 in the right eye and 20/63 in the left eye and he had a bilateral papilledema. Computed tomography and magnetic resonance imaging of the brain and orbit were performed and showed an infiltrative mass in the orbit with extension to the ethmoid bone and both optic nerve

Ophthalmic manifestations of acute leukaemias: the ophthalmologist's role.

With evolving diagnostic and therapeutic advances, the survival of patients with acute leukaemia has considerably improved. This has led to an increase in the variability of ocular presentations in the form of side effects of the treatment and the ways leukaemic relapses are being first identified as an ocular presentation. Leukaemia may involve many ocular tissues either by direct infiltration, haemorrhage, ischaemia, or toxicity due to various chemotherapeutic agents. Ocular involvement may also be seen in graft-versus-host reaction in patients undergoing allogeneic bone marrow transplantati

Transient leukaemia cutis in chronic lymphocytic leukaemia.

Leukaemia cutis arises due to cutaneous infiltration of neoplastic leukocytes or their precursors. Recent evidence suggests that this sign does not necessarily herald a poor prognosis. We describe a 72-year-old woman with B-cell chronic lymphatic leukaemia (CLL) who developed a papular eruption on her breast at the site of a recent herpetic eruption. Histology and immunostaining showed a dense dermal B-cell infiltrate in keeping with leukaemia cutis. The papules cleared in 6 months without treatment, leaving atrophic scars. The histological features and possible aetiological mechanisms of post

Leukemia cutis.

Leukemia cutis is an uncommon manifestation of leukemia that is strongly associated with the presence of extramedullary disease at other sites. Patients usually present with leukemia cutis concomitantly with systemic leukemia or after leukemia has been diagnosed. Acute monocytic, myelomonocytic, and the T-cell leukemias show the highest incidence of leukemia cutis. The lesions show varied morphology and can be difficult to distinguish both clinically and histopathologically from nonspecific cutaneous lesions, which occur much more frequently. Immunohistochemistry is useful in making the distin

A Sudden Expansion: Acute Subdural Hematoma From Extramedullary Acute Myeloid Leukemia.

Chloromas are extramedullary agglomerations of malignant myeloid blast cells, also known as myeloid sarcoma. Myeloid sarcoma of the central nervous system (CNS) is a very rare presentation of extramedullary leukemic mass infiltration. This may be seen as an intracranial mass due to invasion of the brain parenchyma or as leukemic infiltration of the meninges. Such infiltration may present as a subdural hematoma (SDH), particularly in coagulopathic or immunocompromised patients undergoing chemotherapy. We present a 63-year-old male with acute myeloid leukemia (AML), previously treated with azaci

Inhibition of the SDF-1alpha-CXCR4 axis by the CXCR4 antagonist AMD3100 suppresses the migration of cultured cells from ATL patients and murine lymphoblastoid cells from HTLV-I Tax transgenic mice.

Adult T-cell leukemia (ATL) is a T-cell malignancy caused by human T lymphotropic virus type I, and presents as an aggressive leukemia with characteristic widespread leukemic cell infiltration into visceral organs and skin. The molecular mechanisms associated with leukemic cell infiltration are poorly understood. We have used mouse models of ATL to investigate the role of chemokines in this process. Transfer of splenic lymphomatous cells from transgenic to SCID mice reproduces a leukemia and lymphoma that is histologically identical to human disease. It could be shown that lymphomatous cells e

Case report: atypical, unilateral optic nerve infiltration as the first sign of acute lymphoblastic leukemia (ALL) relapse.

BACKGROUND: We describe a case of an atypical presentation of leukemic optic nerve infiltration. CASE PRESENTATION: A patient with acute lymphoblastic leukemia (ALL) in remission suffered from sudden right eye vision loss. At the time of presentation, the affected eye presented with an afferent pupillary defect, while the fundus examination was normal. A complete work up of the patient revealed no signs of ALL relapse, but MR imaging of the optic nerve showed contrast agent uptake consistent with optic nerve infiltration. The patient developed a fulminant ALL relapse and died shortly after. H

Management of mantle cell leukemia with cardiac involvement leading to cardiogenic shock.

Mantle cell lymphoma is an aggressive subtype of B cell non-Hodgkin lymphoma. It can progress to leukemic phase but frank leukemic picture at initial presentation is not common. Leukemic phase indicates advance stage of the disease and generally associated with extensive extra-nodal involvement. Pericardial invasion has been reported, however we could not find a report of myocardial infiltration by this disease since the appraisal of the term "mantle cell lymphoma" in 1992. Here we report a case of cardiac involvement by mantle cell leukemia leading to cardiogenic shock which complicates the t

STIM1 and STIM2 Mediate Cancer-Induced Inflammation in T Cell Acute Lymphoblastic Leukemia.

T cell acute lymphoblastic leukemia (T-ALL) is commonly associated with activating mutations in the NOTCH1 pathway. Recent reports have shown a link between NOTCH1 signaling and intracellular Ca2+ homeostasis in T-ALL. Here, we investigate the role of store-operated Ca2+ entry (SOCE) mediated by the Ca2+ channel ORAI1 and its activators STIM1 and STIM2 in T-ALL. Deletion of STIM1 and STIM2 in leukemic cells abolishes SOCE and significantly prolongs the survival of mice in a NOTCH1-dependent model of T-ALL. The survival advantage is unrelated to the leukemic cell burden but is associated with t

Leukemia Cutis Exhibiting the Koebner Phenomenon: A Rare Clinical Presentation of Acute Myeloid Leukemia.

Leukemia cutis with predilection for sites of prior cutaneous trauma, often described as a Koebner-like phenomenon, is a rare but recognized occurrence. We present a unique case of an, otherwise, healthy male who developed leukemia cutis at a prior unrelated biopsy site, leading to a new diagnosis of acute myeloid leukemia (AML). Certain AML subtypes with extramedullary propensity, particularly M4 and M5, as seen in our patient, are associated with an increased risk of leukemia cutis. Unlike their medullary counterparts, extramedullary AML cells preferentially migrate to and persist in tissues

Case Report: Optic nerve infiltration associated with sequential CRVO and CRAO in isolated CNS relapse of AML.

Optic nerve infiltration as the initial manifestation of isolated central nervous system (CNS) relapse in acute myeloid leukemia (AML) has rarely been reported. We present the case of a 70-years-old man with AML in clinical remission who presented with the sudden onset of a visual field defect and vision loss in the left eye. Initially the patient was diagnosed with non-arteritic anterior ischemic optic neuropathy (NAION) at a local hospital based on findings of decreased vision, optic disc edema and an inferior sectoral visual field defect connected to the physiological blind spot in the left

CD36 promotes leukemia progression and immune-microenvironment remodeling in MLL-AF9-driven AML.

CD36, a fatty-acid translocase, is increasingly implicated in acute myeloid leukemia biology and treatment resistance, yet its contribution to leukemogenesis is still unclear. Using the MLL-AF9 model, we transduced hematopoietic stem/progenitor cells (HSPCs) from Cd36-knockout (KO) or wild-type (WT) mice and assessed leukemic potential with in vitro assays, transplants, and transcriptomic, metabolomic, and immune profiling. Both Cd36KO- and Cd36WT-HSPCs underwent efficient MA9-driven transformation, with comparable colony formation and Hox/Meis1 pathway activation, indicating Cd36 is dispensab

Transcriptional regulation of expression of carbohydrate ligands for cell adhesion molecules in the selectin family.

Cell adhesion mediated by selectins and their carbohydrate ligands is involved in the adhesion of cancer cells to endothelial cells during the course of hematogenous metastasis of cancer. In patients with leukemia, this adhesion is involved in the extravascular infiltration of leukemic cells. Extravasation and tissue infiltration of malignant cells in patients with adult T-cell leukemia is mediated by the interaction of selectins and their carbohydrate ligand sialyl Lewis X, which is strongly and constitutively expressed on the leukemic cells. Constitutive expression of Lewis X in these cells

Cutaneous findings in hairy cell leukemia.

Hairy cell leukemia is a distinctive clinicopathologic entity that is histologically characterized by infiltration of the spleen, bone marrow, and blood by uniform mononuclear cells--"hairy cells." We reviewed the prevalent cutaneous findings, specific and nonspecific, in 113 cases of hairy cell leukemia. Only one patient had specific lesions (skin infiltration by leukemic cells). Nonspecific cutaneous findings included recurrent infections, ecchymoses, petechiae, pallor, drug reactions and reactions to transfusions, and nonherpetic ulcerations. Although leukemic infiltration of the skin in ha
