Experience with the levonorgestrel-releasing intrauterine system among HIV-infected women.

BACKGROUND: Safe and effective contraception is needed for women infected with human immunodeficiency virus (HIV). The levonorgestrel-releasing intrauterine system (LNG-IUS) is an effective long-term contraceptive that reduces menstrual bleeding and may reduce the risk of pelvic inflammatory disease. Yet, little is known about LNG-IUS use in HIV-infected women. SUBJECTS AND METHODS: Six HIV-infected women had the LNG-IUS inserted between March 1998 and October 2002, and were systemically followed for a mean of 45 months. Indications for LNG-IUS use were contraception in four women and menorrh

Morphological and functional changes in human endometrium following intrauterine levonorgestrel delivery.

The levonorgestrel releasing intrauterine system (LNG-IUS) provides a novel contraceptive method. Intrauterine LNG induces a dramatic transformation of the endometrium, characterized by extensive decidualization. This is associated with strong expression of local factors associated with decidualization, including prolactin receptor and insulin-like growth factor binding protein-1. A striking discovery was the down-regulation of oestrogen and progesterone receptors in all components of the endometrium after insertion of the LNG-IUS, with a gradual return between 6 and 12 months post-insertion.

A five-year clinical trial of levonorgestrel silastic implants (Norplant TM).

Silastic implants containing the progestin, levonorgestrel, were tested as a long-term contraceptive system in 101 women. During five full years of use, no pregnancies occurred. The 5-year continuation rate was 54%. Menstrual irregularities were the most frequent reason for termination of use but only during the first year. More than half of the terminations for this reason were in the first year. Some of the subjects elected to continue use of the implants beyond 5 years, allowing release rate data to be obtained through 6 years. From the second through the sixth year of use, the implants del

Model for the prediction of therapeutic failures of the 52 mg levonorgestrel intrauterine device.

INTRODUCTION: The 52 mg levonorgestrel intrauterine device (LNG-IUD) is used for contraception and the treatment of heavy menstrual bleeding. However, it may be associated with complications such as expulsion, early removal, and, in some cases, hysterectomy. Few studies have evaluated the clinical factors predicting these failures. OBJECTIVE: To develop a predictive model using indications for LNG-IUD use (contraception or treatment of PALM-COEIN conditions) to predict risk of failure (expulsion, early removal, and hysterectomy). METHODS: A retrospective observational study of 651 women unde

A Levonorgestrel-Releasing Intrauterine Device in the Right Upper Quadrant: A Case Report.

Abdominal pain is a common emergency department (ED) presentation. Studies suggest that female patients represent a majority of ED patients with abdominal pain. Abdominal pain requires a broad set of differential diagnoses, including gynecologic and obstetric causes. We describe the case of a patient who presented to the ED for right lower quadrant discomfort after the post-partum placement of a levonorgestrel-releasing intrauterine device (IUD). ED imaging revealed an IUD in the right upper quadrant. On laparoscopy, an intra-omental IUD was found near the hepatic flexure.

Body Mass Index and Levonorgestrel Device Expulsion in Adolescents and Young Adults.

STUDY OBJECTIVE: To evaluate the relationship between body mass and levonorgestrel intrauterine device (LNG-IUD) expulsion in adolescents and young adults (AYA). DESIGN, SETTING, PARTICIPANTS & INTERVENTIONS: A retrospective chart review was conducted of nulliparous females aged 10-24 years who had a 52-milligram LNG-IUD placed between November 2017 and May 2021 by pediatric and adolescent gynecology providers at a tertiary children's hospital, including those who underwent metabolic and bariatric surgery (MBS). Primary analysis focused on 10-19 year olds as they had comparable anthropometric

Factors Predicting Removals of the Levonorgestrel-Releasing Intrauterine System in an Adolescent Cohort.

OBJECTIVE: Use of 52-mg levonorgestrel intrauterine system (LNG-IUS) in adolescents for heavy menstrual bleeding (HMB), dysmenorrhea, and contraception has increased, yet little is known about the factors predicting removal and dissatisfaction in adolescents. The aim of this study was to identify factors predicting LNG-IUS removal in adolescents. METHODS: This was a retrospective cohort study including all adolescents (9-19 years) who underwent LNG-IUS insertion between 2012 and 2021 (n = 536). A medical record review was conducted and data were collated on medical and gynecological history,

Pregnant with a perforated levonorgestrel intrauterine system and visible threads at the cervical os.

We present a case in which a patient presented with a pregnancy of unknown location and normally rising human chorionic gonadotropin (hCG) levels but with a levonorgestrel intrauterine device (LNG-IUD) present. The LNG-IUD had been placed 4.5 years ago. Although unintended, this pregnancy was desired. Strings were clearly visible and initial 2D ultrasound suggested intrauterine location of the LNG-IUD. The LNG-IUD could not be removed however. The patient was managed expectantly with close follow-up, serial beta-hCGs and serial ultrasounds until definitive diagnosis of the location of the preg

Cervical perforation by the strings of a levonorgestrel releasing-intrauterine system: a case report.

A very rare case of perforation of the uterine cervix by the strings of a levonorgestrel releasing-intrauterine system (LNG-IUS) in a 47-year-old woman is presented. The method to retrieve the strings back into the cervical canal, leaving the LNG-IUS in situ, is described. We suggest that the threads of an intrauterine contraceptive occasionally may penetrate tissue like setons do, when used for treatment of fistulae.

Contraceptive efficacy of emergency contraception with levonorgestrel given before or after ovulation.

BACKGROUND: The contraceptive efficacy of emergency contraceptive pills containing levonorgestrel (LNG-EC) has been estimated in most previous studies by judging the day of ovulation from presumptive menstrual cycle data, thus providing poorly reliable estimates. METHODS: In the present study, the efficacy of LNG-EC was determined in 393 cycles by dating ovulation on the basis of reliable hormonal and ovarian parameters validated by a database constructed in a separate study. In addition, the efficacy was determined separately for cycles in which LNG-EC was given before or after ovulation. R

Contraceptive efficacy of emergency contraception with levonorgestrel given before or after ovulation.

BACKGROUND: We aimed to evaluate whether emergency contraception with levonorgestrel (LNG-EC) administered after ovulation is equally effective to LNG-EC administered before ovulation. STUDY DESIGN: We studied a cohort of women attending a family planning clinic for EC. From interview, we recorded menstrual history, time of intercourse and of intake of LNG-EC. On the day of intake of LNG-EC and during 5 days' follow-up, blood samples were taken for examination of luteinizing hormone, estradiol and progesterone concentrations, and vaginal ultrasound examinations were done for size of the leadi

Perforation of the cervix by the threads of a levonorgestrel-releasing intrauterine system: a case report.

Asymptomatic retraction of intrauterine device (IUD) threads into the cervical canal or uterine cavity is a common complication. We present the rare case of a 44-year-old woman with the threads protruding through an opening in the anterior cervical lip about 1 cm above the external cervical os. Despite this presentation, her management was straightforward. We review the literature and discuss the possible antecedents of the phenomenon in this patient.

Is there a role for use of levonorgestrel intrauterine system in women with chronic pelvic pain?

This review focuses on the available evidence for the use of levonorgestrel (LNG) intrauterine system (IUS) in women with chronic pelvic pain (CPP). We have searched MEDLINE, Pubmed, Embase, and the Cochrane electronic library with the keywords "chronic pelvic pain," "endometriosis," "Mirena IUS," and "levonorgestrel IUS" from 1956 through April 2008. Review articles, randomized trials, prospective cohort studies, and retrospective studies were analyzed and the available evidence included in this review. Case reports were not included in the analysis. Laparoscopic surgery was shown to be usefu

The levonorgestrel-releasing intrauterine system as an alternative to hysterectomy in peri-menopausal women.

INTRODUCTION: Hysterectomy is one of the most common surgical procedures performed in gynecological practice, and the most common indication for hysterectomy is menorrhagia or excessive menstrual bleeding. Approximately 10-15% of fertile women have menorrhagia, defined as a menstrual blood loss (MBL) of >80 mL. Since operative treatment in the form of a hysterectomy carries a not insignificant risk of morbidity and mortality, various forms of medical treatment have been advocated as initial treatment. RESULTS: Antifibrinolytic agents, e.g., tranexamic acid (40-50% reduction), prostaglandin sy

Immediate postabortal contraception with the levonorgestrel intrauterine device, Norplant, and traditional methods.

Women seeking legal first trimester abortion were counseled concerning contraception methods available for use immediately postabortion. Fifty women each accepted hormonal methods that were available only in the clinic and were novel to the country, the levonorgestrel IUD and Norplant implants, whereas another 50 chose either coitus interruptus or abstinence. All were experienced contraceptors, but larger percentages of women selecting the levonorgestrel IUD or implants had used the pill or IUDs previously, were under age 30, and weighed less than 60 kg compared to the other study participants

When Ultrasound and Hysterometry Disagree: Challenges in Levonorgestrel-Releasing Intrauterine System Placement.

Insertion of a levonorgestrel-releasing intrauterine system (LNG-IUS) may be technically challenging in women with altered uterine anatomy or difficult cervical access. We report the case of a 24-year-old nulligravid woman with obesity and a subdermal contraceptive implant who sought transition to an LNG-IUS. Pre-procedural transvaginal ultrasound (TVUS) demonstrated a retroverted uterus with apparently normal uterine dimensions. However, during insertion, hysterometry measured approximately 4 cm, raising concern regarding a reduced uterine cavity and possible perforation risk. The procedure w
