Perioperative Resuscitation and Life Support (PeRLS): An Update.

Cardiovascular collapse and arrest in the periprocedural setting and intensive care unit differ from arrests in other contexts (such as out-of-hospital or hospital ward) because clinicians almost always witness the event, and the most likely precipitating cause may be known. In comparison to other settings, the response can be timelier and more focused on treating the underlying cause(s). Since many patients deteriorate over minutes to hours, clinicians can evaluate the patient expeditiously, generate a diagnosis, and initiate appropriate treatment more rapidly than in other arrest circumstanc

Providing nutritional support for patients with cancer cachexia.

Malnutrition is the single most common secondary diagnosis for patients with advanced cancer, and can affect up to 85% of patients. Anorexia and weight loss affect up to 80% of patients and are responsible for death in up to 20% of cases. Cancer cachexia is a feature of advanced cancer characterised by anorexia and a progressive loss of body weight, reduced immune response, poor treatment response, and poor quality of life. Providing effective nutritional support for patients who experience cancer cachexia at the end of life is very challenging and requires a multidisciplinary team approach to

Practical cancer cachexia management in palliative care - a review of current evidence.

PURPOSE OF REVIEW: To explore the current evidence relating to the practical management of cancer cachexia in palliative care. RECENT FINDINGS: The authors found a growing evidence base including the publication of several expert guidelines since 2020. Guidelines identified the need for individualised nutritional and physical exercise support as the mainstay of cachexia management. Dietician and allied health professional referrals are recommended for the best patient outcomes. Limitations of nutritional support and exercise are acknowledged. Patient outcomes from multimodal anti-cachexia the

Emergency Neurologic Life Support: Meningitis and Encephalitis.

Bacterial meningitis and viral encephalitis, particularly herpes simplex encephalitis, are severe neurological infections that, if not treated promptly and effectively, lead to poor neurological outcome or death. Because treatment is more effective if given early, the topic of meningitis and encephalitis was chosen as an Emergency Neurological Life Support protocol. This protocol provides a practical approach to recognition and urgent treatment of bacterial meningitis and encephalitis. Appropriate imaging, spinal fluid analysis, and early empiric treatment is discussed. Though uncommon in its

Ethical and Legal Considerations in the Management of an Unbefriended Patient in a Vegetative State.

BACKGROUND: Patients without surrogates are referred to as "unbefriended." Because these patients do not have representatives to assist with medical decision-making, patient autonomy and self-determination, fundamental concepts of American healthcare, are jeopardized. METHODS: We present a case of an unbefriended patient in a vegetative state and discuss the ethical and legal complications associated with management of unbefriended patients. RESULTS: An unbefriended patient was admitted to our hospital with a cardiac arrest in the setting of an intracerebral hemorrhage. Despite aggressive me

Brain death: ethical challenges to palliative care concepts of family care.

Brain death is a controversial issue that is often difficult for families to understand or accept. Palliative care interventions can help families to accept the death. However, delaying pronouncement of brain death may be detrimental to the family and lead to financial, ethical, and legal complications, including the potential for insurance fraud. We describe a case of brain death in which the passage of time along with continuation of life support without concomitant testing for brain death led to decreased acceptance of the patient's death by the family. Clinicians should weigh the risks and

American Heart Association 2025 Guidelines: Basic life support, advanced cardiovascular life support, pediatric advanced life support, and neonatal resuscitation.

The American Heart Association (AHA) regularly updates its evidence-based practice recommendations for basic life support, advanced cardiovascular life support, pediatric life support, and neonatal life support. This article reviews the latest AHA guidelines, which were released in October 2025, highlights important changes, and demonstrates how nurses can incorporate these changes into their practice. Examples of new practice recommendations include back blows and abdominal thrusts in the management of severe foreign body obstruction in the conscious adult, use of the two thumbs encircling ap

The use of invasive ventilation is appropriate in children with genetically proven spinal muscular atrophy type 1: the motion against.

Spinal muscular atrophy (SMA) is a relatively common, profoundly disabling and incurable disease that presents in early childhood with hypotonia, weakness and decreased movement. Without ventilatory support, premature death from respiratory insufficiency is universal in children with spinal muscular atrophy type 1 (SMA1). With mechanical ventilation, however, long-term survival in SMA1 has been reported from numerous international centres. Children kept alive by this means experience progressive paralysis and eventually become effectively 'locked in' on the ventilator, with no useful movements

Stopping nutrition and hydration technologies: a conflict between traditional Catholic ethics and church authority.

This article focuses on the troubling effects of the secular values of individual freedom and autonomy and their impact on laws regarding suicide and euthanasia. The author argues that in an increasingly secularized culture, death and dying are losing their meaning and are not thought of within a moral framework. The debate regarding the provision of artificial nutrition and hydration is critically considered in light of the history of Catholic morality as well as within the modern healthcare context, and finally with new insight from the recent statements made by the late pope. Drane argues t

What People with Parkinson's Disease Want.

Parkinson's disease is an incurable, progressive neurodegenerative disease. This condition is complicated by the varying symptoms in individuals who differ in age of onset, symptoms, progression of disease, response to treatment and prognosis. In this paper, we focus on quality of life achieved through a combination of comprehensive health care, continuous support, and self care. Determining what people with Parkinson's disease want is like assembling multiple puzzles simultaneously. While we surmise that patient centered care, support programs, access to comprehensive health care, and relevan

"The death rattle" in the intensive care unit after withdrawal of mechanical ventilation in neurological patients.

The noise produced by oscillatory movements of secretions in the oropharynx, hypopharynx, and trachea during inspiration and expiration in unconscious terminal patients is often described as "the death rattle." The secretions are produced by the salivary glands and bronchial mucosa. These patients are usually too weak to expectorate or swallow the migrating secretions. Sputum usually only accumulates in these areas if there is a significant impairment of the cough reflex, as in deep coma or near death. Reported incidence of death rattle in terminal patients varied between 6 and 92%. Death ratt

Life support in hemodialysis emergencies-treatment standard.

Hemodialysis removes uremic toxins and excess water and sodium from the blood of patients with kidney failure. For the last 60 years, this technique has been extensively improved in order to optimize patient survival and minimize adverse events. Indeed, life-threatening complications related to hemodialysis have become very rare with the use of modern machines and equipment. These hemodialysis-related serious events that became less frequent include hemolysis, air embolism, dialyzer reactions and dialysis disequilibrium syndrome. On the other hand, other medical emergencies are regularly encou

CHILDREN WITH MULTIPLE CONGENITAL DEFECTS: WHAT ARE THE LIMITS BETWEEN THERAPEUTIC OBSTINACY AND THE TREATMENT OF UNCERTAIN BENEFIT?

OBJECTIVE: Therapeutic approach of children with multiple malformations poses many dilemmas, making it difficult to build a line between the treatment of uncertain benefit and therapeutic obstinacy. The aim of this paper was to highlight possible sources of uncertainty in the decision-making process, for this group of children. CASE DESCRIPTION: An 11-month-old boy, born with multiple birth defects and abandoned by his parents, has never been discharged home. He has complex congenital heart disease, main left bronchus stenosis and imperforate anus. He is under technological support and has go

ILCOR Pediatric Life Support 2025: What is New and Why it Matters?

The 2025 International Liaison Committee on Resuscitation (ILCOR) Pediatric Life Support (PLS) update introduces ten significant changes aimed at improving the timeliness, quality, and physiologic precision of pediatric resuscitation. Based on the review of 39 PICOST questions, the update highlights early cardiopulmonary resuscitation (CPR) for bradycardia with poor perfusion, renewed emphasis on pediatric early warning systems (PEWS) for early deterioration detection, and reaffirmation of bag-mask ventilation as the preferred initial airway technique. Both ABC and CAB sequences are now accept
