Elective one-stage abdominal operations after percutaneous catheter drainage of pyogenic liver abscess.

During the past 10 years, 15 patients have had percutaneous catheter drainage (PCD) of pyogenic liver abscesses (PLA) at a major teaching hospital. Five PLA were related to biliary tract disease, two were secondary to colonic diverticulitis, two developed after abdominal surgery, and the remaining were associated with hepatic trauma, gastric ulcer, Crohn's ileitis, and colon cancer. Two abscesses were cryptogenic. Mean diameter of PLA was 8 cm and ranged from 2-14 cm. Three patients had multiple PLA. All patients were initially treated by PCD without major complications. However, one patient r

Biochemical and biological characterizations and ribotyping of Actinomyces pyogenes and Actinomyces pyogenes-like organisms from liver abscesses in cattle.

Actinomyces pyogenes is the second most frequently encountered pathogen, next only to Fusobacterium necrophorum, in liver abscesses of feedlot cattle. Ninety-one isolates, presumptively identified as A. pyogenes, isolated from liver abscesses of cattle were studied. Biochemical characteristics determined by the API 20 Strep kit were similar to those reported previously for A. pyogenes isolated from other infections, except that 18% of isolates hydrolyzed esculin. Nine isolates that resembled A. pyogenes in morphology and in certain biochemical characteristics, but fermented mannitol and/or raf

Droplet Transmission of Streptococcus sanguinis Causing Pyogenic Liver Abscess in a Male Patient With a Transplanted Liver.

Streptococcus sanguinis is abundantly present in the oral cavity and colonizes the enamel as a natural part of the microbiome. Through the formation of a healthy biofilm, this bacterium provides a protective role. However, under certain conditions, it has the ability to become pathogenic. A pyogenic liver abscess (PLA) occurs through the contiguous spread of infectious pathogens from neighboring structures or following a trauma. The isolation of S. sanguinis as the pathogen in liver abscesses has only been reported twice. Here, we report the third case of PLA caused by S. sanguinis occurring i

Does Liver Resection Remain a Viable Option in Patients With Pyogenic Liver Abscess? A Single-Center Experience.

Pyogenic liver abscesses (PLAs) are relatively rare but often fatal if left untreated. Antibiotic therapy combined with percutaneous procedures has replaced surgery as the cornerstone of treatment. However, open surgical drainage or liver resection may be a last resort. This study aimed to review our experience in treating PLA, with a focus on the conditions requiring partial liver resection as the last viable curative option. Medical records of patients with PLA admitted to Jordan Hospital between October 2014 through October 2020 were retrospectively reviewed. Medical and demographic data of

A liver abscess deprived a healthy adult of eyesight: endogenous endophthalmitis associated with a pyogenic liver abscess caused by serotype K1 Klebsiella pneumonia.

Klebsiella pneumonia usually causes urinary tract infections, pneumonia, and other infectious diseases in hospitalized and immunocompromised patients. Among the types of Klebsiella pneumonia, serotype K1 is known to be a highly virulent pathogen. We herein report the case of a healthy 63-year-old man with a pyogenic liver abscess and bilateral endogenous endophthalmitis caused by serotype K1 Klebsiella pneumonia. Although the patient received percutaneous abscess drainage and antibiotic therapy, he lost his eyesight. To improve the poor prognoses of ocular complications, providing both an earl

Optimal timing of pyogenic liver abscess evacuation: The role of early ultrasound-guided intervention.

In this editorial, we comment on the article published by Qiu et al. Pyogenic liver abscess is a serious clinical condition requiring timely and effective intervention. Ultrasound (US)-guided techniques - whether needle aspiration (NA) or catheter drainage - are key minimally invasive treatments, especially in patients with multiple or deep-seated abscesses where conventional surgery is often impractical. The timing and choice of evacuation method significantly influence clinical outcomes. Although catheter drainage may be necessary for larger or refractory collections, NA represents a less in

Serum heparin binding protein as a biomarker for sepsis in pyogenic liver abscess patients.

BACKGROUND: Pyogenic liver abscess (PLA) is a severe infectious condition characterized by hepatic parenchymal pus accumulation, posing diagnostic and management challenges despite medical advances. Sepsis ensuing from PLA signifies a critical clinical scenario with notable morbidity and mortality implications. METHODS: A retrospective analysis encompassing 202 PLA patients over five years discerned 79 with sepsis (PLAS group) and 123 without sepsis (PLA group). Baseline characteristics, pathogen distribution, and serum heparin-binding protein (HBP) levels were assessed, alongside statistical

Panophthalmitis complicated with asymptomatic pyogenic liver abscess caused by Klebsiella pneumoniae: A case report.

RATIONALE: Panophthalmitis is a severe, vision-threatening condition that may result from endogenous spread of infection, often presenting with systemic symptoms. Klebsiella pneumoniae (Kp) is a known cause of endogenous panophthalmitis, typically associated with underlying conditions such as diabetes and often linked to pyogenic liver abscesses. However, cases presenting solely with ocular symptoms are exceedingly rare. This report presents a unique case of Kp panophthalmitis with a concurrent asymptomatic liver abscess, emphasizing the importance of systemic evaluation in atypical ocular inf

Pyogenic liver abscess secondary to gastric perforation of an ingested toothpick: A case report.

BACKGROUND: Liver abscess due to foreign body-induced gastrointestinal tract perforation is a rare event that could be misdiagnosed due to low suspicion. Less than 100 cases have been reported to date. CASE SUMMARY: We report a case of a 53-year old female patient with pyogenic liver abscess secondary to ingestion of a toothpick with penetration through the lesser curvature of the stomach. The patient presented with persistent epigastric pain. Abdominal computed tomography demonstrated the presence of a linear radiopaque object associated with abscess formation in the left liver lobe. Molecular epidemiology and virulence factors of pyogenic liver abscess causing Klebsiella pneumoniae in China.

The molecular epidemiology and prevalence of virulence factors of isolates from patients with Klebsiella pneumoniae liver abscess (KLA) in mainland China are unknown. Klebsiella pneumoniae isolates were obtained from drainage samples aseptically collected from patients with pyogenic liver abscess (PLA). The genetic similarity of KLA isolates was analyzed by pulsed-field gel electrophoresis. The hypermucoviscosity (HV) phenotype was identified by a positive string test. The K1 and K2 genotypes, the pLVPK-derived genetic loci, aerobactin gene, kfu and alls were detected by PCR amplification. The

Diabetes with pyogenic liver abscess--A perspective on tongue assessment in traditional Chinese medicine.

BACKGROUND: This novel study provides a time series analysis of tongue features extracted from a diabetic patient with pyogenic liver abscess (PLA), treated with the integration of western medicine and traditional Chinese medicine (TCM). The features, namely, tongue color, tongue fur thickness and fur color, identified from a series of tongue images taken every two days, exhibit significant transitions matching closely with the progression of disease. These tongue features could serve as effective, non-intrusive indices for different progression stages of diabetes with PLA. CASE PRESENTATION:

Unusual Presentation of Pyogenic Liver Abscess in a Healthy Young Pilot: A Case Report.

Pyogenic liver abscess is a very rare, yet serious, condition, usually accompanied by different risk factors. In this article, we present the case of pyogenic liver abscess in an otherwise healthy male patient. What makes our patient's case unique is the fact that he does not have any past medical history and initially had very vague symptoms that were not very consistent with a diagnosis of a liver lesion, hence the delay in further testing and the conclusion of liver abscess diagnosis. Treatment includes draining of the abscesses, as well as a course of antibiotics. It is important to perfor
