[Analysing the Need for Long-Term Care: Potential of Data from Long-Term Care Assessments of the Bavarian Medical Service for Public Health Research and Practice].

BACKGROUND: Despite demographic changes, there is still no systematic and comparable differentiation of nursing care reporting on a small-scale level in Germany, where outpatient long-term care is depicted. This article presents findings of care assessment data of the Medical Service of Bavaria and draws conclusions for future reporting on nursing. METHODS: For the analysis, anonymised initial long-term care assessments of the Bavarian Medical Service of 2019 were evaluated exemplarily using descriptive methods. The study describes the characteristics of persons with a care level recommendati

Resident care needs and work stressors in special care units versus non-specialized long-term care units.

Differences in how elderly residents' care needs affect staff's experiences of work stressors between special care units (SCUs) for dementia and psychiatric residents and non-SCUs were investigated. The data were drawn from 390 staff members in 38 long-term care SCUs, and 587 staff in 53 non-SCUs in Finland. Residents' care needs were based on the Resident Assessment Instrument (RAI) system measured by the Minimum Data Set 2.0. Work stressors (time-pressure and role-conflicts) were assessed with a staff survey questionnaire. Multiple-group regression analysis showed that residents' dependency

Nurses' Experiences of Caring for Children on Long-Term Mechanical Ventilation.

BACKGROUND: The number of children with long-term home mechanical ventilation (LTV) is increasing globally. Parents become experts on their children's needs and sometimes know more about LTV treatment than non-specialist healthcare professionals. Nurses caring for LTV-dependent children in intensive care settings adjust to a collaborative caregiving role; however, this interaction has not been described in general paediatric wards. AIM: To describe nurses' experiences of caring for children and adolescents with established LTV at general paediatric wards. STUDY DESIGN: We conducted individua

The etiology of delirium in long-term care settings: a Lernaean Hydra.

PURPOSE: Delirium is a frequent and critical condition in long-term care (LTC) settings. The Diagnostic and Statistical Manual of Mental Disorders (DSM-5) states that delirium requires three key elements: acute onset, fluctuating course, and disturbances in attention and awareness. As the Hydra, a mythological monster with multiple heads that doubles when cut down, delirium is multifactorial in nature. Understanding the multicomponents of delirium is decisive for effective prevention, early detection, and management. METHOD: This paper aimed to outline etiologies of delirium in LTC settings.

Characteristics of patients with cancer in European long-term care facilities.

PURPOSE: Up to 26% of residents in nursing homes (NHs) are affected by cancer. Their care represents a challenge, because NHs are not usually considered a setting focused on oncologic management and care. The aim of this paper is to describe socio-demographic and clinical features of patients with cancer residing in European NHs. METHODS: Cross-sectional study based on data from the Services and Health for Elderly in Long TERm care (SHELTER) study. Participants were assessed through the interRAI-LTCF, which includes cancer assessment. RESULTS: Among 4140 participants (mean age 83.4 years; fe

Needs of people with dementia in long-term care: a systematic review.

With the aging of the population and the projected increase of dementia in the coming years, it is crucial that we understand the needs of people with dementia (PWD) in order to provide appropriate care. The aim of this study is to determine, using the best evidence possible, the care needs of PWD living in long-term care (LTC). A total of 68 studies, published between January 2000 and September 2010, were identified from six databases. From the selected studies, 19 needs of PWD were identified. The existing evidence suggests that psychosocial needs such as the need to engage in daily individu

Validating the wheelchair outcome measure for residents in long-term care.

PURPOSE: To determine the retest reliability and construct validity of the self-report and proxy versions of the wheelchair outcome measure (WhOM) among residents in long-term care. METHOD: The WhOM was administered to 55 wheelchair users living in residential care (38 self-respondents and 17 proxy respondents). Retest data were collected to estimate reliability using intraclass correlation coefficients (ICCs) for each of the tool scores (Importance, Satisfaction and Importance × Satisfaction). For construct validity testing, Spearman's correlation coefficients were used to assess whether WhO

Thrombotic risk and immobility in residents of long-term care facilities.

BACKGROUND: Hospitalized patients and residents of long-term care (LTC) facilities account for about 60% of all cases of venous thromboembolism (VTE), as incidence is correlated with increasing age, immobility, and underlying medical conditions. The primary aim of the study was to develop an evidence-based VTE risk stratification tool and definition of immobility for residents in LTC facilities. METHODS: Using the Delphi process, a panel of vascular thrombotic and geriatric experts reviewed and ranked statements of VTE risk and immobility derived from randomized controlled trials, meta-analys

A prospective study of long-term care institutionalization among the aged.

A statewide probability sample of 1,625 elders living in Massachusetts are studied prospectively to identify key determinants of long-term care (LTC) institutionalization. One-hundred forty-seven elders, 9 per cent of the original cohort, entered a LTC institution during the six-year investigation. Using logistic multiple regression, we examine the predictive power of 19 independent variables grouped into six categories: demographic characteristics, attitude, social context, long-term care needs, physical disability, and mental/emotional disability. Five variables are significantly related to
