Clinical and applied thrombosis/hemostasis : official journal of the International Academy of Clinical and Applied Thrombosis/Hemostasis2021eng
The purpose of this study was to identify patients at higher risk of deep venous thrombosis (DVT) in the uninjured lower extremity both preoperatively and postoperatively in patients with lower extremity fractures. We collected the clinical data of patients with lower extremities fractures who presented at Xi'an Honghui Hospital between 1 July, 2015 and 31 October, 2017. Doppler ultrasonography was used to diagnose the DVT. Patients were examined pre- and postoperatively. The patients were divided into thrombosis group and no thrombosis group according to the preoperative and postoperative ult

Background: Lower extremity swelling accompanies many conditions, including gynecological surgery and chronic venous dysfunction. Some clinical and quantitative assessment methods exist, but other ways to rapidly assess early lymphedema and track changes are needed. Our goal was to use lower to upper extremity intraside tissue dielectric constant (TDC) ratios with the specific aim of providing normal reference values potentially useful as a comparison measure against which persons at risk for lower extremity lymphedema might be evaluated. Methods and Results: TDC ratios (calf/forearm, foot/for

Increased pulse pressure reflects pathologic arterial stiffening and predicts cardiovascular events and mortality. The effect of pulse pressure on outcomes in lower extremity bypass patients remains unknown. We thus investigated whether preoperative pulse pressure could predict amputation-free survival in patients undergoing lower extremity bypass for atherosclerotic occlusive disease. An institutional database identified 240 included patients undergoing lower extremity bypass from 2005 to 2014. Preoperative demographics, cardiovascular risk factors, operative factors, and systolic and diastol

Juvenile dermatomyositis (JDM) is a rare but complex and potentially life-threatening autoimmune disease of childhood. Significant proportions of patients have residual weakness, muscle atrophy, joint contractures, and calcinosis. Recently, new clinical findings, such as lipodystrophy accompanied with increased fat deposition in certain areas, have been reported. So far, it is not known whether the redistribution of body fat may be the type of lipedema of lower extremity. We describe a 39-year-old woman who was diagnosed with JDM at the age of 7. Later she developed symmetrical lipodystrophy o

There has been a paradigm shift in the management of lower extremity injury from amputations to more complex limb salvage techniques. Despite advancements in orthopedic surgery, trauma surgery, plastic surgery, and vascular surgery, not every patient is a candidate for complex lower extremity limb salvage. For this reason, novel advancements in plastic and reconstructive surgery have targeted not only limb salvage but also functional amputations. Here we review the history and advancements in microsurgical limb salvage and lower extremity amputation to discern how today's surgeon can better ai

Lower extremity (LE) reconstruction presents unique challenges due to complex wound environments, compromised vascularity, and high functional demands. Compared with other anatomic regions, free flap reconstruction in the LE demonstrates higher complication and failure rates. Early microsurgical intervention improves outcomes, though negative pressure wound therapy can bridge to delayed coverage. In oncologic patients, radiation and chemotherapy impair tissue quality and recipient vessels, necessitating meticulous planning and vessel selection outside the zone of injury. Anatomic location infl

BACKGROUND: Unhoused patients are a group at high risk for lower extremity amputation because of higher rates of cardiovascular disease prevalence, higher rates of foot infection treatment failure, and poorer diabetes control. The aim of this study is to identify any disparities in preoperative optimization or postoperative outcomes following lower extremity amputation between housed and unhoused patients, with the intention of modifying patient management and enhancing long-term outcomes in this high-risk population.
METHODS: This is a single institution, retrospective analysis of 1,304 pati

INTRODUCTION: Patients undergoing revascularization of the lower extremities have unacceptably high rates of major adverse cardiac and limb events despite the routine use of antiplatelet therapy. Optimization of antithrombotic therapy provides an opportunity to reduce this risk. Recent large, randomized trials have demonstrated substantial benefit from the combination of low-dose rivaroxaban and aspirin compared with aspirin alone. Despite this new evidence, uptake remains limited.
AREAS COVERED: This review will outline the drug profile of rivaroxaban, summarize the key efficacy and safety d

BACKGROUND: Because of the rarity of dysplasia epiphysealis hemimelica (DEH), little is known about the relationship between disease classification and clinical symptoms or patient outcomes. This studies therefore aims to characterize DEH of the lower extremity and correlate radiographic classification to presenting symptomatology and need for surgical intervention.
METHODS: A multi-center, retrospective review of all patients with DEH of the lower extremity over a 47-year period was conducted. Demographic data, presenting complaints, treatments, and symptoms at final follow-up were recorded.

The senior author first coined the "orthoplastic" approach to traumatic lower extremity reconstruction, by which multidisciplinary surgeons and specialists work together for optimal patient success. The goals of lower extremity salvage are to optimize limb appearance, restore unrestricted pain-free ambulation, and improve quality of life. Composite traumatic defects require an organized approach, and the reconstructive ladder is used for strategies of varying complexity for repair of soft-tissue wounds. The lower rungs of the ladder include simpler reconstructive options such as the use of ski

Baseball is unique in its multiple facets: pitching, hitting, base rounding, and fielding are distinct activities that require different athletic skills to perform at a high level. Likewise, these different aspects of the game can contribute to a multitude of varying injuries. While high-velocity overhead throwing, along with batting, can produce a plethora of upper extremity injuries that often garner attention, injuries to the lower extremity can severely impact a player's performance and ability to compete. The rigors of the short, explosive sprinting required for base running, as well as t

Indications for lower extremity reconstruction in children are unique because most result from congenital conditions (eg, constriction ring, lymphedema, syndactyly, nevi, vascular anomalies). Like adults, pediatric patients also suffer from effects following extirpation and trauma. Principles of reconstruction are based on the condition and type of deformity. The pediatric population typically has fewer comorbidities than adults that can negatively affect outcomes (eg, diabetes, peripheral vascular disease), although children can be less compliant with postoperative care. Growth, development,

Both lower extremities and lower back are common sources of injury for cyclists. For providers to optimize care within this area of sports medicine, they need to understand the most common sources of injury in this population. Cycling presents a unique challenge: treating both the athlete and the complex relationship between rider and bicycle. Physicians should not replace the role of a professional bike fitter and should view these individuals as integral members of the team to alleviate current and prevent future injury. This article explores common lower extremity and lumbar back overuse in

BACKGROUND: The ability to ambulate following major lower extremity amputation, either below (BKA) or above knee (AKA), is a major concern for all prospective patients. This study analyzed ambulatory rates and risk factors for nonambulation in patients undergoing a major lower extremity amputation.
METHODS: A retrospective review of 811 patients who underwent BKA or AKA at our institution between January 2009 and December 2014 was conducted. Demographic information and co-morbid conditions, including the patients' functional status prior to surgery, at 6 months, and at latest follow up were r

Lower extremity arterial disease (LEAD) affects over 200 million people worldwide but, as of today, is not adequately treated and represents an important cause of morbidity, mortality and disability. Although the use of antithrombotic therapy is recommended to prevent cardiovascular adverse events, patients with LEAD often receive inadequate prescription for antithrombotic agents (antiplatelets and anticoagulants). Historically, the inadequate use of antithrombotic drugs in this clinical setting has been ascribed to the lack of high quality scientific data obtained from clinical trials enrolli

Lower extremity peripheral artery disease (PAD) refers to atherosclerotic disease that involves the iliac, femoral, or more distal arteries of the lower extremities. This condition affects 8 to 12 million Americans. Risk factors include advanced age, hypertension, dyslipidemia, diabetes, and cigarette smoking. Approximately 10% to 30% of patients with PAD present with the classic symptom of intermittent claudication. Some patients experience symptoms such as pallor, hair loss, or nonhealing wounds, and up to half of patients are asymptomatic. There are differing recommendations from various or

OBJECTIVE: Lower extremity venous aneurysms may lead to serious morbidity in patients, including pulmonary embolism (PE) and chronic venous insufficiency. Presently, because of the low incidence of these aneurysms, no consensus for their treatment exists. The purpose of this study was to review the presentation and management of lower extremity venous aneurysms at our institution.
METHODS: A retrospective review of all patients with isolated lower extremity venous aneurysms treated at a single tertiary care medical center from 2005 to 2017 was conducted.
RESULTS: Five male and six female pat
