Perinatal outcomes associated with magnesium sulfate for preeclampsia without severe features.

OBJECTIVE: To evaluate neonatal outcomes in patients with preeclampsia without severe features who received magnesium for seizure prophylaxis. METHODS: Retrospective cohort study of patients with preeclampsia without severe features delivering ≥32 weeks' gestation at our institution (November 2021-May 2023). The primary outcome was a composite of adverse neonatal outcomes (neonatal intensive care unit [NICU] admission, umbilical artery pH < 7.1, umbilical artery base deficit ≥12, 5-min Apgar <3, neonatal death, culture proven sepsis, and mechanical ventilation). Secondary outcomes included th

Magnesium sulfate in severe perinatal asphyxia: a randomized, placebo-controlled trial.

OBJECTIVE: The goal was to study whether postnatal magnesium sulfate infusion could improve neurologic outcomes at discharge for term neonates with severe perinatal asphyxia. METHODS: Forty term (> or =37 weeks of gestation) neonates with severe perinatal asphyxia were studied in a prospective, longitudinal, placebo-controlled trial. Patients were assigned randomly to receive either 3 doses of magnesium sulfate infusion at 250 mg/kg per dose (1 mL/kg per dose) 24 hours apart (treatment group) or 3 doses of normal saline infusion (1 mL/kg per dose) 24 hours apart (placebo group). Both groups a

Magnesium sulfate treatment alters fetal cerebellar gene expression responses to hypoxia.

Prenatal perturbation of brain circulation and oxygenation is a leading cause of perinatal brain damage affecting about 0.3-0.9% of births. Hypoxia-ischemia (HI) in preterm human infants at gestational week 23-32 results in neurodevelopmental abnormalities in childhood, presenting as learning disability, seizure activity, motor impairment and in the most severe cases, death. Here, we examined the potential of MgSO4 treatment, prior to foetal hypoxia, to attenuate hypoxia induced damage in a murine model of maternal hypoxia. We studied the time course of maternal hypoxia and MgSO4 pre-treatment

Asymptomatic paroxysmal atrial fibrillation during intravenous magnesium sulfate treatment in preeclampsia.

Magnesium sulfate is commonly used as the anticonvulsant drug of choice for preeclampsia and eclampsia. It is effective, nontoxic to both mother and fetus, and side effects are rare within the therapeutic range. This report describes paroxysmal atrial fibrillation occurring at low serum levels of the drug during the treatment of preeclampsia.

Magnesium sulfate versus phenytoin for seizure prophylaxis in pregnancy-induced hypertension.

Seizure prophylaxis is standard intrapartum therapy for patients with pregnancy-induced hypertension. Magnesium sulfate is used in the United States in spite of limited literature comparing its efficacy with other anticonvulsants. Fifty patients with pregnancy-induced hypertension were prospectively randomized to receive magnesium sulfate or phenytoin for seizure prophylaxis. Patients were observed for toxicity, side effects, and labor outcomes, and the neonates were evaluated for side effects of the therapy. Three patients were excluded with adverse reactions to medications (one in magnesium

[Anti-arrhythmic effects of intravenous magnesium sulfate in paroxysmal supraventricular tachycardia].

The anti-arrhythmic effects of intravenous magnesium sulfate, on bouts of supraventricular tachycardia (SVT) secondary to reentry phenomenon, are evaluated in twelve patients undergoing an electrophysiological testing, because of paroxysmal SVT, the pathway of which is an intranodal reentry (eight patients) or includes an atrio-ventricular accessory route (orthodromic SVT: four patients). At the completion of the basic testing, a stable SVT is induced and an intravenous bolus of 3 grams of magnesium sulfate is administered in three minutes. The length of the SVT cycle is significantly increase

Effect of maternal magnesium sulfate treatment on fetal heart rate variability.

The effect of maternal magnesium sulfate treatment on fetal heart rate variability (FHRV) was evaluated by comparing the maternal serum magnesium level drawn during labor to the degree of FHRV seen concurrently on fetal rate heart monitor strips. The records of 57 patients treated with intravenous magnesium sulfate for hypertensive disorders of pregnancy were studied. Fetal heart rate variability was measured by both subjective assessment and objective measurement. The difference in beats/min between the average high and low fetal heart rates, at the time the magnesium level obtained, was used

Magnesium sulfate may unmask subclinical excess vasopressinase-induced arginine vasopressin disorder.

CONTEXT: Gestational arginine vasopressin (AVP) disorder is classified into 3 categories: AVP deficiency (AVP-D), AVP resistance, and excess vasopressinase-induced AVP disorder. This is the first case report of gestational AVP disorder linked with magnesium sulfate administration. CASE DESCRIPTION: A 26-year-old woman pregnant with twins was admitted to our hospital with threatened preterm labor (TPL). The day after continuous intravenous magnesium sulfate administration for TPL was initiated, the patient suddenly developed thirst, polydipsia, and polyuria. Because the plasma AVP level was ex

Magnesium Sulfate in Pediatric Emergency Medicine: Pharmacology, Evidence, and Clinical Applications.

Magnesium sulfate (MgSO₄) is a well-established therapeutic agent with extensive historical use, predominantly recognized in obstetrics. However, its critical role in pediatric emergency care remains underrecognized despite substantial clinical evidence supporting its efficacy in severe asthma exacerbations, refractory seizures, and life-threatening cardiac arrhythmias. This review aims to consolidate and critically appraise the current evidence regarding MgSO₄ pharmacologic mechanisms, clinical applications, and therapeutic impact across major pediatric emergencies, emphasizing the potential

Postpartum Eclampsia Unresponsive to Magnesium Sulfate Therapy: An Uncommon Case Report.

Despite advances in obstetric care, postpartum eclampsia continues to be a major cause of morbidity and mortality among mothers. Although the most common treatment for eclampsia during the postpartum phase is magnesium sulfate (MgSO4), a small percentage of individuals show resistance to this approach. Clinical challenges arise from MgSO4 resistance, which calls for different approaches to care to avoid unfavorable consequences for mothers. In this case study, we provide a thorough clinical description of a 19-year-old primigravida who gave birth at 37 weeks and developed postpartum eclampsia

Interrupted versus continuous magnesium sulfate and blood loss at cesarean delivery.

OBJECTIVE: Magnesium sulfate is standard of care for prevention of eclampsia in women with preeclampsia with severe features. The American College of Obstetrics and Gynecology endorses its use throughout labor, delivery and the immediate postpartum period. Some providers pause magnesium sulfate infusion preoperatively due to concern for increased risk of uterine atony and postpartum hemorrhage. Using a non-inferiority analysis, we investigated the effect of interrupted versus continuous infusion of magnesium sulfate on postpartum hemorrhage in women with preeclampsia with severe features under

Breast engorgement and galactorrhea during magnesium sulfate treatment of preterm labor.

Breast engorgement and galactorrhea occasionally occur during tocolysis with ritodrine. We are not aware of breast engorgement and galactorrhea associated with other tocolytics. We report the first case of breast engorgement and galactorrhea during tocolysis with intravenous magnesium sulfate in a generally healthy 24-year-old woman admitted for tocolysis at 30 weeks' gestation. Breast engorgement and galactorrhea gradually subsided after magnesium sulfate was discontinued. The rapid disappearance of both the galactorrhea and the breast engorgement after discontinuation of magnesium sulfate tr

Severe hyperkalemia during magnesium sulfate therapy in two pregnant drug abusers.

Severe hyperkalemia developed in two pregnant intravenous drug abusers during prolonged parenteral magnesium sulfate therapy in the absence of any obvious cause. The serum potassium concentration returned to normal after magnesium therapy was discontinued. Hypermagnesemia, acting through several possible mechanisms, was probably responsible for the reversible hyperkalemia observed.

Iatrogenic Hypermagnesemia in a Patient With Preeclampsia Caused by Misinterpretation of the Magnesium Reporting Unit Following Magnesium Sulfate Administration.

Because of the lack of standardization, different laboratories report plasma magnesium sulfate or magnesium level in different units, leading to errors in diagnosis and case management; especially when a patient's workup was done in a hospital and then transferred to another hospital. Failure to recognize or understand the reporting units can confuse clinicians and healthcare workers, leading to erroneous interpretations and, consequently, to misdiagnosis of hypermagnesemia or hypomagnesemia. In this report, we present a 32-year-old female patient at 31 weeks of gestation with a history of a m

Pharmacokinetics and Pharmacodynamics of Intravenous Magnesium Sulfate in Pediatric Acute Asthma Exacerbations.

Pediatric asthma exacerbations represent a significant cause of emergency department use and hospitalizations. Despite available treatment options, many children's exacerbations are refractory to standard therapies and require adjunct treatments. The Intravenous Magnesium: Prompt use for Asthma in Children Treated in the Emergency Department study investigated the pharmacology of intravenous magnesium sulfate (IVMg) in treating pediatric asthma exacerbations. Specifically, the objectives of the study included (1) externally validating a previously published population pharmacokinetic model and
