Anatomical and morphometrical approaches on the auditory ossicles in European ground squirrel (Spermophilus citellus).

INTRODUCTION: The study of the middle ear and especially the ossicular chain in different species can bring interesting insights into the biomechanics of hearing. The ground squirrel's middle ear anatomy has not been studied as thoroughly as its other systems and organs. METHODS: Our study describes the ear ossicles in this little-studied species, providing the morphological and morphometrical characteristics and a series of comparative data, in an attempt to characterise also some functional anatomy of the incus, malleus and stapes. RESULTS AND DISCUSSION: The malleus comprises the typical

Carhart notch 2-kHz bone conduction threshold dip: a nondefinitive predictor of stapes fixation in conductive hearing loss with normal tympanic membrane.

OBJECTIVE: To evaluate the significance of the Carhart notch (a 2-kHz bone conduction threshold dip [2KBD]) in the diagnosis of stapes fixation by comparing its incidence among ears with various ossicular chain abnormalities. DESIGN: Retrospective study. SETTING: University hospital. PATIENTS: A total of 153 ears among 127 consecutive patients with a congenital ossicular anomaly or otosclerosis. MAIN OUTCOME MEASURES: The 2KBD depth was defined as the threshold at 2 kHz minus the mean of thresholds at 1 and 4 kHz. The presence of 2KBD (depth, ≥10 dB), 2KBD depth, relationship between 2KBD

Floating-Harbor syndrome associated with middle ear abnormalities.

Floating-Harbor syndrome is a rare syndrome of unknown etiology, which was first described in 1973. A triad of main features characterizes Floating-Harbor syndrome: short stature, characteristic face, and an expressive speech delay. We present a patient in whom the hearing thresholds improved insufficiently after placement of grommets. High-resolution CT scan of the temporal bone showed a prominent soft-tissue thickening suspected of causing fixation of the malleus, and fusion of the malleus head with the body of the incus. To our knowledge this is the first reported abnormal middle ear anatom

Ossiculoplasty with a cartilage-connecting hydroxyapatite prosthesis for tympanosclerotic stapes fixation.

Many cases of tympanosclerotic stapes fixation are accompanied by fixation or erosion of malleus and/or incus. This status of the ossicular chain is one of the reasons that ossiculoplasty for tympanosclerotic stapes fixation is more difficult than that for otosclerosis. We conducted a retrospective review of seven patients who were operated on for tympanosclerotic stapes fixation between 2002 and 2006. All of the patients had abnormal conditions of the malleus and/or incus and underwent stapedectomy and total ossiculoplasty with hydroxyapatite prosthesis (Apaceram T-7 type), which has a planar

[An improved animal model for chronic perforation of the tympanic membrane].

BACKGROUND: An inexpensive and valid animal model of chronic tympanic membrane perforation is needed. METHOD: Twelve male rats were selected for different surgical procedures (subtotal tympanic membrane perforation with local microflaps, re-perforation without flaps, partial excision of the handle of malleus). The inhibition of spontaneous healing was accomplished by the application of prednisolon or mitomycin directly onto the tympanic membrane. RESULTS: Only by additional partial excision of the handle of malleus followed by local application of mitomycin were we able to achieve a persiste

[Recovery rate in chemotherapy of glanders].

It was shown that out of all the studied methods for the estimation of the recovery rate in animals with experimental malleus the solid-phase enzyme immunoassay with parallel growing of the test material and subsequent isolation of the antigens and L-forms proved to be the most efficient. In addition to the routine criteria of the recovery the malleinic test was a valuable diagnostic means for the estimation of the recovery rate and the efficacy of the antibacterial drugs in a model of guinea pigs. The immunological investigations revealed a dependence of the host sanation on the term of the a

CT evaluation of malformed external and middle ear and its surgical correlation.

This study evaluated the diagnostic value of high resolution CT (HRCT) for atresia of the external auditory meatus (EAM) and isolated middle ear malformation. CT scan of the temporal bone was done in 33 patients with such disorders and 5 patients with otosclerosis etc. were studied in the same way for comparison. Coronal HRCT clearly showed conditions of the atresia plate and malformation of the malleus, incus, abnormal course of facial canal and changes in the vestibular window. The axial HRCT is useful to demonstrate the articulations in between the malleus, incus, stapes and the cochlear wi

The discomallear ligament: anatomical, microscopical, and radiologic analysis.

BACKGROUND: Several anatomic relationships between the ear and the temporo-mandibular joint have been proposed to account for the presence of tinnitus during temporo-mandibular disorders. Among the otomandibular structures, the discomallear ligament (DML) is interposed between the malleus and the retrodiscal capsular complex. The aim of present paper was to study through dissection the frequency and morphology of DML, to characterize its type of collagen, and to evaluate the DML on routine computed tomography (CT). METHODS AND RESULTS: The study has been conducted on five un-embalmed adult ca

Micro-CT of the human ossicular chain: Statistical shape modeling and implications for otologic surgery.

The ossicular chain is a middle ear structure consisting of the small incus, malleus and stapes bones, which transmit tympanic membrane vibrations caused by sound to the inner ear. Despite being shown to be highly variable in shape, there are very few morphological studies of the ossicles. The objective of this study was to use a large sample of cadaveric ossicles to create a set of three-dimensional models and study their statistical variance. Thirty-three cadaveric temporal bone samples were scanned using micro-computed tomography (μCT) and segmented. Statistical shape models (SSMs) were the

Pars tensa and tympanicomalleal joint: proposal for a new anatomic classification.

PURPOSE: The tympanic membrane (TM) belongs to the ear. Despite its place in the ear anatomy, can we give it also a different anatomic classification? The main objective is to clarify the nature of TM, tympanic bone and malleus to propose a new anatomic classification. METHODS: This cadaveric study was performed in two human heads and six fresh temporal bones. A study of the temporomandibular joint, external acoustic meatus (EAM), TM and middle ear structures was conducted. A medical literature review englobing anatomy, embryology, histology and phylogeny of the ear was performed and the resu

Anatomical measurement of the ossicles in patients with congenital aural atresia and stenosis.

OBJECTIVES: Our aims were to measure and compare anatomical parameters of the ossicles in normal, congenital aural stenosis (CAS), and congenital aural atresia (CAA) ears. METHODS: This retrospective study was performed using three-dimensional reconstructed images derived from computed tomography scans of 20 normal subjects, 20 CAS patients, and 20 CAA patients. RESULTS: The lengths of the malleus handle and long process of the incus were greater in normal ears than in CAS and CAA ears (all P < 0.05). The angles of the incudostapedial joint and between the short and long processes of the inc

Goodhill syndrome: a case report.

Conductive hearing loss with a normal tympanic membrane is a common reason for otolaryngology consultation, with otospongiosis being the most frequent cause and House syndrome being extremely rare, requiring systematic investigation. We report the case of a 31-year-old woman who presented with conductive hearing loss with a normal tympanic membrane. A temporal bone computed tomography (CT) scan confirmed a House-Goodhill syndrome due to fixation of the malleus head. Surgical intervention was considered to remove the attic bone synostosis with the malleus head, resulting in a significant clinic

High-resolution CT findings in children with a normal pinna or grade I microtia and unilateral mild stenosis of the external auditory canal.

A subset of patients presents with unilateral conductive hearing loss, a normal pinna or grade I microtia, and mild external auditory canal stenosis. The physical findings of microtia and a small external canal are commonly absent or subtle in this group of patients, who are being commonly referred for imaging to evaluate isolated conductive hearing loss. We present a case series of patients with unilateral conductive hearing loss and characteristic ossicular abnormalities, commonly anterior fixation of the malleus. All patients had a significantly increased distance from the cochlear promonto

Homograft tympanic membrane myringoplasty.

During a six-year period, homograft tympanic membranes have been used successfully for myringoplasty or tympanoplasty in 85% of 175 cases. This percentage is not as high as might be expected with fascia, and the homografts have not been as dependable in the wet, draining ear. However, the results do show that the homograft is an acceptable grafting material. The eardrums are not suggested as substitutes for fascia, but should be available for difficult cases. Their use is indicated in large or total defects to restore the natural anatomical conical shape of the eardrum, particularly in congeni

A study of the vibration of the basilar membrane in human temporal bone preparations by the use of the Mössbauer effect.

Using the Mössbauer technique, we have studied the vibration of the human basilar membrane and malleus head in the sound frequency range 0.2--9.0 kHz and at a sound pressure of 100 dB at the tympanic membrane. The displacement frequency response curves for the basilar membrane and the malleus head have similar shapes, with a maximum at about 1 kHz. Below and above 1 kHz the curves have a slope of about 10 dB/octave and--100 dB/octave, respectively. In addition, the basilar membrane has a maximum displacement at a frequency dependent distance from the stapes. A simple hydrodynamic model for the

[Numerical study on the effect of middle ear malformations on energy absorbance].

In order to study the effect of middle ear malformations on energy absorbance, we constructed a mechanical model that can simulate the energy absorbance of the human ear based on our previous human ear finite element model. The validation of this model was confirmed by two sets of experimental data. Based on this model, three common types of middle ear malformations, i. e. incudostapedial joint defect, incus fixation and malleus fixation, and stapes fixation, were simulated by changing the structure and material properties of the corresponding tissue. Then, the effect of these three common typ

Reconstruction of the ossicular chain with hydroxyapatite prostheses.

To achieve a lasting result in reconstruction of the ossicular chain, strict material and surgical criteria must be combined. The best biomaterial, apart from bone, is hydroxyapatite, the mineral matrix of living bone tissue. For the reconstruction, the assembly technique is used instead of a columella. Direct contact with the drum membrane is not reliable if a columella is used, and extrusion or lateralization may influence postoperative results. In the case of a missing incus, an assembly prosthesis of dense hydroxyapatite is placed on top of the stapes head and the handle of the malleus, in

Unexplained conductive hearing loss.

We report a series of unusual clinical cases which exhibit what we have referred to as unexplained conductive hearing loss. Audiometrically, these cases typically display mild to moderate conductive or mixed-type hearing loss and good word discrimination (80% to 100%). The otologic evaluations also lend support to the audiometric findings. When polytome roentgenograms were obtained thr results showed a normal inner ear system. Most often, the otologists' clinical impression was a fixed malleus or congenital stapes fixation, and exploratory tympanotomy with inspection of the ossicular chain was

[Anatomical and functional results of middle ear reconstruction with tympano-ossicular allografts].

Twenty five patients with severely damaged middle ears underwent reconstructive surgery with using total tympano-ossicular allografts. Two years long follow-up showed average air-bone gap about 20 dB or better in 44% of patients. Materials and methods used for reconstruction in two groups (part I and II) were compared, basing on anatomical and functional results. In patients with total perforation and lack of malleus, reconstruction with the tympanic membrane allograft attached to the malleus handle and shaped incus columella revealed long-term hearing results improvement (63.6% closure of air
