Strategic camouflage treatment of skeletal Class III malocclusion (mandibular prognathism) using bone-borne rapid maxillary expansion and mandibular anterior subapical osteotomy.

This case report presents the camouflage treatment that successfully improved the facial profile of a patient with a skeletal Class III malocclusion using bone-borne rapid maxillary expansion and mandibular anterior subapical osteotomy. The patient was an 18-year-old woman with chief complaints of crooked teeth and a protruded jaw. Camouflage treatment was chosen because she rejected orthognathic surgery under general anesthesia. A hybrid type of bone-borne rapid maxillary expander with palatal mini-implants was used to correct the transverse discrepancy, and a mandibular anterior subapical os

Interferences between mandibular proximal and distal segments in orthognathic surgery for patients with asymmetric mandibular prognathism depending on different osteotomy techniques.

OBJECTIVE: The aim of this study was to evaluate, using computer simulation, the amount of interference between the proximal and distal segments of the mandible incurred by 3 different osteotomy methods. STUDY DESIGN: With the 3-dimensional facial computerized tomography data of 20 patients showing mandibular prognathism with asymmetry, simulation surgeries of conventional sagittal split ramus osteotomy (SSRO), intraoral vertical ramus osteotomy (IVRO), or short lingual osteotomy (SLO) was performed. The distal segments were moved backward (3, 6, or 9 mm) and rotated (2 degrees, 4 degrees, or

Is symphyseal split osteotomy a stable method in the treatment of transverse mandibular width discrepancies?

OBJECTIVE: Mandibular symphyseal split osteotomy is an orthognathic procedure used to correct transverse mandibular deficiencies. Limited data exist regarding the stability of transverse mandibular widening achieved with this technique. We set out to evaluate the stability of transverse mandibular widening following symphyseal split osteotomy and report outcomes when combined with genioplasty. STUDY DESIGN: This retrospective cohort study included patients who underwent symphyseal split osteotomy for mandibular transverse discrepancies between September 2021 and January 2023. The predictor va

The biomechanical effects of bimaxillary osteotomies to the patients with mandibular retraction under incisal clenching.

The purpose of this study is to investigate the biomechanical effect of bimaxillary osteotomies on patients with mandibular retraction. Mandibular retraction, as a typical maxillofacial deformity, and has great adverse effects on TMJ. Bimaxillary osteotomies are widely used to correct symptoms of mandibular retraction. It is necessary to understand the effect of surgery on temporomandibular disc (TMJ). Five patients with mandibular retraction and 10 asymptomatic subjects were involved in this study. Finite element models of preoperative, postoperative and control group were constructed based o

Piezoelectric Trapezoidal Osteotomy for the Retrieval of a Displaced Dental Implant in the Osteoporotic Mandibular Body.

A female patient in her mid-seventies was referred to our department for the removal of a displaced implant within the right posterior mandible. A trapezoidal window was made using a piezoelectric device. The implant was extracted, and the osteotomized window was replaced without fixation. Caution should be exercised during implant placement in patients with osteoporosis because of the risk of implant displacement. Displaced implants can be easily retrieved by piezoelectric trapezoidal osteotomy.

Mapping the Mandibular Lingula in Pierre Robin Sequence: A Guide to the Inverted-L Osteotomy.

BACKGROUND AND PURPOSE: The inverted-L osteotomy for mandibular distraction in Pierre Robin sequence (PRS) is a useful technique for avoiding injury to the tooth root and inferior alveolar nerve. Identification of the lingula is understudied and may decrease iatrogenic complications. This study aims to map the position of the lingula in the micrognathic mandible and compare the location of the lingula in relative normal mandible. METHODS: This is a retrospective cohort study of symptomatic PRS patients. Three-dimensional CT scans were reviewed and the relative lingula position described. RES

[Preoperative determination of the position of mandibular canal for planning sagittal ramus osteotomy of the mandible].

BACKGROUND: In the literature the incidence of permanent nerve lesions ranges from 3 to 39%. Therefore, we think that it is necessary to discuss the procedure of individual risk assessment and management. Standard imaging techniques for the preoperative planning and preparation of a sagittal split osteotomy usually include a panoramic radiograph and lateral cephalometric radiograph. If an assessment of the transversal thickness of the lower jaw and cortical substance is required, or the position of the inferior alveolar nerve needs to be determined for preoperative planning, computed tomograph

Extraoral subcondylar ramus osteotomy for correction of mandibular prognathism. The surgical technique and complications.

1178 patients with mandibular prognathism and/or asymmetry, in some cases combined with maxillary retrognathism, were treated by extraoral horizontal or oblique ramus osteotomy during the period from 1939 to 1989. The described percutaneous retromandibular approach is a simple, rapid and reliable technique which can usually be performed under local anaesthesia. Very few complications, particularly neurological, were observed. These aspects justify consideration of this technique when indications for a simple, straight set-back procedure of the mandible exist, and there is no tendency to bite-o

Radiographic changes of the temporomandibular joint after oblique sliding osteotomy of the mandibular rami.

64 patients with mandibular prognathism were operated upon with oblique sliding osteotomies of the mandibular rami. The patients were followed with cephalometric and temporomandibular joint (TMJ) radiographs for 3 years. 60% of the TMJs showed skeletal remodelling 3 years postoperatively. The condylar remodelling was significantly correlated to the degree of postoperative displacement of the condyle. The average condyle displacement postoperatively was about 3 mm inferiorly and anteriorly, but the position was almost completely back to normal during the control period, due to a combination of

Radiographic Evidence of Dental Complications after Mandibular Distraction Osteogenesis: Inverted-L versus Oblique Osteotomy.

BACKGROUND: Patients with micrognathia undergoing mandibular distraction osteogenesis (MDO) for functional and aesthetic improvement are at significant risk for dental complications. The authors investigated the association of 2 osteotomy patterns-oblique and inverted-L-with risk to developing dentition. METHODS: A senior orthodontist (H.D.N.) performed a retrospective review of dental radiographs of patients undergoing MDO with confirmed oblique or inverted-L osteotomies between 2012 and 2022. Images were assessed for evidence of missing, damaged, or displaced teeth, and proportion of affect

Two cases of parapharyngeal space tumor resected by a double split mandibular osteotomy technique.

Parapharyngeal space tumors have poor subjective symptoms and often grow until diagnosed; therefore, mandibular transection may be needed to obtain a wider field of view during surgery. However, if a median lower lip incision is performed for the mandibular transection, esthetic problems occur after surgery. Here, we report two cases of parapharyngeal space tumors that were removed with a mandibular lateral segment-osteotomy technique without median lower lip incision to avoid esthetic problems. Case 1 was a 49-year-old woman. She was aware of a right tonsillar swelling, and an imaging test re

Individual stereolithographic template-guided curved osteotomy for unilateral prominent mandibular angle: a case report.

Mandibular angle osteotomy is commonly used for prominent mandibular angle contouring. Because of difficult control of the line, shape, and amount of osteotomy, many complications such as asymmetry, undercorrection, overcorrection, and formation of a second mandibular angle after surgery occurred commonly. In addition, it is more difficult to implement osteotomy exactly the same as preoperative design in curved osteotomy owing to the arc-shaped osteotomy line. Therefore, further studies are needed to explore ways to make osteotomy accurately identical with preoperative design. In this report,
