Isolated unilateral upper alveolar numbness in silent sinus syndrome.

We present a case of a 59-year-old man with left upper alveolar numbness of 2 years' duration in the absence of sinonasal symptoms. On physical examination, he demonstrated mild left facial asymmetry and diminished sensation of his left upper alveolus from the left second upper incisor to first canine. CT imaging revealed chronic sinusitis changes of the left maxillary sinus, with reduced volume and depressed anterior wall. The patient underwent functional endoscopic sinus surgery to re-establish maxillary sinus ventilation. He was noted to have some improvement of his upper alveolar paraesthe

Implication of the inferolateral trunk of the cavernous internal CAROTID artery in cranial nerve blood supply: Anatomical study and review of the literature.

The inferolateral trunk (ILT) is one of the two more common branches of the cavernous internal carotid artery (ICA). Its knowledge is important for skull base surgery and endovascular interventional procedures. The ILT is described with superior, anterior and posterior branch, which is the complete form. These branches vascularize the oculomotor, trochlear, trigeminal and abducens nerves into the cavernous sinus and superior orbital fissure (SOF) courses, and through the foramens rotundum and ovale. We performed 21 injected embalmed cadaveric dissections combined with six specimen tomodensitom

Primary diffuse large B-cell lymphoma of the lacrimal sac with involvement of maxillary sinus: a case report and review of literature.

Primary lacrimal sac lymphoma is a rare condition, often presenting with nasolacrimal duct obstruction. Herein, we present a unique case of diffuse large B-cell lymphoma (DLBCL) involving the lacrimal sac, maxillary sinus, and infraorbital nerve. Prompt diagnosis via biopsy is essential for timely treatment and the prevention of tumor progression. A 66-year-old female presented with intractable epiphora, infraorbital nerve hypesthesia, and medial canthal swelling. Imaging revealed a soft tissue mass in the right maxillary sinus extending into the right inferior orbit and nasal cavity. A biopsy

[Oral manifestations of zona. Apropos of a case].

Oral manifestations of herpes zoster are very less common than cutaneous. Only a few cases of oral herpes zoster in children had been already described. Authors report a case of maxillary superior nerve's herpes zoster. Oral lesions are encountered in case of viral disease of the second and third branch of trigeminal nerve (VII, VIII). Dental pain is usually the first sign and can induce misdiagnosis. The diagnosis is based on the specific ulcerative lesions, strictly unilateral, developed in the field of sensitive innervation of the maxillary nerves. The use of antiviral drugs seems to be act

Post-COVID-19 Unilateral Upper Lip Numbness: A Case Report.

This is a case of a 63-year-old female with post-COVID-19 unilateral upper lip pain and numbness. Neurologic examination did not reveal any deficits other than deficits on pinprick in the maxillary division (V2) of the left trigeminal nerve. Brain neuroimaging showed signs of acute inflammation of the left maxillary sinus. Neuropraxia of the infraorbital nerve, a branch of the trigeminal nerve, was the diagnosis considered. Reports on trigeminal neurosensory changes following acute sinusitis are few, and isolated trigeminal neuropathy is rare except in cases of dental disorders. Up to this wri

Maxillary sinus barotrauma--case report and review.

A case is presented in which a mucous retention cyst that obliterated the right maxillary sinus caused symptoms due to pressure on two separate branches of the second branch of the fifth cranial nerve during a chamber dive to 112 feet. The symptoms of pain and numbness occurred at different times during and after the dive. Referred pain to the maxillary teeth was due to pressure on the posterior superior alveolar branch and paresthesia with numbness and tingling of the lip and cheek was caused from pressure on the infraorbital nerve prior to its emergence through the infraorbital foramen. Communication of infraorbital nerve and facial nerve: anatomic and histologic study.

The maxillary nerve, second division of the trigeminal nerve, is entirely sensory. It has been reported that drooling may occur later in the event of fracture of the zygoma in which hypesthesia prevails. The aim of the study is to elucidate additional detailed anatomy of the infraorbital plexus, consisting of the superior labial branch of the infraorbital nerve and facial nerve in the cheek. The authors dissected infraorbital nerves and facial nerves in 16 cadavers. Most terminals of the zygomatic branch of the facial nerve emerged from under the levator labii superiors and zygomatic muscle an

Somatotopic organization of trigeminal ganglion: three-dimensional reconstruction of three divisions.

Clearing the somatotopic organization of trigeminal ganglion can help us to improve the precision of treatment for trigeminal neuralgia. The distribution of primary afferent perikarya of 3 branches of trigeminal nerve in the trigeminal ganglion was investigated in the rabbit, and 3D model was reconstructed then. After application of wheat germ agglutinin-horseradish peroxidase and DiI to the cut endings of the 3 branches of trigeminal nerve, ophthalmic cells were found in the anteromedial part of the trigeminal ganglion, mandibular cells in the posterolateral part, and maxillary cells in the m

Endoscopic Endonasal Approach to the Lateral Wall of the Cavernous Sinus: A Cadaveric Feasibility Study.

OBJECTIVE:: A transcranial extradural approach to the middle cranial fossa (MCF) requires separation of the dural layers of the lateral wall of the cavernous sinus. The authors tested the feasibility of an endonasal approach for this separation. METHODS:: A cadaveric feasibility study was conducted on the sides of 14 dry skulls and 10 fresh cadaveric heads. An endonasal, transsphenoidal, transpterygoid approach was taken to the MCF. The maxillary struts and medial greater wing of the sphenoid below the superior orbital fissure were drilled with transposition of the maxillary nerve. The latera

Anatomical study of trigeminal-facial nerve communications: Application to facial transplant surgery.

Understanding the distribution of nerves in the superficial layer of the face and the communication (nerve anastomosis) between them is essential for preventing paralysis during tumor removal and for planning during nerve grafting. However, communication between the peripheral branches of the trigeminal and facial nerves has not been fully elucidated owing to the delicacy of their structures and the ambiguity of definitions in previous reports. In this study, we used whole-mount nerve staining to differentiate between the two types of interaction between the trigeminal and facial nerves. "Nerv

Anterior superior alveolar nerve injury after extended endoscopic medial maxillectomy: a preclinical study to predict neurological morbidity.

BACKGROUND: Endoscopic medial maxillectomies (EMMs) are used to optimize exposure of the maxillary sinus and retromaxillary areas. Although in type D EMM (Sturmann-Canfield procedure) the anterior superior alveolar nerve (ASAN) is always at risk of injury, only 29% of patients complained of alveolar process and dental anesthesia. The purpose of this anatomical study is to assess the neural anastomotic network of the ASAN (ASAN-NAN) and describe different extensions of type D EMMs in a preclinical setting. METHODS: The ASAN and its medial anastomotic branches (MABs) and lateral anastomotic bra

Evidence Suggesting that the Buccal and Zygomatic Branches of the Facial Nerve May Contain Parasympathetic Secretomotor Fibers to the Parotid Gland by Means of Communications from the Auriculotemporal Nerve.

BACKGROUND: The auriculotemporal nerve is one of the peripheral nerves that communicates with the facial nerve. However, the function of these communications is poorly understood. Details of how these communications form and connect with each other are still unclear. In addition, a reliable anatomical landmark for locating these communications during surgery has not been sufficiently described. METHODS: Microdissection was performed on 20 lateral hemifaces of 10 soft-embalmed cadavers to investigate facial-auriculotemporal nerve communications with emphasis on determining their function. Morphometry and anatomical variations of the inferior oblique muscle as relevant to the strabismus surgeries.

Effective outcome of inferior oblique (IO) corrective surgeries demands a detailed knowledge of morphometry and variations of IO. Our aim was to study and morphometrically define the surgical anatomy of the IO muscle and its variations. Also to provide easily identifiable surgical coordinates to locate, the IO origin and the oculomotor nerve entry point into the IO. Dissection was performed on 16 cadaveric orbits. IO anatomy, variations, morphometry and relevant surgical distances were measured using digital caliper. IO with multiple bellies was found in five specimens. The IO mean length was

Importance of anatomy in dental implant surgery.

BACKGROUND: The knowledge of nerves and vessels in the maxillofacial region, particularly the anatomical structures in the maxilla, mandible, tongue muscles, and salivary glands, is essential for dental surgeons. In addition, the structures in the mandibular canal, palate, and maxillary sinus should be understood well. HIGHLIGHT: The arteries and nerves in the maxillofacial region were observed in this study. Some variations in the origin of the inferior alveolar artery were found. Notably, the variations in the origin of the inferior alveolar artery from that of the external carotid artery a

Posttraumatic maxillary and ophthalmic herpes zoster in an immune-competent female - A unique presentation.

Herpes Zoster is a neuro-cutaneous disorder caused by reactivation of Varicella-Zoster Virus. A number of factors such as old age, psychological stress, low immune states, radiation exposure and physical trauma have been implicated for reactivation. Post traumatic herpes zoster involving maxillary and ophthalmic division of trigeminal nerve is very rare and has been reported in presence of Immune deficiency and following iatrogenic trauma (nerve block injection). We report a unique presentation of simultaneous ophthalmic and maxillary herpes zoster following a facial injury with a wooden stick

Bilateral pterygomeningeal arteries penetrating the skull anterior to the foramen ovale.

PURPOSE: The pterygomeningeal (accessory meningeal) artery is an inconstant branch of the middle meningeal or maxillary artery. A variant that penetrates the skull anterior to the foramen ovale is rare, and bilateral coexistence has not been reported. A cadaver with bilateral variants of this type is described. METHODS: The head of a 79-year-old man was examined during a gross anatomy course. Origin, course, and relations of the variant vessels were recorded on both sides. RESULTS: On the left, an accessory middle meningeal branch of the middle meningeal artery penetrated the greater wing ap

A Case of Osteopetrosis with Orbital Inflammation Secondary to Maxillary Osteomyelitis.

INTRODUCTION: Osteopetrosis is a rare heritable disorder characterized by increased bone density resulting from osteoclast dysfunction. Major complications include bone fracture, osteomyelitis, anemia, and cranial nerve compression. Optic atrophy can occur due to compression of the optic nerve. Although osteomyelitis of the jaw is a common complication, it rarely occurs in the maxilla. Here, we report a case of a 74-year-old female with osteopetrosis who developed maxillary osteomyelitis, leading to orbital inflammation. CASE PRESENTATION: She was referred to our clinic for 2 months of ptosis

Clinical techniques in veterinary dermatology Regional anaesthesia of the canine and feline nose.

Local anaesthesia techniques are now essential parts of the multimodal analgesic approach applied in the care of small animals. The sensory innervation of the nose is provided by the maxillary division of the trigeminal nerve, so nasal pain can be prevented or alleviated by blocking branches of this nerve. Maxillary or infraorbital nerve block can be used if invasive procedures are performed on the nose of the canine and feline patients. Clear anatomical landmarks facilitate the placements of these blocks, which impose relatively low risk to the patients. These blocks do not require any advanc
