Erbium-doped yttrium aluminium garnet laser-assisted access osteotomy for maxillary sinus elevation: a human and animal cadaver study.

OBJECTIVE: To evaluate the usability of a variable square pulse (VSP) erbium-doped yttrium aluminium garnet (Er:YAG) laser for a lateral access osteotomy to the maxillary sinus in the course of a sinus elevation procedure. MATERIALS AND METHODS: In six formalin-fixed human heads and six fresh sheep heads, a VSP Er:YAG laser was used to perform a bilateral maxillary access osteotomy. For the osteotomies, the Er:YAG laser was applied with a pulse energy of 1000 mJ, a pulse duration of 300 mus, and a frequency of 12 Hz. The spot size was 0.9 mm, and the handpiece was kept approximately 10 mm fro

Clinical analysis and management of bone non-union after Le Fort I maxillary osteotomy: An 8-year retrospective study.

This study aimed to investigate the characteristics and potential etiological factors of maxillary non-union, a rare but serious complication following Le Fort I maxillary osteotomy (LF-I). From a cohort of 1618 patients who underwent the procedure performed by a single clinical team between January 2017 and December 2024, five cases (0.31 % incidence) of non-union were identified, predominantly in females with a mean age of 29.6 years. Analysis of these cases revealed several critical risk factors. The primary surgical movements associated with non-union were maxillary retraction and impactio

Evaluation of surgically assisted rapid maxillary expansion with piezosurgery versus oscillating saw and chisel osteotomy - a randomized prospective trial.

BACKGROUND: Ultrasonic bone-cutting surgery has been introduced as a feasible alternative to the conventional sharp instruments used in craniomaxillofacial surgery because of its precision and safety. The piezosurgery medical device allows the efficient cutting of mineralized tissues with minimal trauma to soft tissues. Piezoelectric osteotome has found its role in surgically assisted rapid maxillary expansion (SARME), a procedure well established to correct transverse maxillary discrepancies. The advantages include minimal risk to critical anatomic structures. The purpose of this clinical com

Comparison of bipartite versus tripartite osteotomy for maxillary transversal expansion using 3-dimensional preoperative and postexpansion computed tomography data.

PURPOSE: To evaluate tripartite paramedian versus bipartite median osteotomy in surgically assisted rapid maxillary expansion. Tripartite osteotomy was performed between the lateral incisors and canines at the former premaxillary junction to avoid midline diastema, septal and columellar dislocation, and asymmetric expansion, minimizing high-distraction forces through bilateral doubled osteotomy on periodontia and thus reducing vestibular attachment loss and producing stable callus formation to avoid relapse. PATIENTS AND METHODS: The preoperative and postexpansion computed tomography data fro

Mandibular deviation and canted maxillary occlusal plane treated with miniscrews and intraoral vertical ramus osteotomy: functional and morphologic changes.

Conventionally, the combination of mandibular and maxillary osteotomy is used to correct both mandibular deviation and maxillary canted occlusal plane. This case report describes functional and morphologic changes after an alternative treatment with miniscrew anchorage instead of maxillary osteotomy. A boy, aged 16 years 5 months, had temporomandibular joint disease symptoms, facial asymmetry with mandibular deviation, and a maxillary canted occlusal plane. The maxillary molars on the right side were extruded 3 mm compared with those on the left side. In addition, there were functional problem

Pre- and postoperative middle ear function and muscle activity of the soft palate after total maxillary osteotomy in cleft patients.

The function of the tensor and levator veli palatini muscles was examined electromyographically in 26 patients with cleft palate, before and after Le Fort I-osteotomy. Hearing ability was determined audiometrically and via tympanometry and the maxillary advancement determined three dimensionally on the basis of model operation and lateral teleradiography. Due to the operative oedema, temporary functional impairment in palatal muscle tissue and sound transmission ability in the middle ear can occur. In several cases, maxillary osteotomy produced an improvement in ventilation of the middle ear a

Biomechanical assessment of the maxillary position after Le Fort I osteotomy using rapidly biodegradable miniplates and screws: an experimental study.

Absorbable plates have become increasingly common in orthognathic surgery. Multiple plate types are available, including poly-L-lactic acid (PLLA) and PLLA/polyglycolic acid (PGA), the latter of which is expected to be absorbed more rapidly. However, the application of these materials in Le Fort I osteotomy is not clearly established. In this study, the strength and biomechanical properties of PLLA and PGA-co-PLLA absorbable plates were compared using an in vitro LeFort I osteotomy model. Basic physical strength was evaluated using tensile, bending, indentation, and handling test s. Biomechani

Evaluation of inter-maxillary fixation techniques for multi-piece Le Fort I osteotomies using finite element analysis.

OBJECTIVE: A strong postoperative occlusal relationship is essential for the long-term stability of the jaw relations post orthognathic surgery. In multi-piece Le Fort I osteotomy, obtaining a satisfactory inter-maxillary fixation (IMF) of the mobilized segments in the correct position and according to the preoperative plan is difficult. Herein, we aimed to evaluate three different IMF techniques (tooth-supported, bone-supported, or hybrid IMF) using finite element analysis (FEA) of the occlusal surfaces of four models: three multi-piece (lateral incisor-canine, central incisor-central incisor

Le Fort I Osteotomy for Removal of an Extensive Complex Odontoma Involving the Maxillary Sinus: A Case Report and Literature Review.

Odontoma is a common odontogenic tumour in the maxillomandibular region, but its occurrence in the maxillary sinus is rare. Small- to medium-sized odontomas can usually be removed without difficulties, depending on their location and proximity to adjacent structures. Nevertheless, access to extensive odontomas may be complicated. The aim of this study was to report a case of surgical removal of an extensive complex odontoma from within the maxillary sinus by using the Le Fort I osteotomy whilst also providing a review of the existing literature on intrasinus complex odontoma, with particular e

Implant Treatment after Sagittal Splitting Ramus Osteotomy and Alveolar Ridge Augmentation in Patient with Mandibular Prognathism and Multiple Missing Maxillary Teeth.

An abnormal maxillomandibular ridge relationship frequently hinders oral implant treatment in patients with jaw deformities. Here, we describe a patient who was experiencing difficulty using dentures due to multiple maxillary tooth loss and mandibular prognathism. Treatment comprising sagittal splitting ramus osteotomy and alveolar ridge augmentation using bone grafts harvested from the mandibular ramus followed by implant treatment yielded good outcomes. The patient was a 47-year-old woman presenting with an unstable upper partial denture. Although prior prosthetic treatment for mandibular pr

Maxillary Expansion and Mandibular Setback Surgery With and Without Mandibular Anterior Segment Osteotomy to Correct Mandibular Prognathism With Obstructive Sleep Apnea.

Mandibular prognathism is usually treated with mandibular setback surgery. However, this approach reduces the pharyngeal airway space, and can aggravate obstructive phenomena in patients with obstructive sleep apnea (OSA). While maxillary expansion is known to lead to an increase in the pharyngeal airway volume (PAS), its effect on the PAS in mandibular setback surgery has not yet been reported. The authors report a surgical approach with maxillary expansion in 2 patients with mandibular prognathism that was accompanied by OSA: maxillary midsagittal expansion with minimum maxillary advancement
