In non-small cell lung cancer with mediastinal lymph node metastasis, intranodal growth is regarded as prognostically more favourable than extranodal growth. We evaluated the clinical implications. Mediastinal lymph node metastases removed at mediastinoscopy and/or surgery were classified as intranodal, extranodal or indefinite. "Minimal N2 disease" denoted a solitary, intranodal metastasis, "extranodal" at least one extranodal lymph node metastasis, and "indefinite" more than one intranodal or at least one indefinite metastasis. Although in patients with resected N2 disease, c. 21% of the nod

The present report is on a 72-year-old male patient with combined paraneoplastic sensory neuronopathy (PSN) and Lambert-Eaton myasthenic syndrome (LEMS) with small cell lung cancer. He noticed a painful paresthesia of the legs which advanced over seven days, and both hands became numb and painful. Three months later, he was found to have small cell lung cancer by mediastinoscopic examination. PSN was diagnosed by clinical symptoms and anti-Hu antibody, and LEMS was diagnosed by the waxing phenomenon on an electromyogram (incremental in compound muscle action potential up to 120%) and autoantib

The diagnostic strategy for asymptomatic patients with persistent bilateral bilar adenopathy often involves invasive procedures. The authors used Bayesian analysis to: 1) estimate the relative prevalences of diseases causing bilateral bilar adenopathy; 2) assess changes in the prevalence of disease by race, the presence of other clinical symptoms, and geography; and 3) determine the value of relevant noninvasive tests, including the angiotensin-converting enzyme (ACE) assay, gallium scan, and purified protein derivative (PPD), in order to assess when a strategy of watchful waiting is appropria

Surgeons are aware that most mediastinal goiters can be excised through a Kocher transverse collar incision, but in rare circumstances a partial-complete median sternotomy or a thoracotomy are mandatory. During an operation to remove a large cervico-mediastinal goiter (CMG) a profound, not massive, bleeding in the anterior mediastinum developed. Bleeding was unsuccessfully treated with packing. Instead, to perform an urgent sternotomy we used telescopic imaging to identify the source of hemorrhage, and a metallic clip was used to stop the bleeding. Since then we have prospectively used the tel

A 65-year-old male with a history of prostate adenocarcinoma treated with radical prostatectomy in 2013 was referred for evaluation of a new pulmonary lesion. Surveillance chest computed tomography (CT) revealed a newly developed 25 × 20 mm irregular parahilar mass in the right upper lobe. 18F-fluorodeoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) showed intense uptake in the lesion (SUVmax 6.6) and mediastinal lymph nodes (SUVmax 12.4), raising concern for malignancy or metastatic recurrence. However, multiple bronchoscopic and mediastinoscopic biopsies were negative

INTRODUCTION: Despite the advent of PET scanning and endoscopic minimally invasive methods of sampling mediastinal lymph nodes, surgical assessment, particularly by mediastinoscopy, remains an important tool for staging non-small cell lung cancer.
METHODS: We carried out a retrospective review of mediastinoscopic lymph node biopsies taken at The Royal Infirmary of Edinburgh between 1996 and 2006 and performed additional histological investigations on select cases.
RESULTS: In total, 89/802 (11%) patients had a negative mediastinoscopy but final resection stage of N2/N3. Within this group, 41

Recent studies have shown that preoperative chemotherapy is feasible and advisable for patients with stage IIIA mediastinoscopically confirmed N2 involvement in non-small cell lung cancer. Such studies confirm the crucial pillar of attaining complete resection, and the importance of downstaging N2 disease as a useful surrogate for long-term survival. Resectability rate is about 70% in almost all studies examined. Better means of predicting resectability and relapse are addressed, such as the potential role of positron emission tomography and some hints for the management of replication error-p

To determine the role of open tube thoracoscopy in the diagnosis and management of thoracic diseases, a retrospective review of 48 patients was conducted. A mediastinoscope was used for open tube thoracoscopy. Eleven patients underwent diagnostic thoracoscopy, and 4 patients had thoracoscopy for cancer staging. All 11 diagnostic procedures yielded a diagnosis, and thoracic malignancies were staged accurately in all patients. Thirty-three patients underwent therapeutic thoracoscopy. Twelve therapeutic thoracoscopies were done for pneumothorax, 9 for loculated parapneumonic effusion or empyema,

From January 1990 to December 1995 79 patients with mediastinal lesions were seen for parasternal mediastinoscopy at the Ruhrlandklinik Essen. Diagnosis was achieved in 91.1%. In 7 cases (8.9%) the diagnosis was not established. Six intraoperative complications occurred following mediastinoscopy: minor bleeding in five instances from the internal mammary vessels and one significant bleeding by injuring the V. cava. Postoperative minor wound infections occurred in five patients. Two pneumothoraces had to be managed by chest tube drainage. One patient with metastatic lung cancer died of respirat

Three cases of primary lipomatous tumors of the anterior mediastinum with prominent muscle differentiation are presented. The patients were two women and one man between the age of 52 and 68 years. All patients presented with progressive shortness of breath. Radiographically, all patients demonstrated anterior mediastinal tumors which were surgically resected. In two cases, the gross findings were those of circumscribed tumors, while one lesion was described as an ill-defined and unencapsulated neoplasm. Histologically, two cases corresponded to a well-differentiated liposarcoma, while the thi

Gastric conduit ulcer is a postoperative complication of esophagectomy. However, penetration into the pericardium is extremely rare and often fatal. Although surgical drainage is standard treatment, optimal management when surgery is difficult remains unclear. A 60-year-old man in shock was transferred to our hospital 2 years after mediastinoscopic esophagectomy with gastric conduit reconstruction for esophageal cancer. Computed tomography revealed air and fluid in the pericardial cavity. Upper gastrointestinal endoscopy revealed a perforated gastric conduit ulcer. The patient was diagnosed wi

OBJECTIVE: The European Society of Thoracic Surgeons (ESTS) has proposed a revised preoperative lymph node staging guideline for patients with potentially resectable non-small cell lung cancer (NSCLC). We aimed to assess the validity of this revised ESTS guideline and survival results in our patient cohort.
METHODS: A total of 571 patients with potentially resectable NSCLC seen between January 2004 and November 2013 were included in the study. The preoperative mediastinal staging was performed by video-assisted cervical mediastinoscopy or video-assisted mediastinoscopic lymphadenectomy in all

Here we report a case that was diagnosed as sarcoidosis but required differential diagnosis from pneumoconiosis. A 51-year-old asymptomatic man, who showed signs of bilateral hilar lymphadenopathy (BHL) on a chest X-ray taken during a medical check, was given a diagnosis of sarcoidosis, based on the results of mediastinoscopic mediastinal lymph node biopsy. Because of the presence of large and small nodular lesions adjacent to the pleura extending from the bilateral upper lobes into the lung field, and continuous bead-like, small nodular lesions in the right interlobar pleura, pleural sarcoido

The significance of computed tomography of the thorax and mediastinoscopy in pretherapeutic mediastinal assessment for the staging of lung cancer remains controversial. The presents study was designed to establish a standard approach to cervical mediastinoscopy for otolaryngologists, who in Denmark traditionally are involved in the staging of non-small cell lung cancer. Sixty-four potentially operable patients with non-small cell lung cancer underwent thoracic computed tomography prior to bronchoscopy and cervical mediastinoscopy. Thirty-six of the 43 mediastinoscopically negative patients add

The use of talc for pleurodesis is a controversial issue. I have used talc in patients with malignant pleural effusion, recurrent pneumothorax, chylothorax and in selected cases of empyema. During pleuroscopy talc is insufflated through the lumen of the mediastinoscope. Up to 2 gr of purified talc is sprinkled under vision over the entire pleural surface. Of our total experience with 83 patients, 77 were available for follow up. Among 52 patients with pleural effusion, complete pleurodesis was achieved in 43 and partial pleurodesis in 5, with 4 failures. Of 21 patients with recurrent pneumotho

BACKGROUND: Clinical N0 lung cancer is surgically treated using a variety of approaches. Nevertheless, a comparison of the effectiveness of these approaches in evaluating lymph node (LN) remains to be conducted.
OBJECTIVE: To evaluate the upgrading rate of LN staging, the total number of resected LNs, and the number of resected LN stations in clinical N0 lung cancer among four surgical approaches.
METHODS: A comprehensive search was conducted across the PubMed, EMBASE, MEDLINE, and CENTRAL databases, encompassing papers published between 1 January 2005, and 31 March 2025. The pooled effect s

A 71-year-old man presented with a thymic mass involving the superior vena cava. A mediastinoscopical biopsy initially suggested a diagnosis of type A thymoma. After neoadjuvant chemotherapy, the patient underwent en-bloc thymectomy and vascular resection for a pathology-confirmed type B3 thymoma involving the superior vena cava, the left brachiocephalic vein and the distal part of the right brachiocephalic vein. Adjuvant radiotherapy was administered. Two years after the primary surgery, abdominal computed tomography (CT) and whole body fluorodeoxyglucose (18-FDG) positron emission tomography

AIMS: To present six cases of cystic well-differentiated squamous cell carcinoma of the thymus.
METHODS AND RESULTS: The patients were six men aged between 48 and 75 years (average: 61.5 years) who were symptomatic with chest pain, shortness of breath, and dyspnoea. Diagnostic imaging showed anterior mediastinal masses, and surgical resection was accomplished in all cases. Grossly, the tumours measured 40-90 mm in greatest diameter (average: 65 mm), and were described as ill-defined lesions with a prominent cystic component and focal areas of haemorrhage and necrosis. Histologically, they wer

BACKGROUND: Tuberculous lymphadenitis is a common condition in underdeveloped and developing countries. Anthracosis is the black pigmentation of bronchi caused by the deposition of carbon, silica and quartz particles in the macrophages of the bronchial mucosa and submucosa. There is a potential relationship between bronchial anthracofibrosis and tuberculosis (TB).
OBJECTIVES: A 66 year old woman was referred to a chest diseases clinic when non-productive cough was noticed during a preoperative examination. Computerized tomography (CT) of thorax showed an infiltration in the right upper lobe a
