Buprenorphine Dispensing Following Medicaid Expansion Amid Unwinding in North Carolina.

IMPORTANCE: The continuous Medicaid enrollment provision adopted during the COVID-19 pandemic ended in March 2023, after which 24 million US residents were disenrolled. Prior studies suggest this "unwinding" was associated with increased discontinuation of buprenorphine, an effective treatment for opioid use disorder. On December 1, 2023, North Carolina expanded Medicaid to low-income adults under the Patient Protection and Affordable Care Act (ACA). OBJECTIVE: To evaluate changes in buprenorphine dispensing following North Carolina's Medicaid expansion. DESIGN, SETTING, AND PARTICIPANTS: Th

Perspectives on Medicaid Coverage for Adult Home-Based Palliative Care.

BACKGROUND: The Department of Vermont Health Access (DVHA) administers Medicaid and currently reimburses for institutional palliative care (PC) for both adults and children. While home-based palliative care (HBPC) is reimbursed for pediatric beneficiaries, HBPC for adults is an uncovered service. OBJECTIVE: To assess the need for adult HBPC Medicaid coverage and evaluate barriers to providing HBPC services from the perspective of local medical professionals and home health agencies (HHAs) in the state of Vermont. DESIGN: Qualitative and descriptive study. SETTING/SUBJECTS: Six clinicians fr

Retention among Medicaid beneficiaries in opioid treatment programs in California.

BACKGROUND: Continuing medications for opioid use disorders (MOUD) for an extended period is important to achieve good outcomes, but many patients do not remain in treatment for even six months. The goal of this study was to develop and describe case-mix adjusted retention rates among California Opioid Treatment Programs (OTP). METHODS: We developed measures of 30-, 90-, and 180-day retention using data from the California Outcomes Management System for Medicaid beneficiaries admitted as an outpatient to an OTP between July 1, 2021 and June 30, 2022. We applied case-mix adjustment to the meas

Opioid utilization after orthopaedic trauma hospitalization among Medicaid-insured adults.

Opioids are vital to pain management and sedation after trauma-related hospitalization. However, there are many confounding clinical, social, and environmental factors that exacerbate pain, post-injury care needs, and receipt of opioid prescriptions following orthopaedic trauma. This retrospective study sought to characterize differences in opioid prescribing and dosing in a national Medicaid eligible sample from 2010-2018. The study population included adults, discharged after orthopaedic trauma hospitalization, and receiving an opioid prescription within 30 days of discharge. Patients were i

Medicaid Expansion and Perinatal Health Outcomes: A Quasi-Experimental Study.

OBJECTIVE: There has been little evidence of the impact of preventive services during pregnancy covered under the Affordable Care Act (ACA) on birthing parent and infant outcomes. To address this gap, this study examines the association between Medicaid expansion under the ACA and birthing parent and infant outcomes of low-income pregnant people. METHODS: This study used individual-level data from the 2004-2017 annual waves of the Pregnancy Risk Assessment Monitoring System (PRAMS). PRAMS is a surveillance project of the Centers for Disease Control and Prevention and health departments that a

Tennessee Medicaid patient immediate postpartum long-acting reversible contraception utilization.

OBJECTIVES: To understand immediate postpartum long-acting reversible contraception (IPP LARC) desire and utilization trends among publicly insured patients delivering at one academic hospital in a state with health care barriers and high short-interval birth rates. STUDY DESIGN: We conducted a retrospective cohort study of electronic delivery records between March 2018 and June 2023 for publicly insured patients. Patient demographics, IPP LARC desire and utilization trends were compared using χ2 or Fisher exact tests. Binary logistic regression explored the relationship between IPP LARC util

Substance use disorder treatment carve outs in Medicaid managed care.

INTRODUCTION: Medicaid managed care organizations (MCO) play a major role in addressing the nation's epidemic of drug overdose and mortality by administering substance use disorder (SUD) treatment benefits for over 50 million Americans. While it is known that some Medicaid MCO plans delegate responsibility for managing SUD treatment benefits to an outside "carve out" entity, the extent and structure of such carve out arrangements are unknown. This is an important gap in knowledge, given that carve outs have been linked to reductions in rates of SUD treatment receipt in several studies. To addr

Medicaid Enrollment and Service Use Among Adults With Down Syndrome.

IMPORTANCE: Down syndrome is the leading genetic cause of intellectual disability and automatically qualifies individuals for Social Security Insurance. Therefore, Medicaid is the major health insurance provider for a population at high risk for dementia, obesity, and premature mortality. Despite the importance of Medicaid for adults with Down syndrome, little is known about how this population uses Medicaid. OBJECTIVE: To describe enrollment in, health care use in, and cost to Medicaid for adults with Down syndrome compared with adults with intellectual disability and a random sample of adul

Hepatitis C Screening Among Medicaid Patients With Schizophrenia, 2002-2012.

OBJECTIVE: Although people with schizophrenia are disproportionately affected by Hepatitis C virus (HCV) compared to the general population, HCV screening among US Medicaid recipients with schizophrenia has not been characterized. Following 1998 CDC recommendations for screening in high-risk populations, we estimated the proportion of Medicaid recipients with and without schizophrenia screened for HCV across states and over time. Examining patterns of screening will inform the current public health imperative to test all adults for HCV now that safer and more effective treatments are available

Reducing Cardiovascular Maternal Mortality by Extending Medicaid for Postpartum Women.

Maternal mortality has been increasing in the United States over the past 3 decades, while decreasing in all other high-income countries during the same period. Cardiovascular conditions account for over one fourth of maternal deaths, with two thirds of deaths occurring in the postpartum period. There are also significant healthcare disparities that have been identified in women experiencing maternal morbidity and mortality, with Black women at 3 to 4 times the risk of death as their White counterparts and women in rural areas at heightened risk for cardiovascular morbidity and maternal morbid

Medicaid Expansion Improved Perinatal Insurance Continuity For Low-Income Women.

Insurance churn, or moving between different insurance plans or between insurance and uninsurance, is common during the perinatal period. We used survey data from the 2012-17 Pregnancy Risk Assessment Monitoring System to estimate the impact of Affordable Care Act-related state Medicaid expansions on continuity of insurance coverage for low-income women across three time points: preconception, delivery, and postpartum. We found that Medicaid expansion resulted in a 10.1-percentage-point decrease in churning between insurance and uninsurance, representing a 28 percent decrease from the prepolic

Medicaid Reimbursement for Common Spine Procedures: Are Compensation Rates Consistent?

STUDY DESIGN: Health Services Research. OBJECTIVE: The purpose of this study is to determine the variability of Medicaid (MCD) reimbursement for patients who require spine procedures, and to assess how this compares to regional Medicare (MCR) reimbursement as a marker of access to spine surgery. SUMMARY OF BACKGROUND DATA: The current health care environment includes two major forms of government reimbursement: MCD and MCR, which are regulated and funded by the state and federal government, respectively. METHODS: MCD reimbursement rates from each state were obtained for eight spine procedur

Variations in Utilization of Carpal Tunnel Release Among Medicaid Beneficiaries.

PURPOSE: To evaluate the null hypothesis that Medicaid patients receive carpal tunnel release (CTR) at the same time interval from diagnosis as do patients with Medicare Advantage or private insurance. METHODS: We conducted a retrospective review using a database containing claims records from 2007 to 2016. The cohort consisted of patient records with a diagnosis code of carpal tunnel syndrome (CTS) and a procedural code for CTR within 3 years of diagnosis. We stratified patients into 3 groups by insurance type (Medicaid managed care, Medicare Advantage, and private) for an analysis of the ti

Disparities in influenza treatment among disabled Medicaid patients in Georgia.

OBJECTIVES: We explored possible disparities in seasonal influenza treatment in Georgia's disabled Medicaid population. We sought to determine whether racial/ethnic, geographic, or gender disparities existed in antiviral drugs usage in the treatment of influenza. METHODS: Medicaid claims were analyzed from 69 556 clients with disabilities enrolled in a Georgia Medicaid disease management program. RESULTS: There were 519 patients who met inclusion criteria (i.e., adults aged 18-64 years with an influenza diagnosis on a 2006 or 2007 Medicaid claim). Roughly one third (36.2%) of patients were c

Changing trends in pediatric antipsychotic use in Florida's Medicaid program.

OBJECTIVE: This study describes the changing trends in antipsychotic use among youths aged 18 years and younger and in age subgroups (zero to five, six to 12, and 13 to 18 years) in the Florida Medicaid program. METHODS: The study used Florida Medicaid claims data associated with approximately 1.2 million children and adolescent enrollees per year to describe monthly antipsychotic use from July 2002 to December 2005. A preliminary examination of trends indicated that antipsychotic use might be different for the periods before May 2004 and after April 2004. For this reason, piecewise regressio

Pharmacists' reactions to the Wisconsin Medicaid drug-use review program.

Pharmacists' reactions to inquiries on Medicaid drug-use review (DUR) and how the information supplied by such inquiries influenced pharmacists' interactions with physicians were studied. A questionnaire was mailed in September 1991 to all 137 pharmacies that had been contacted in 1989 by the Wisconsin Medicaid DUR program about potentially inappropriate use of histamine H2-receptor antagonists. The survey sought to determine whether pharmacists used information in the DUR letters and, if so, how. A section for consultants was included to identify possible differences in responses between cons
