Assessing Intellectual Disability in Culturally and Linguistically Diverse Adults: A United Kingdom Delphi Study.

BACKGROUND: United Kingdom (UK) clinicians report growing demand for intellectual disability diagnostic assessment among adults from culturally and linguistically diverse backgrounds, yet practice is variable, and evidence to guide assessment is limited. This study aimed to establish expert consensus on research, policy, and clinical practice priorities. METHOD: A mixed-methods Delphi design was used, comprising three rounds. Twenty UK-based psychology experts met the inclusion criteria and completed iterative online surveys. Round 1 responses were analysed using template-based thematic analy

The interplay of physician awareness and reporting of incidentally found coronary artery calcium on the clinical management of patients who underwent noncontrast chest computed tomography.

Studies have shown that coronary artery calcium (CAC) incidentally identified on a noncontrast chest computed tomography (NCCT) performed for noncardiac indications has diagnostic and prognostic value. The frequency by which radiologists report incidental CAC and its impact on patient management are unknown. This study included 204 consecutive patients (63 ± 17 years, 59% men) without a history of coronary artery disease referred for an NCCT for noncardiac indications. The presence of CAC was determined by an expert cardiologist and compared with the radiology report. For each patient, the med

"Part of the policy": A qualitative study of PCP attitudes toward urine drug testing for chronic opioid prescriptions.

OBJECTIVE: Guidelines for safer chronic opioid prescribing recommend regular urine drug testing (UDT); however, evidence on the best use of this testing is less clear, and wide variation exists in practice. Our objective in this qualitative study was to assess provider attitudes, barriers, and facilitators to ordering and interpreting UDT for patients prescribed chronic opioids. DESIGN, SETTING, AND SUBJECTS: Qualitative study among family and internal medicine physicians working at several clinics within a large community and academic health system. METHODS: Semistructured interviews were a

Application of symmetry evaluation to deep learning algorithm in detection of mastoiditis on mastoid radiographs.

As many human organs exist in pairs or have symmetric appearance and loss of symmetry may indicate pathology, symmetry evaluation on medical images is very important and has been routinely performed in diagnosis of diseases and pretreatment evaluation. Therefore, applying symmetry evaluation function to deep learning algorithms in interpreting medical images is essential, especially for the organs that have significant inter-individual variation but bilateral symmetry in a person, such as mastoid air cells. In this study, we developed a deep learning algorithm to detect bilateral mastoid abnor

Infecting dose and severity of malaria: a literature review of induced malaria.

The influence of infecting dose on severity of malaria is unknown. The medical literature contains descriptions and analyses of thousands of people, volunteers and neurosyphilis patients, in whom malaria was induced artificially. In some studies it is possible to relate measures of dose to outcome. In this paper a systematic review of this literature is presented. Dose was inversely related to prepatent and incubation period, as expected. A few studies suggested a relation between dose and severity of disease but others did not. Difficulties in interpreting the studies are emphasized, and the

[Diagnostic accuracy of fibrin D-dimer].

Dyspnoea and chest pain are common causes of referral to emergency departments. Both symptoms are non-specific and may be caused by ST elevation myocardial infarction, aortic dissection, pulmonary embolism, or infection. Fibrin D-dimer is often used for ruling out suspicion of acute diseases, but the clinical utility is limited by a modest specificity which lowers with age to approximately ten per cent at the age of 80 years. This review summarises the recommended approaches and highlights potential pitfalls when utilizing and interpreting fibrin D-dimer in the diagnostic work-up of commonly s

Review of Imaging With Focus on New Techniques in Aortic Dissection.

The imaging evaluation of a patient with aortic dissection can be undoubtedly complex, requiring that the interpreting physician understands the classification systems and vocabulary used. This can be made all the more challenging by advances in medical imaging that reshape the understanding of aortic dissection. The purpose of this paper is to provide a review of recent advances in the imaging modalities, and select modality-specific technologies, commonly used to study aortic dissection, including computed tomography, magnetic resonance imaging, and ultrasound. This is followed by an overvie

Recent Advances in the Histopathology of Drug-Induced Liver Injury.

Drug-induced liver injury (DILI) is constantly changing as new drugs are approved and as new herbals and dietary supplements (HDS) reach the market. The pathologist plays a key role in the evaluation of DILI by classifying and interpreting the histologic findings considering patients' medical history and drug exposure. The liver biopsy findings may suggest alternative explanations of the injury and additional testing that should be performed to exclude non-DILI causes. Recent reports of iatrogenic liver injury are reviewed with attention to immunomodulatory and antineoplastic agents as well as

Interpreting Osteogenesis Imperfecta Variants of Uncertain Significance in the Context of Physical Abuse: A Case Series.

Unexplained childhood fracture(s) warrant consideration of physical abuse and osteogenesis imperfecta (OI). Genetic OI testing may identify "variants of unknown significance (VUS)." Interpretation of VUS in context of potential abuse may have protective, criminal, and medical impacts. This case series explores practices regarding clinicians' interpretation of VUS during child abuse evaluations. Variability was noted regarding factors considered for interpreting clinical significance. Based on these cases, recommendations for careful and thorough evaluation are detailed, including proposed use

Attitudes of physicians regarding receiving and storing patients' genetic testing results for cancer susceptibility.

In order to determine interest in and support for a genetic counseling program for heritable cancers, a four-item questionnaire was sent to 700 physicians in San Diego County likely to encounter patients with significant family histories of cancer. Included in the questionnaire was an item requesting information about physician attitudes and practices regarding their record keeping for patient results of genetic testing for cancer susceptibility. Ninety-two questionnaires were returned for a response rate of 13%. The low response rate introduces caution when interpreting the results, particula

Mechanisms of inflammatory bowel diseases: Insights from human genetics and chemical biology.

Human genetics has proven remarkably productive in linking genetic variants to a wide variety of diseases. Discovery initiatives from medical and population genetics have been especially prolific in generating new leads that point to underlying mechanisms of immune-mediated inflammatory diseases. While synthesizing leads from genetics into a functional understanding of disease pathophysiology is gaining momentum, challenges and opportunities abound. Here, we review advancements from combined efforts in functional genomics and chemical biology to construct and dissect mechanisms of mucosal immu
