Respiratory failure. Conventional and high-tech support.

Although significant progress has been made in the treatment of patients with acute lung failure in the critical care setting, the mortality rate from acute lung injury and ARDS is unacceptably high, given the numbers of patients treated for these syndromes each year. The improved understanding of the pathophysiology of respiratory failure from basic science and clinical research is reflected in improved survival rates over the years. Advances in the mechanical ventilator (through microprocessor technology); biosurface technology; liquid ventilation; and, in some cases, returning to so-called

Simulated clinical evaluation of conventional and newer fluid-warming devices.

The purpose of the study was to evaluate the ability of five commercially available devices utilizing a variety of heat exchange technologies to deliver normothermic (37 degrees C) fluids. Conditions of slow (6.5 mL/ min), moderate (13-25 mL/min), and rapid (gravity and pressure driven flows, roller clamp wide open) infusion were simulated. Fluid temperatures were measured using rapid response thermistors after the fluid exited the heat exchanger (T outlet) and before delivery to the patient intravenously (IV) (T distal). Devices tested were the FW537, H1000, Hotline, BairHugger, and Flotem II

Noninvasive nasal mask ventilation for acute respiratory failure. Institution of a new therapeutic technology for routine use.

OBJECTIVE: We successfully implemented the delivery of noninvasive mechanical ventilation for patients with acute respiratory failure, a previously controversial use of this technique, using a simplified ventilator (BiPAP) with nasal mask. Pilot work showed this mode of support to be effective when administered by the members of a research team, and in the current study we were able to transfer this responsibility to usual care providers. SETTING: Almost 90 percent of the patients in this study were in either the 16-bed medical or 31-bed surgical intensive care units at our hospital. SUBJECT

A HEMODILUTION TECHNIQUE FOR OPEN-HEART SURGERY: A PRELIMINARY CLINICAL REPORT.

With the extension of open-heart procedures and the increasing success of open-heart surgery, a great strain has been placed on many hospital laboratories and on the Red Cross for the maintenance of equipment and the supply of blood. In order to decrease both of these needs, hemodilution techniques combined with disposable oxygenators have been introduced. The authors have used a disposable oxygenator with low molecular weight dextran prime, along with moderate hypothermia, in a consecutive series of 15 patients operated upon for congenital and acquired heart lesions. In this consecutive serie

Appropriate technology to prevent maternal mortality: current research requirements.

Maternal mortality involves a complex mixture of clinical, infrastructural and social issues and will require a multifaceted approach if we are to see meaningful reductions occur. That includes thoughtful attention to appropriate technologies for low-resource countries. An international group of specialists meeting in Bellagio, Italy, in 2003 identified important needs and opportunities related to new and underutilized technologies to reduce pregnancy-related mortality. Research to fill in critical information gaps was a recurrent theme. Research, whether it is for product development, for bui

Proceedings of a Consensus Conference: pathogen inactivation-making decisions about new technologies.

Significant progress has been made in reducing the risk of pathogen transmission to transfusion recipients. Nonetheless, there remains a continuing risk of transmission of viruses, bacteria, protozoa, and prions to recipients. These include many of the viruses for which specific screening tests exist as well as pathogens for which testing is currently not being done, including various species of bacteria, babesiosis, variant Creutzfeld-Jacob disease, hepatitis A virus, human herpes virus 8, chikungunya virus, Chagas disease, and malaria. Pathogen inactivation (PI) technologies potentially prov

Laboratory quality improvement in Thailand's northernmost provinces.

PURPOSE: In Thailand nearly 1000 public health laboratories serve 65 million people. A qualified indicator of a good quality laboratory is Thailand Medical Technology Council certification. Consequently, Chiang Rai Regional Medical Sciences Center established a development program for laboratory certification for 29 laboratories in the province. This paper seeks to examine this issue. DESIGN/METHODOLOGY/APPROACH: The goal was to improve laboratory service quality by voluntary participation, peer review, training and compliance with standards. The program consisted of specific activities. Trai

"Do not eat those apples; they've been on the ground!": polio epidemics and preventive measures, Sweden 1880s-1940s.

This article will address how Swedish scientists, physicians and public health officers tried to combat the polio epidemics in the pre-vaccine era. It shows that once polio was considered as an epidemic disease the preventive measures used were based on the hindrance of other infectious diseases. It also illustrates how epidemiological and laboratory studies to some degree affected the thoughts of how polio should be prevented, and that Swedish ideas and experiences differed from those put forward in the USA.

New assistive technology for passive standing.

The anesthetic skin of patients with spinal cord injuries makes these patients a high-risk population for burn injuries. Innovations in rehabilitation engineering can now provide the disabled with mechanical devices that allow for passive standing. Passive standing has been shown to counteract many of the effects of chronic immobilization and spinal cord injury, including bone demineralization, urinary calculi, cardiovascular instability, and reduced joint range of motion and muscular tone. This article will describe several unique assistive devices that allow for passive standing and an impro

Families' experiences of caring at home for a technology-dependent child: a review of the literature.

Recent medical advances have led to the emergence of a group of chronically ill children who are dependent upon technology for their survival. Many of these children are cared for at home by their parents. This paper presents an overview of the literature that has examined the experiences of families caring for a technology-dependent child at home. The social, emotional and financial impact on families and their perspectives on the services supporting them at home is described. The paper concludes by identifying areas where further research is needed.

The origin story of rapamycin: systemic bias in biomedical research and cold war politics.

METEI (Medical Expedition to Easter Island) was a Canadian-led expedition to Easter Island in 1964 that led to the discovery of rapamycin, launching a billion-dollar drug industry and major field of biomedical research. Stanley's Dream, by medical historian Jacalyn Duffin, provides remarkable details about METEI and raises important and timely questions about systemic bias in biomedical studies, the relationship between science and geopolitics, as well as obligations of pharmaceutical companies to indigenous communities. As such, this book is a must-read for those interested in the intersectio

An overview of assistive technology for persons with multiple sclerosis.

Multiple sclerosis (MS) is a progressive neurologic disease clinically characterized by episodes of focal disorder of the cranial nerves, spinal cord, and the brain. MS affects a significant number of young adults, and they most often face a future of progressive functional losses as more of their central nervous system and cranial nerves are affected. As the disease progresses, they have new impairments with accompanying limitations in activities, restrictions to their participation in life, and compromised quality of life. Assistive technology includes any item that is used to maintain or im

Multimodality aesthetic skin rejuvenation.

Patients requesting skin correction and rejuvenation are motivated by a number of clinical problems. Photo damage, abnormal pigmentation or vascularity, textural problems, rhytides, and laxity due to chronological aging are the primary complaints of the majority of patients. Advances in new technology in the past decade have provided the skin correction specialist with new options for treatment. Specialists with multiple technology options then face a new dilemma. What is the best treatment plan for individual patients when many options exist? Over the past five years we have developed an appr
