Saving Money and Reducing Waste With a Tailored Hand Surgery Kit.

BACKGROUND AND OBJECTIVES: There is growing interest in containing cost and decreasing waste in the operating room. As part of a quality improvement initiative, we redesigned the supply kit used for 2 common surgical procedures (carpal tunnel release and trigger finger release) performed under local anesthesia. METHODS: A hand surgeon, a medical student, and an operating room nurse examined each item that would be necessary for performing carpal tunnel release and trigger finger release. A new disposable supply kit was formulated on the basis of their recommendations and was implemented over

Greening the Operating Room: Results of a Scalable Initiative to Reduce Waste and Recover Supply Costs.

Operating rooms generate 42% of a hospital's revenue and 30% of hospital waste. Supply costs are 56% of a total operating room (OR) budget. US academic medical centers use 2 million pounds ($15 million) of recoverable medical supplies annually. Forming a multidisciplinary leadership team, we analyzed sources of waste focusing on our Department of Neurosurgery. We developed an 8-wk pilot project to recycle "blue wrap," the number 5 plastic polypropylene material that is ubiquitously used in ORs across the country to wrap instrument pans and implant trays for sterilization. Blue wrap can be bale

A comparative carbon footprint analysis of disposable and reusable vaginal specula.

BACKGROUND: Healthcare systems in the United States have increasingly turned toward the use of disposable medical equipment in an attempt to save time, lower costs, and reduce the transmission of infections. However, the use of disposable instruments is associated with increased solid waste production and may have negative impacts on the environment, such as increased greenhouse gas emissions. OBJECTIVE: The purpose of this study was to inform this discussion; we applied life cycle assessment methods to evaluate the carbon footprints of 3 vaginal specula: a single-use acrylic model and 2 reus

Health care waste management in community-based care: experiences of community health workers in low resource communities in South Africa.

BACKGROUND: In South Africa, community health workers (CHWs) working in community-based care (CBC) programmes provide care to patients most of whom are living with HIV/AIDS and tuberculosis (TB). Although studies have shown that the caregiving activities provided by the CHWs generate health care waste (HCW), there is limited information about the experiences of CHWs on health care waste management (HCWM) in CBC. This study explored HCWM in CBC in Durban, South Africa from the perspectives CHWs. METHODS: We used three ethnographic approaches to collect data: focus group discussions, participan

An observational study of safe and risky practices in funeral homes in South Africa.

BACKGROUND: Funeral home personnel are at risk of exposure to infectious hazards. The high prevalence of infectious diseases in South Africa means that these workers and family members of deceased individuals are vulnerable to infection if proper safety measures and equipment are not used. OBJECTIVES: To collect observational information on funeral industry practices in order to assess the safety of handling corpses and exposure to risk that could result in disease transmission. METHODS: A cross-sectional study was conducted across two locations from August to October 2015. Funeral homes in

Survey of cataract surgeons' and nurses' attitudes toward operating room waste.

In an online survey of more than 1300 cataract surgeons and nurses, 93% believed that operating room waste is excessive and should be reduced; 78% believed that we should reuse more supplies; 90% were concerned about global warming; and 87% wanted medical societies to advocate for reducing the surgical carbon footprint. The most commonly cited reasons for excessive waste were regulatory and manufacturer restrictions on reuse or multiple use of devices, supplies, and pharmaceuticals. More than 90% believed that profit, liability reduction, and failure to consider carbon footprint drive manufact

Prevalence of selected bacterial infections associated with the use of animal waste in Louisiana.

Human health is a major concern when considering the disposal of large quantities of animal waste. Health concerns could arise from exposure to pathogens and excess nitrogen associated with this form of pollution. The objective was to collect and analyze health data related to selected bacterial infections associated with the use of animal waste in Louisiana. An analysis of adverse health effects has been conducted based on the incidence/prevalence rates of campylobacteriosis, E. coli O157:H7 infection, salmonellosis and shigellosis. The number of reported cases increased during the summer mon

Health-care waste incineration and related dangers to public health: case study of the two teaching and referral hospitals in Kenya.

There are practically no low cost, environmentally friendly options in practice whether incineration, autoclaving, chemical treatment or microwaving (World Health Organisation in Health-care waste management training at national level, [2006] for treatment of health-care waste. In Kenya, incineration is the most popular treatment option for hazardous health-care waste from health-care facilities. It is the choice practiced at both Kenyatta National Hospital, Nairobi and Moi Teaching and Referral Hospital, Eldoret. A study was done on the possible public health risks posed by incineration of th

Degradation of iopromide by combined UV irradiation and peroxydisulfate.

The aqueous degradation of iopromide, an iodinated X-ray contrast media (ICM) compound, by the combination of UV(254) irradiation and potassium peroxydisulfate (K(2)S(2)O(8)) has been studied in laboratory scale experiments. The influence of various parameters on the performance of the treatment process has been considered, namely the UV irradiation light intensity, the initial concentrations of iopromide and peroxydisulfate, and the initial solution pH. Iopromide degradation increased with UV light intensity and peroxydisulfate concentration, but decreased with initial pH. Under specific cond

[Innovation in nineteenth century healthcare: disinfection stations].

The possibility of limiting and preventing epidemics by cleansing everything that could be touched by patients, was an intuitive thought well before microbial discoveries. In accordance with the "miasmas theory", morbid substances were emanated from the bodies of the patients. A step forward took place in the 19th century with Pasteur and Koch's discoveries and progress in chemical disinfectants. The law was slow to adapt: in Italy only in 1888 was a Code drawn up to establish some "Disinfection Stations", places for the sterilisation of infectious material from patients' homes. Later, a simil

Universal precautions--a critical review.

The route of HIV transmission are now well defined. For health care workers the major occupational risk is from parenteral exposure to infected blood or other body fluids. To prevent such exposures, it would be prudent for HCWs to assume that all patients are potentially infected and a set of precautions applicable universally be followed in contacts with all patients. The provisions of "Universal Precautions" apply to blood, CSF, genital secretions and all body fluids. It is essential that barrier protection and washing of hands be practiced, body fluids be handled with care, correct steriliz

Multidisciplinary work on barium contamination of the karstic upper Kupa River drainage basin (Croatia and Slovenia); calling for watershed management.

The present work was designed as an extension of a previous study of a barium anomaly observed in stream sediments of the Kupa River. In its upper part the Kupa River drains a region underlain by a trans-boundary aquifer. The river is a significant water resource in a region of tourism, sport, and fishing in both Croatia and Slovenia. The contamination source is situated in Homer (Lokve), Croatia, where barite was mined until 10 years ago. The barium processing waste material (<3-mm fraction) was carelessly deposited in gardens, forests, and into a sinkhole, which has an underground link with

Three cases of intravenous sodium benzoate and sodium phenylacetate toxicity occurring in the treatment of acute hyperammonaemia.

Intravenous sodium benzoate and sodium phenylacetate have been used successfully in the treatment of acute hyperammonaemia in patients with urea cycle disorders. They provide alternative pathways for waste nitrogen disposal and help maintain nitrogen homeostasis. However, we report three patients with hyperammonaemia who received inappropriate doses of intravenous sodium benzoate and sodium phenylacetate that resulted in severe complications. Ambiguous medical prescriptions and inadequate cross-checking of drug dosage by physicians, nurses and pharmacists were the main causes of these incident

Study on the process of medical waste disinfection by microwave technology.

Microwave sterilization technology is an environmentally friendly non-incineration disposal method for medical waste. The core sterilization technology of "microwave sterilization, steam makes the system heat up quickly" was adopted to build a microwave sterilization screw conveyor system that can continuously and dynamically type microwave sterilization. The platinum resistance temperature sensor is used to measure the temperature of the steam at the outlet of the system and the temperature of the material at the outlet. The microwave sterilization process for medical waste was studied on the
