Polypharmacy Management in Older Patients.

Medications to treat disease and extend life in our patients often amass in quantities, resulting in what has been termed "polypharmacy." This imprecise label usually describes the accumulation of 5, and often more, medications. Polypharmacy in advancing age frequently results in drug therapy problems related to interactions, drug toxicity, falls with injury, delirium, and nonadherence. Polypharmacy is associated with resulting increased hospitalizations and higher costs of care for individuals and health care systems. To reduce polypharmacy, we delineate a systematic, consultative approach to

The Burden of Repeat Prescribing Medications after a Related Adverse Drug Event.

The use of medications such as glyburide, neuroleptics and benzodiazepines is potentially dangerous in the elderly, as they are linked to adverse drug events such as hypoglycemia and serious falls. We used administrative data from the province of Ontario to determine how often these medications are continued after a patient experiences one of these adverse events. Over a 12-year period, we identified 31,262 patients who had a hospital presentation with hypoglycemia or a fall within three months of starting the previously listed medications. Re-dispensing of these high-risk medications occurred

Deprescribing in Hospice and Palliative Care: A Rapid Review of Clinical Tools, Nursing Practice Considerations, and Nursing Policy Implications.

Adults with serious illnesses and limited life expectancy often continue medications with little meaningful benefit, contributing to polypharmacy, adverse drug events, patient distress, and caregiver burden. In hospice and palliative care, deprescribing is a systematic, patient-centered process used when medication-related harms outweigh potential benefits. This rapid review examined contemporary evidence on deprescribing practices for adults with serious illnesses, with emphasis on medication burden, deprescribing patterns, current tools and frameworks, and the nursing role in leading, coordi

What is left of futility? The convergence of anencephaly and the Emergency Medical Treatment and Active Labor Act.

In 1986, Congress passed legislation intended to prevent hospitals from "dumping" patients who were unable to pay for their care. This legislation was appended to the Consolidated Omnibus Budget Reconciliation Act as the Emergency Medical Treatment and Active Labor Act (EMTALA). The EMTALA imposes two sets of duties on all hospitals that receive Medicare funds. Since 98% of hospitals in the United States participate in the Medicare program, EMTALA applies to nearly all hospitals in the United States. In short, hospitals with an emergency department must provide an appropriate screening examina

Genitourinary Emergencies in Older Adults.

Older adults are frequently seen in the emergency department for genitourinary complaints, necessitating that emergency physicians are adept at managing a myriad of genitourinary emergencies. Geriatric patients may present with acute kidney injury, hematuria, or a urinary infection and aspects of how managing these presentations differs from their younger counterparts is emphasized. Older adults may also present with acute urinary retention or urinary incontinence as a result of genitourinary pathology or other systemic etiologies. Finally, genital complaints as they pertain to older adults ar

Impact of the addition of clinical pharmacy services to the cardiac catheterization laboratory.

PURPOSE: Our cardiology pharmacy team recently expanded services to the cardiac catheterization laboratory (CCL) with the addition of a dedicated pharmacist; since that time, numerous process improvement initiatives have been implemented and medication review has been expanded. METHODS: We conducted a single-center retrospective chart review. The primary outcome was the percentage of patients discharged from the CCL on appropriate guideline-directed medical therapy components after percutaneous coronary intervention (PCI) before and after integration of dedicated pharmacist services in the CC

Less Is More: Optimized pharmacotherapy with improved coNtinuity of CarE in hospitaLized oLder peOple (LIMONCELLO): study protocol of a cluster randomized controlled trial.

BACKGROUND: Polypharmacy is associated with negative health outcomes including drug-related hospital admissions, particularly in older patients. Approximately half of drug-related (re)admissions are potentially preventable, and are therefore a target for healthcare interventions. Several studies have investigated the effects of in-hospital medication reviews on clinically relevant outcome measures, such as drug-related readmissions (DRAs). The identified factors for success of the intervention were multicomponent interventions, multidisciplinary approaches, attention to transitional care and t

Professional, structural and organisational interventions in primary care for reducing medication errors.

BACKGROUND: Medication-related adverse events in primary care represent an important cause of hospital admissions and mortality. Adverse events could result from people experiencing adverse drug reactions (not usually preventable) or could be due to medication errors (usually preventable). OBJECTIVES: To determine the effectiveness of professional, organisational and structural interventions compared to standard care to reduce preventable medication errors by primary healthcare professionals that lead to hospital admissions, emergency department visits, and mortality in adults. Effect of pharmacist care on medication adherence and cardiovascular outcomes among patients post-acute coronary syndrome: A systematic review.

BACKGROUND: The impact of collaborative and multidisciplinary health care on the outcomes of care in patients with acute coronary syndromes (ACS) is well-established in the literature. However, there is lack of high quality evidence on the role of pharmacist care in this setting. OBJECTIVE: This systematic review aimed to evaluate the impact of pharmacist care on patient outcomes (readmission, mortality, emergency visits, and medication adherence) in patients with ACS at or post-discharge. METHODS: The following electronic databases and search engines were searched from their inception to Se

Evaluation of the Role of a Clinical Pharmacist on Medication Management in an Adult Burn Clinic via a Collaborative Drug Therapy Management Protocol.

Historically, pharmacists have not been formally involved in managing burn clinic patients. Collaborative Drug Therapy Management (CDTM) protocols allow pharmacists working within a defined context to independently assume responsibility for direct patient care activities. The objective of this study was to evaluate the number and type of medication-related interventions made by a clinical pharmacist, in an adult burn clinic, via a CDTM protocol. The protocol allows pharmacists to independently manage the following disease states: pain, agitation, delirium, insomnia, venous thromboembolism, ski

Strategies for Promoting Antiretroviral Therapy Adherence Among Participants at a U.S. Long-Term Care Facility for People With HIV: A Qualitative Study.

Evidence suggests that HIV antiretroviral therapy (ART) adherence is suboptimal in long-term care (LTC) settings. We studied an HIV-specialized LTC facility to understand how a person-centered care model impacts ART adherence among older adults with HIV (OAWH). We conducted semistructured, in-depth interviews with 11 OAWH and five LTC staff/clinicians asking about barriers and facilitators to ART adherence. Interviews were recorded, transcribed verbatim, and analyzed using deductive and inductive rapid analysis matrices. Three themes emerged: functional support, routines, and fostering residen

How to handle polypharmacy in heart failure. A clinical consensus statement of the Heart Failure Association of the ESC.

The multiplicity of coexisting comorbidities affecting patients with heart failure (HF), together with the availability of multiple treatments improving prognosis in HF with reduced ejection fraction, has led to an increase in the number of prescribed medications to each patient. Polypharmacy is defined as the regular use of multiple medications, and over the last years has become an emerging aspect of HF care, particularly in older and frailer patients who are more frequently on multiple treatments, and are therefore more likely exposed to tolerability issues, drug-drug interactions and pract

Evaluating the quality, feasibility and patient satisfaction of medication history taking by telephone for patients with scheduled admissions: a pilot study.

INTRODUCTION: Medication history taking at hospital admission is still prone to errors. Despite numerous quality improvement initiatives, new strategies to improve medication history taking are still sought and evaluated. Unfortunately, the gold standard research methodology for evaluation is resource-intensive, as it requires each patient to complete two medication history interviews. Therefore, a new study design and quality parameter were developed. AIM: We aimed to pilot our newly developed study design and quality parameter in a study on medication history taking by telephone. METHOD: I

Digital Health Technologies and Standardized Protocols for Patient Safety in Hospital-Based Settings: A Scoping Review.

BACKGROUND: Patient safety remains a major concern in hospital-based settings because of adverse events, medication errors, communication failures, delayed recognition of clinical deterioration, and inconsistent adherence to safety protocols. Digital health technologies are increasingly used to support patient safety, but their contribution may depend on integration with standardized protocols and multidisciplinary workflows. OBJECTIVE: This scoping review aimed to map evidence on the integration of digital health technologies and standardized protocols for supporting patient safety in hospit
