Understanding health insurance and the delay in care for partial meniscectomies: a comparison between public and private coverage.

BACKGROUND: Meniscus injuries are one of the most common orthopedic pathologies. Arthroscopic partial meniscectomy can be performed if the injury has failed nonoperative management or is irreparable. Socioeconomic disparities within health insurance coverage have been known to hinder access to surgical intervention. PURPOSE: The purpose of this study is to examine the impact of insurance status and socioeconomic demographics on the timeliness of care for meniscus tears in a high-volume urban safety-net hospital. METHODS: A retrospective review was conducted of patients aged 18 or older who u

Movement Strategies During the Penultimate and Final Steps of a Change of Direction Following Arthroscopic Partial Meniscectomy.

Persistent knee offloading is well-documented after arthroscopic partial meniscectomy (APM). Yet, it remains unclear if this extends to higher-demanding change-of-direction (CoD) tasks and how step-specific strategies influence joint loading. We aimed to compare joint power and relative contributions in the surgical limb, contralateral limb, and healthy individuals during a 90° CoD. A secondary aim examined the relationship between knee negative power at the penultimate and final foot contacts across limb sequences. Kinematic and kinetic data were collected from 24 post-APM and 20 healthy indi

The Cost-Effectiveness of Meniscal Repair Versus Partial Meniscectomy in the Setting of Anterior Cruciate Ligament Reconstruction.

PURPOSE: To compare the cost-effectiveness of anterior cruciate ligament (ACL) reconstruction with meniscal repair to ACL reconstruction with partial meniscectomy. METHODS: A decision-analytic Markov disease progression model with a 40-year horizon was created simulating outcomes after both meniscal repair and partial meniscectomy at the time of ACL reconstruction. Event probabilities, costs, and utilities were used for the index procedures. The development of osteoarthritis and subsequent knee replacement were either calculated or selected from published literature. Difference in cost, diffe

Evaluation of the rat stifle joint after transection of the cranial cruciate ligament and partial medial meniscectomy.

Osteoarthritis (OA) was induced in the rat stifle joint by partial medial meniscectomy (PMM) and transection of the cranial cruciate ligament (CCL). At 10 weeks after destabilization, joint morphologic and pathologic changes were observed, scored, and compared. The intact rat stifle joint was observed in a mid-saggital plane. Articular cartilage of the distal portion of the femur and proximal portion of the tibia had thicker and thinner sites, and the thicker sites were located caudally on the distal portion of the femur and centrally on the proximal portion of the tibia. The two separate tria

The long term outcome of open total and partial meniscectomy related to the quantity and site of the meniscus removed.

Two hundred patients were randomly selected for either a partial or a total meniscectomy for a meniscal tear during open operation. They were followed for a median of 7.8 years after operation. After partial meniscectomy, posterior horn tears had the worst outcome, but this was only apparent when more than two-thirds of the meniscus had been removed. The amount of meniscal tissue excised was inversely related to the function of the knee, except with bucket-handle tears treated by partial meniscectomy. Patients with bucket-handle, anterior and posterior horn tears had similar functional results

Arthroscopic partial meniscectomy outcomes are comparable in patients with synovitis treated by synovectomy and those without synovitis.

PURPOSE: To determine whether patients undergoing arthroscopic partial meniscectomy (APM) with synovitis treated by synovectomy exhibit similar outcomes to those without synovitis after 24-month follow-up. METHODS: A total of 181 patients who underwent primary APM at a single institution between 2018 and 2022 were included: 87 without synovitis and 94 with synovitis treated via synovectomy involving at least two compartments. Synovitis extent was assessed intraoperatively by the surgeon. Nine Knee injury and Osteoarthritis Outcome Score (KOOS) subscales, including three derived from the Weste

The value of reducing arthroscopic partial meniscectomy in the treatment of degenerative meniscus tears: a budget impact analysis.

AIMS: Numerous studies have shown that arthroscopic partial meniscectomy (APM) is not (cost-) effective in patients with symptoms attributed to a degenerative meniscus tear. We aimed to assess the budget impact of reducing APM in routine clinical practice in this population. MATERIALS AND METHODS: A patient-level state transition model was developed to simulate patients recently diagnosed with a degenerative meniscus tear. Three strategies were compared: "current guideline" (i.e., postpone surgery to at least 3 months after diagnosis), "APM at any time" (i.e., APM available directly after dia

The Potential of Using an Autogenous Tendon Graft by Injecting Bone Marrow Aspirate in a Rabbit Meniscectomy Model.

Bone marrow aspirate (BMA) is an excellent source of cells and growth factors and has been used successfully for bone, cartilage, and soft-tissue healing. This study aimed to investigate the histological and biomechanical properties of autogenous tendon graft by injecting BMA and its protective effect against degenerative changes in a rabbit model of meniscal defects. Adult white rabbits were divided into untreated, tendon, and tendon + BMA groups, and meniscal defects were created in the knees. The tendon graft and articular cartilage status were evaluated by macroscopic and histological anal

Comparative Efficacy of Root Repair versus Partial Meniscectomy and Observation for Patients with Meniscus Root Tears.

The purpose of this study was to (1) compare the efficacy of root repair versus partial meniscectomy and observation for patients with meniscus root tears; (2) compare osteoarthritis (OA) progression and functional outcomes in patients with isolated repair and with adjuvant surgery; and (3) evaluate OA progression and functional outcomes after pullout repair or all-inside repair of meniscus root tears. We performed a systematic literature search in PubMed, Embase, Scopus, and the Cochrane Library up to September 1, 2020, to identify studies that evaluated the efficacy of root repair versus par

The cost-effectiveness of meniscal repair versus partial meniscectomy: A model-based projection for the United States.

BACKGROUND: Meniscal tears are the most common knee condition requiring surgery, and represent a substantial disease burden with clinical and cost implications. The success rates partial meniscectomy and meniscal repair have been studied, but limited information is available investigating their long-term costs and effects. Our objective was to assess the long-term cost-effectiveness of meniscal repair compared to meniscectomy. METHODS: We constructed a decision-analytic Markov disease progression model, using strategy-specific failure rates and treatment-specific probabilities for the develop

Effect of medial meniscus tear and partial meniscectomy on balancing capacity in response to sudden unidirectional perturbation.

Medial meniscus tears and medial partial meniscectomies can damage or remove mechanoreceptors in the meniscus; this inevitably affects knee joint proprioception. Few studies have addressed this subject and none has examined balance in response to sudden perturbation, which requires complex coordination. This study investigated changes in balance in response to sudden unidirectional perturbations after a medial meniscus tear and medial partial meniscectomy in the 3rd and 12th postoperative months. We compared balancing capacity after sudden unidirectional (horizontal) perturbation in 20 control

Articular cartilage MR imaging and thickness mapping of a loaded knee joint before and after meniscectomy.

OBJECTIVE: We describe a technique to axially compress a sheep knee joint in an MRI scanner and measure articular cartilage deformation. As an initial application, tibial articular cartilage deformation patterns after 2 h of static loading before and after medial meniscectomy are compared. METHODS: Precision was established for repeated scans and repeated segmentations. Accuracy was established by comparing to micro-CT measurements. Four sheep knees were then imaged unloaded, and while statically loaded for 2 h at 1.5 times body weight before and after medial meniscectomy. Images were obtaine

Radiographic joint space narrowing and histologic changes in a rabbit meniscectomy model of early knee osteoarthrosis.

The purpose of this study was to compare weightbearing radiographs with histologic cartilage evaluation in a rabbit meniscectomy model of the early stage of osteoarthrosis. Fifteen rabbits had a medial meniscectomy performed in one knee and a sham operation in the other knee. Five rabbits each were sacrificed at 13, 25, and 40 weeks after surgery. Radiographic joint space width and histologic cartilage changes of the medial knee compartment were quantified. Five non-operated knees and five knees in which the meniscus had been removed immediately before the evaluations served as control specime

Rehabilitation of knee flexor and knee extensor muscle strength in patients with meniscectomies, ligamentous repairs, and chondromalacia.

The purpose of this investigation was to determine the relationship of torque developed by knee extensor muscles to torque developed by knee flexor muscles as a result of rehabilitation programs for patients with chondromalacia, ligamentous repairs, and meniscectomies. Peak torque of knee extensor and flexor muscles was measured at slow dynamic and rapid dynamic tension on the involved and homologous noninvolved limb. The rehabilitation program produced significant changes in the torque of the involved limb at both dynamic tensions for each group of patients. The knee extensor muscles of the i

Is Concomitant Contralateral Arthroscopic Meniscectomy Effective in Patients Undergoing Unilateral Total Knee Replacement for Knee Osteoarthritis?

Background/Objectives: In patients requiring unilateral total knee arthroplasty who have relatively mild but symptomatic degenerative osteoarthritis in the contralateral knee, there is ongoing debate regarding whether active intervention, such as arthroscopic surgery, should be performed concurrently or whether conservative management is more appropriate. This study compares patients who underwent simultaneous arthroscopic surgery on the contralateral knee with those who received only conservative treatment, and evaluates the effectiveness of performing arthroscopic surgery concurrently with t

Undergoing Meniscectomy Within One Year Before Primary Total Knee Arthroplasty Is Associated With Worse Postoperative Outcomes.

BACKGROUND: Patients undergoing primary total knee arthroplasty (TKA) who have a history of meniscectomy have worse postoperative functional outcomes, increased rates of early postoperative complications, and higher revision rates. Despite knowing this, to the best of our knowledge, it has not been previously studied whether the timing of meniscectomy before TKA impacts functional outcomes after undergoing TKA. Compared to patients who underwent meniscectomy more than one year before TKA, do patients who have meniscectomy less than one year before TKA have significantly different postoperative

Bucket-Handle Meniscus Tear Management With Meniscectomy Versus Repair Correlates With Patient, Socioeconomic, and Hospital Factors.

INTRODUCTION: Bucket-handle meniscus tears are common knee injuries that are often treated surgically with meniscectomy or meniscal repair. Although clinical factors may influence the choice of one treatment approach over the other, the influence of patient, socioeconomic, and hospital factors remains poorly characterized. This study aimed to estimate the relative nationwide utilization of these two procedures and delineate a variety of factors that are associated with the selection of one treatment approach over the other. METHODS: Meniscal repair and meniscectomy procedures conducted for is
