A hypermobile lateral meniscus remains an important and underappreciated cause of lateral compartment knee pain despite increasing understanding of the complex anatomy surrounding the popliteal hiatus. Recent anatomic and histologic studies continue to refine knowledge of the meniscofibular ligament and the popliteomeniscal fascicle, but microscopic anatomy alone should not dictate surgical decision-making. Histologic evidence of ligamentous tissue does not establish biomechanical function nor validate tear repair techniques. The priority in symptomatic lateral meniscus instability should rema

Meniscus allografts must be transplanted shortly after harvest or be preserved until transplantation, typically by freezing. However, the effects of preservation on the structure-function relationship of the meniscus are not well understood. The objective of this study was to propose a physical mechanism by which preservation affects the tensile properties of the meniscus. It is hypothesized that the separation of collagen fibres from voids formed during freezing and fresh storage decrease tensile strength and modulus. Medial and lateral porcine menisci were dissected on the day of slaughter a

The discoid meniscus is a morphologic variant associated with altered biomechanics and an increased risk of tearing, yet the systems used to classify it have historically emphasized shape over function. This review examines how classification of the discoid meniscus has evolved, from early morphologic schemes such as the Watanabe classification to more comprehensive frameworks that account for peripheral rim stability, tear characteristics, and functional relevance. We discuss later systems proposed by Klingele, Good, and Yang, which recognized rim stability and tear patterns as key determinan

OBJECTIVE: In this review, we summarize the literature on the evolution of magnetic resonance imaging (MRI), segmentation, and quantitative analysis (qMRI) of the meniscus, while bearing in mind the pivotal role of the meniscus in the development (incidence) and progression of symptomatic and structural knee osteoarthritis (KOA).
DESIGN: We performed a literature search across PubMed and Google Scholar, spanning 35 years (1989-2024). We utilized keywords such as "meniscus", "fibrocartilage", "imaging", "magnetic resonance", "radiography", "morphometry", "quantitative analysis", "knee", "osteo

Medial meniscus ramp tears are tears of the posteromedial capsule or peripheral rim of the posteromedial meniscus that frequently occur with anterior cruciate ligament (ACL) tears. The incidence and prevalence of medial meniscus ramp tears has been increasing in the recent literature due to the increased understanding of the anatomy and diagnosis of these tears. When a patient presents with an ACL tear, a medial meniscus ramp tear should be suspected if the patient has a grade 3+ Lachman or pivot-shift exam, a vertical line of increased signal intensity in the posterior capsule or peripheral m

Knee ligament and meniscus injuries in children and teenagers are becoming more numerous because of increased sports participation but also better diagnosis. Meniscus injuries occur either in a normal meniscus or due to a congenital anomaly. The diagnosis is made clinically and confirmed by MRI. Treatment depends on the findings: meniscoplasty for discoid meniscus and primary repair of meniscus tears. Meniscus preservation is the rule. Injuries to the central pivot of the knee typically involve either the anterior cruciate ligament (ACL) or the tibial spine. Age, anatomy and the mechanism of i

Meniscal surgery is commonplace. Patients who have had prior surgery may return for additional imaging for continued or new pain, limited range of motion, or interval injury. Accurate interpretation of postoperative imaging requires a clear understanding of the normal meniscal anatomy and biomechanics. Surgical goals and current surgical techniques impact the imaging appearance. These techniques are reviewed in the context of the various meniscal tear patterns. Multiple imaging modalities may be used in patient assessment with magnetic resonance (MR) imaging providing key information regarding

The meniscus is the most commonly injured structure in the human knee. Meniscus deficiency has been shown to lead to advanced osteoarthritis (OA) due to abnormal mechanical forces, and replacement strategies for this structure have lagged behind other tissue engineering endeavors. The challenges include the complex 3D structure with individualized size parameters, the significant compressive, tensile and shear loads encountered, and the poor blood supply. In this progress report, a review of the current clinical treatments for different types of meniscal injury is provided. The state-of-the-ar

Meniscus damages are most common in sports injuries and aged knees. One third of meniscus lesions are known as white-white zone or nonvascular zones, which are composed of chondrocyte and extracellular matrix composition only. Due to low vascularization the ability of regeneration in such zones is inherently limited, leading to impossible self-regeneration post damage. Meniscus tissue engineering is known for emerging techniques for treating meniscus damage, but there are questions that need to be answered, including an optimal and suitable cell source, the usability of growth factor, the sele

BACKGROUND: Clinical outcomes and survivorship of meniscus transplants remain unclear, especially when magnetic resonance imaging (MRI) and weightbearing radiographic findings are included as endpoints. Many studies calculate survivorship based only on subsequent operative procedures.
PURPOSE: We prospectively determined long-term survivorship and functional outcomes of 72 consecutive meniscus transplants. The effects of articular cartilage damage, concurrent osteochondral autograft transfer (20 knees), patient age, and tibiofemoral compartment were evaluated.

INTRODUCTION: All-inside posteromedial suture for lesions of the posterior horn of the medial meniscus in anterior cruciate ligament (ACL) repair provides effective freshening and good healing.
HYPOTHESIS: The posteromedial portal provides satisfactory healing rates without increasing morbidity or complications rates.
MATERIAL AND METHODS: Intra- and postoperative complications were collected for a consecutive single-center series of 132 patients undergoing posteromedial hook suture of the medial meniscus in ACL repair. Meniscal healing was assessed as the rate of recurrence of symptomatic m

BACKGROUND: Diagnostic imaging represents the fastest growing segment of costs in the US health system. This study investigated the cost-effectiveness of alternative diagnostic approaches to meniscus tears of the knee, a highly prevalent disease that traditionally relies on MRI as part of the diagnostic strategy.
PURPOSE: To identify the most efficient strategy for the diagnosis of meniscus tears.
STUDY DESIGN: Economic and decision analysis; Level of evidence, 1.
METHODS: A simple-decision model run as a cost-utility analysis was constructed to assess the value added by MRI in various comb

The apoptotic bodies were observed aging in osteoarthritic cartilage and meniscus but there is still a lack of knowledge in the literature about the relationship between meniscal injury and apoptosis. The aim of this study is to investigate the apoptotic changes on meniscal tears caused by degeneration and trauma. Thirty-eight specimens of human meniscal tissues harvested from cadavers and patients were selected from groups below the age of 40. There were 14 normal meniscal tissues from cadavers in the control group. There were 24 meniscal tissues harvested from patients who had had arthroscop

The aim of this study was to examine the possibility of complications in medial meniscus repair using an inside-out suturing device. Anatomical cadaveric study. Six fresh frozen cadaveric lower limbs were used. The posterior horn of the medial meniscus was sutured using three vertical stitches. An anatomical dissection was subsequently performed to check for any possible effects upon the structures of the medial aspect of the knee. In addition, an incision was made in a safety zone in order to ascertain whether it was possible to carry out the suture without affecting the aforementioned struct

PURPOSE: Menisci are known to have receptors mainly concentrated at the anterior and posterior horns. Although they are purported to send afferent impulses to the central nervous system, this function has not been thoroughly evaluated. The purpose of the study was to investigate whether stimulation of the menisci initiates a cortical response. The reaction of the end organ to the reflex arc is also evaluated.
TYPE OF STUDY: Prospective case series.
METHODS: Fourteen patients with normal medial menisci were included in the study. Different parts of the knee joint (the posterior horn and the b

PURPOSE: To quantitate the normal level of the tear meniscus, and to determine whether it is an accurate diagnostic indicator of nasolacrimal drainage obstruction.
METHODS: We performed a retrospective, controlled, case series study on patients evaluated from 2000 to 2002 for epiphora. The height of the tear meniscus was measured using the slit lamp by one experienced observer. Nasolacrimal outflow status was assessed by probing and irrigation. Patency on the nontearing eye was also confirmed by probing and irrigation, and the corresponding tear meniscus levels comprised one control group (59

Between 1985 and 1995, 45 patients underwent closed meniscus repair. There were 30 men and 15 women with a mean age of 32.5 years. In 23 patients, the anterior cruciate ligament was intact (group 1) whereas it was deficient in 22 patients (group 2). All patients were managed with the same postoperative program of partial weight bearing, immediate motion and rehabilitation of the knee. Five patients had a failed meniscal repair and underwent a repeat arthroscopy and a partial meniscectomy. These patients were considered as failures and excluded from the final scoring. After a mean follow-up of

Discoid meniscus is an uncommon dysplasia generally affecting the lateral meniscus. Its congenital or acquired origin remains a question of debate. We report 5 cases or lateral discoid meniscus in 4 girls and 1 boy, aged from 6 to 11 years. The main clinical signs were snapping knee and pain. The discoid meniscus appeared in MRI as an abnormally large meniscus extending into the joint space. It appeared as a central band giving increased signal in 3 out of 5 cases. The significance of this probably trauma-induced image was uncertain. Surgery confirmed MRI findings in 4 cases in which total res
