Hypertension and obesity after the menopause.

It is well known that in pre-menopausal women the incidence of cardiovascular events is lower than in men of the same age and that after the menopause cardiovascular morbidity and mortality in women become similar, if not higher, than that in men indicating that female sex hormones play a relevant protective role upon the vasculature. Among the cardiovascular risk factors, hypertension appears to be more prevalent in postmenopausal women than in men but the precise mechanism through which menopause favours the development of hypertension is still a matter of debate. The prevalence of obesity a

Hormonal changes in the menopause transition.

The menopause is the permanent cessation of menstruation resulting from the loss of ovarian follicular activity. It is heralded by the menopausal transition, a period when the endocrine, biological, and clinical features of approaching menopause begin. A common initial marker is the onset of menstrual irregularity. The biology underlying the transition to menopause includes central neuroendocrine changes as well as changes within the ovary, the most striking of which is a profound decline in follicle numbers. Follicle-stimulating hormone (FSH) is an established indirect marker of follicular ac

Menopause, hormone replacement therapy and cancer.

OBJECTIVE: To comparatively review available evidence on hormone replacement therapy (HRT) and cancer. METHODS: Qualitative literature review. RESULTS: Most potential favorable and adverse effects on cancer risk of HRT are restricted to current users. On the basis of observational epidemiological data, the RR of breast cancer is moderately elevated in current and recent HRT users, and increases by about 2.3% per year with longer duration of use, but the effect drops after cessation and largely, if not totally, disappears after about 5 years. Unopposed estrogen use is strongly related to endo

African perceptions and myths about menopause.

Menopause for most African women marks the end of reproductive potential. For the grand multiparous women deprived of modern contraceptive technologies it is also a relief from pregnancies; but to the childless women it could be the beginning of a depression. Age per se is not as important a consideration as the events surrounding menopause. Cultural beliefs and practices vary with the different communities in Africa. It is important for health providers to identify such beliefs and practices if reproductive health problems that emerge in the climacteric have to be prevented and managed correc

Estrogen and cardiovascular function after menopause.

Nonrandomized trials of postmenopausal estrogen replacement have shown a benefit in the prevention of coronary artery disease. Less clear are the specific mechanisms by which this occurs. Estrogen has beneficial effects on the lipid profile, with significant elevations in high density lipoprotein cholesterol and reductions in low density lipoprotein cholesterol reported. Also, antioxidant properties have been ascribed to estrogen. In addition, estrogen has been shown to prevent paradoxic vasoconstriction in atherosclerotic coronary arteries after acetylcholine and may have calcium channel-bloc

Keratoconus-tetany-menopause: the new association.

This report describes the unusual case of 46-year-old white European woman who presented spontaneous keratoconus. This case suggests a new association: keratoconus-tetany-menopause. This can be explained by Thalasselis' syndrome, a syndrome showing a relation between keratoconus, magnesium (Mg) deficiency, type A behavior, and allergy. Furthermore, Thalasselis' syndrome integrates old and new theories. This report also suggests that the alteration induced by severe but reversible Mg deficiency at the intra- and extracellular levels, associated with metabolic and hormonal imbalances, may trigge

A simple way to manage menopause.

Menopausal women require early therapy to prevent or ameliorate signs and symptoms as well as pathologic consequences of menopause. Estrogen replacement therapy combined with adequate calcium supplementation and exercise is the only well-established prophylactic regimen. Medroxy-progesterone acetate (Curretab, Cycrin, Provera) is added to the treatment regimen to prevent endometrial hyperplasia in women who have not undergone hysterectomy. When estrogen therapy is medically contraindicated, medroxy-progesterone acetate or salmon calcitonin (Calcimar, Miacalcin) combined with calcium supplement

Menopause and menorrhagia: a historical exploration.

Like a parallel article [1] this one also attempts to answer the question why complaints of excessive premenopausal bleeding have now virtually disappeared. Unlike the first, a review, this is a discussion paper. It explores widely, investigating the question from all aspects, even utilizing, if with caution, such unconventional sources as still unproven hypotheses or marginal theories. Presentational attitudes to, and handling of, climacteric complaints appear partially to obscure some symptoms associated with excessive menses. More importantly, however, the examination of the historical reco

Mumps, menarche, menopause, and ovarian cancer.

Clinical history of mumps during childhood, age at menarche, and age at natural menopause were obtained in 119 postmenopausal women with ovarian cancer and 109 postmenopausal control subjects from the general population. Case subjects differed significantly from control subjects in being less certain whether they had had mumps and in being less likely to recall the age at infection. In both groups, a significant inverse correlation was observed between age at menarche and menopause in subjects with a positive clinical history for mumps but not in subjects with a negative clinical history for m

Does the environment affect menopause? A review of the effects of endocrine disrupting chemicals on menopause.

Endocrine disrupting chemicals are widely distributed in our environment. Humans are exposed to these compounds not only through their occupations, but also through dietary consumption and exposure to contaminated water, personal care products and textiles. Chemicals that are persistent in the body and in our environment include dioxins and polychlorinated biphenyls. Non-persistent chemicals including bisphenol A, phthalates and parabens are equally as important because they are ubiquitous in our environment. Heavy metals, including lead and cadmium, can also have endocrine disrupting properti

Nonhormonal management of menopause-associated vasomotor symptoms: 2015 position statement of The North American Menopause Society.

OBJECTIVE: To update and expand The North American Menopause Society's evidence-based position on nonhormonal management of menopause-associated vasomotor symptoms (VMS), previously a portion of the position statement on the management of VMS. METHODS: NAMS enlisted clinical and research experts in the field and a reference librarian to identify and review available evidence. Five different electronic search engines were used to cull relevant literature. Using the literature, experts created a document for final approval by the NAMS Board of Trustees. RESULTS: Nonhormonal management of VMS i

Cardiovascular Risk Associated with Menopause and Menopause Hormone Therapy: A Review and Contemporary Approach to Risk Assessment.

PURPOSE OF REVIEW: Discuss the effects of menopause and menopause hormone therapy (MHT) on cardiovascular risk, and propose a structured, person-centered framework for cardiovascular risk assessment when initiating MHT. RECENT FINDINGS: The risk of atherosclerotic heart disease accelerates during the menopause transition due to hormonal, metabolic, and vascular changes. Both menopause and MHT affect cardiovascular risk factors (i.e. blood pressure, lipids, insulin resistance) and cardiovascular events (i.e. myocardial infarction and stroke). Early clinical trial evidence demonstrated that ora

[A histological and hormonal study of 399 women near the menopause or with confirmed menopause (author's transl)].

This work is concerned with 399 women who were either near the menopause or had had the menopause and on whom 476 hormone levels and 169 examinations of the endometrium had been carried out. It is possible to put the women into three groups. 13 p. 100 showed a diphasic cycle, with more or less normal ovarian function. Among the group of women whose cycle was not diphasic an appreciable number had normal oestrogenic activity, or were indeed hyperoestrogenic. The others were hypo-oestrogenic. Several facts can be derived from this study. Raised levels of E.2 can be found even when there is no cy

Impact of Menopause and Menopausal Hormone Therapy on Liver-Kidney-Metabolic Health.

Menopause is associated with unfavorable metabolic changes that may contribute to the development of metabolic dysfunction-associated steatotic liver disease (MASLD), chronic kidney disease (CKD), and increased cardiometabolic risk. Within the evolving cardiovascular-kidney-metabolic (CKM) framework, MASLD is increasingly recognized as a key driver rather than an outcome of metabolic dysfunction. The liver and kidneys closely interact to regulate metabolism, detoxification, ketogenesis, and endocrine signaling, which together influence whole-body metabolic homeostasis. Menopause and the menopa

Management of menopausal hot flushes. Recommendations from the Spanish Menopause Society.

This project aims to develop recommendations for treating vasomotor symptoms (VMS) based on the Cervantes short-form scale score (menopausal domain) using the best available evidence. A total of 166 studies were selected: 108 randomized controlled trials, 23 systematic reviews, 3 reviews, 3 meta-analyses, 11 case-control studies, 9 observational studies, and 12 transversal studies. To achieve this objective, a series of PICO (Patient, Intervention, Comparison, and Outcome) questions have been established for the treatment of VMS. We evaluate the quality of the scientific evidence and, with the

The Effect of Menopause and Menopausal Hormone Therapy on the Risk of Peripheral Artery Disease.

Peripheral artery disease (PAD), defined as lower extremity arterial disease, constitutes an underestimated aspect of the menopause-associated risk of atherosclerotic cardiovascular disease (ASCVD). Accumulation of ASCVD risk factors, such as atherogenic dyslipidaemia, diabetes, and arterial hypertension, after the transition to menopause may contribute to atherosclerotic plaque formation in peripheral arteries. However, inconsistency exists among studies as to whether transition to menopause increases the risk of PAD, although early menopause (<45 years) or premature ovarian insufficiency may
