Paracelsus Revisited: The Dose Concept in a Complex World.

At the time that Paracelsus coined his famous dictum, 'What is there that is not poison? All things are poison and nothing is without poison. Solely the dose determines that a thing is not a poison', embryonic toxicology was a fairly focused discipline that mainly dealt with occupational poisonings and side effects of pharmaceuticals, such as mercury. While Paracelsus paved the way for the modern threshold concept and the no-adverse effect level, modern-day toxicology is now tussling with highly complex issues, such as developmental exposures, genetic predisposition and other sources of hypers

Clinical characteristics, management, and outcomes of diseases caused by mercury overexposure: a systematic review of case reports and case series.

BACKGROUND: Mercury poisoning remains a serious public health issue due to its multiple exposure routes and diverse clinical presentations. However, existing clinical evidence is fragmented, especially regarding real-world case data. This study addresses the gap by systematically reviewing global case reports to analyze clinical features, diagnosis, and treatments, thereby offering more robust evidence for clinical practice. METHODS: Case reports of diseases caused by accidental mercury exposure, published from January 1950 to April 2025, were identified through a comprehensive search of thre

Perinatal methylmercury poisoning in Iraq.

The signs and symptoms of methylmercury poisoning and the concentrations of mercury in samples of blood, hair, and milk are reported and compared in two infant-mother pairs exposed in the recent Iraq outbreak. In one pair, the infant was born prior to the exposure of the mother, and was exposed only from ingestion of methylmercury in mother's milk. In the other pair, the mother was exposed during pregnancy and did not breast feed the infant, who died 30 days after birth. Both mothers had some signs and symptoms of poisoning, but the infants did not. The infants had maximum estimated blood merc

Suicidal intoxication with mercury chloride.

PURPOSE: Poisoning with elemental metals and metallic compounds was much more frequent in the past, and was related, among other things, to lifestyle and the lack of appropriate toxicological diagnostics. One example is mercury, which is being gradually eliminated but still has many different applications as a pure metal or in the form of various compounds. The paper presents a case of suicidal poisoning with mercury chloride (corrosive sublimate). METHODS: Forensic and toxicological tests including inductively coupled plasma mass spectrometry (ICP-MS) were at the Department of Forensic Medic

Poisoning of dairy heifers by mercurous chloride.

Mercury poisoning was diagnosed in four dairy heifers, three of which died. The clinical signs were variable and included salivation, excessive thirst, extreme depression and severe diarrhoea. Postmortem examinations revealed inflammation and ulceration of the alimentary tract, pulmonary and cardiac haemorrhages, pallor of the kidney cortices and perirenal oedema. The kidney mercury concentrations were in the range 58 to 91 micrograms/g wet tissue. It is believed that the animals were poisoned by the ingestion of soil contaminated with mercurous chloride.

Presentation of a general algorithm to include secondary poisoning in effect assessment.

A general algorithm for effect assessment on secondary poisoning for birds and mammals is presented. This algorithm (Maximum Permissible Concentration = NOECbird/mammal/BCF) was drawn up by analysing an aquatic food chain (water--fish--bird or mammal) and a terrestrial food chain (soil--worm--bird or mammal). NOECs and bioconcentration factors (BCFs) were collected for a set of selected compounds: lindane, dieldrin, cadmium and mercury in both water and soil, PCB153 only in water and DDT and PCP only in soil. BCFs for the terrestrial pathway are frequently < 1 and rarely above 10, though for t

A Brazilian Minamata disease? Neurologists must be aware of mercury exposure and intoxication.

Mercury intoxication poses a significant challenge and growing threat to public health, particularly in the Amazon region. Despite a known history of neurological damage, as evidenced by Japan's Minamata disease, mercury intoxication remains underdiagnosed in Brazil. This review underscores the need for increased clinical awareness among neurologists, as mercury exposure has been linked to over 250 neurological symptoms, including cognitive impairment, cerebellar ataxia, peripheral neuropathy, and psychiatric disturbances. The Indigenous and riverside populations in the Amazon present a high p

Presentation of a general algorithm to include effect assessment on secondary poisoning in the derivation of environmental quality criteria. 2. Terrestrial food chains.

In a previous study a simple algorithm was presented for effect assessment on secondary poisoning of birds and mammals. This algorithm (MPC = NOECfish-eater/BCFfish) was drawn up by analyzing a two-step aquatic food chain (water-fish-bird/mammal). The algorithm was used to test whether quality criteria set for surface water, based on effect assessment for aquatic organisms, constitute a "safe" level for secondary poisoning. The present study analyzes whether this algorithm can equally well be used for effect assessment in a terrestrial food chain. The pathway soil-earthworm-bird/mammal was use

Toxic effects seen in a herd of beef cattle following exposure to ash residues contaminated by lead and mercury.

Lead poisoning was diagnosed in three cattle along with increased mercury levels in the liver and kidney tissues of two of these animals. The clinical signs were different in each case and included salivation, anorexia, delayed menace response, delayed withdrawal reflex, head pressing, localized muscle fasciculation, reduced tongue tone, ataxia, rumen atony and seizures. Blood lead concentration was increased in all three cases to 0.76, 0.37 and 0.454ppm. Post mortem changes characteristic of lead poisoning were only recognized in one case and included cerebro-cortical oedema, cortical neurona

Health risk assessment for human mercury exposure from Cinnabaris-containing Baizi Yangxin Pills in healthy volunteers Po administration.

BACKGROUND: Cinnabaris (α-HgS), a mineral traditional Chinese material medica, has been used in combination with other herbs manifesting some definite therapeutic effects for thousands of years. But the currently reported mercury poisoning incidents raised the doubts about the safety of Cinnabaris-containing traditional Chinese medicines (TCMs). Baizi Yangxin Pills (BZYXP) is a Cinnabaris-containing TCM widely used in clinical practice. This study evaluated the health risk of mercury exposure from BZYXP in healthy volunteers based on the total mercury and mercury species analysis of blood and

The role of thiols, dithiols, nutritional factors and interacting ligands in the toxicology of mercury.

Mercury has been a known as a toxic substance for centuries. Whilst the clinical features of acute mercury poisoning have been well described, chronic low dose exposure to mercury remains poorly characterised and its potential role in various chronic disease states remains controversial. Low molecular weight thiols, i.e. sulfhydryl containing molecules such as cysteine, are emerging as important factors in the transport and distribution of mercury throughout the body due to the phenomenon of "Molecular Mimicry" and its role in the molecular transport of mercury. Chelation agents such as the di

[Clinical aspects of the Niigata Minamata disease].

The Minamata disease was discovered in the Minamata region, Kumamoto Prefecture, Japan, in 1956. Symptoms of this disease included cerebellar ataxia, sensory disturbance, narrowing of the visual field, and hearing and speech disturbances. In 1965, similar conditions were identified in persons living around the Agano River area, Niigata Prefecture, Japan and accordingly termed as the Niigata Minamata disease or the second Minamata disease. Both the diseases have been attributed to poisoning with methyl mercury that was generated during the production of acetaldehyde using mercury as a catalyst.

Neurotoxic effects in workers of the clinical thermometer manufacture plant.

OBJECTIVES: Occupational exposure to mercury can induce adverse health effects, and the central nervous system is the major target of its toxic action. This problem especially arises in plants involved in the manufacture of mercury-containing products, where an appropriate protection against mercury exposure is not ensured. The aim of this study was to assess health effects of mercury, especially neurotoxic effects and oral disorders, in workers employed in a clinical thermometer manufacture plant and to determine mercury concentrations in the workplace ambient air. MATERIALS AND METHODS: The

Molecular Fates of Organometallic Mercury in Human Brain.

Mercury is ubiquitous in the environment, with rising levels due to pollution and climate change being a current global concern. Many mercury compounds are notorious for their toxicity, with the potential of organometallic mercury compounds for devastating effects on the structures and functions of the central nervous system being of particular concern. Chronic exposure of human populations to low levels of methylmercury compounds occurs through consumption of fish and other seafood, although the health consequences, if any, from this exposure remain controversial. We have used high energy res

Mercuric oxide poisoning treated with whole-bowel irrigation and chelation therapy.

Most reported cases of inorganic mercury poisoning are from mercuric chloride. We report a case of mercuric oxide (HgO) powder ingestion. A 31-year-old man presented to an emergency department after ingestion of approximately 40 g of HgO. Soon after ingestion, he developed nausea, vomiting, and abdominal cramping. Abdominal radiograph revealed densely radiopaque material in the stomach. Gastrointestinal decontamination was accomplished with activated charcoal and whole-bowel irrigation with polyethylene glycol solution (Golytely) for 24 hours until repeat abdominal radiographs no longer demons

[Determination of total amounts of mercury in unfrothing preparations].

Mercury and its organic (CH3 HgCl) and inorganic (HgCl2) compounds are very poisonous. The analytical control of trace amounts of metals in products that are used in the food industry is of great importance. In this work, the determination of mercury, present at trace levels in industrial unfrothing preparations. Mercury was determined by the cold-vapour atomic absorption spectrometry. The method allows for accurate determination of mercury and as little as 0.01 mg ph Hg per kilogram of product can be determined.

Elemental mercury toxicity due to aspiration following intentional massive ingestion.

Deliberate poisoning with intentional ingestion of elemental mercury is reported not to result in systemic toxicity due to minimal absorption from the gastrointestinal tract. We report a case of a 43 year old male who intentionally ingested 200 ml elemental mercury which resulted in abdominal pain and vomiting. The patient subsequently aspirated globules of mercury which was confirmed on chest x-ray and his blood mercury levels were markedly raised. He was treated with chelating agents and managed in a negative pressure room to reduce the risk of staff being exposed to exhaled mercury vapour f

[Long-term follow-up after intravenous injection of mercury--two cases report].

In this paper we present the long-term follow-up of two patients, after injection of metallic mercury. Case 1. In 1997, 29-years-old man injected himself to left elbow about 20 ml of metallic mercury by mistake (he was heroin abuser for short time). Mercury concentration in the blood was 400 microg/L. X-ray of the chest, abdomen and affected elbow area showed radiopaque foreign material (depots of mercury). Depots of mercury were also visible on the tricuspid valve in echocardiography. Mercury from the soft tissue left elbow pit was partially surgically removed. During 15 years follow-up two t

Diagnostic value of a chelating agent in patients with symptoms allegedly caused by amalgam fillings.

The chelating agent 2,3 dimercaptopropane-1-sulfonate (DMPS) has been used in a mercury mobilization test for diagnoses in illnesses allegedly associated with the presence of amalgam restorations. DMPS is an accepted antidote to heavy metal poisoning. The aim of the present study was to evaluate the diagnostic value of DMPS in patients with symptoms self-related to their amalgam fillings. The subjects consisted of four groups: 19 healthy controls without amalgam experience; 21 healthy controls with amalgam fillings; 20 patients who claimed symptoms of "mercury poisoning" from dental amalgam; a

[Toxic nephropathy secondary to occupational exposure to metallic mercury].

Toxic nephrophaties secondary to occupational exposure to metals have been widely studied, including membranous nephropathy by mercury, which is rare. Occupational poisoning by mercury is frequent, neurological symptoms are the main form of clinical presentation. Secondary renal involvement in chronic exposure to metallic mercury can cause glomerular disease by deposit of immune-complexes. Membranous glomerulopathy and minimal change disease are the most frequently reported forms. Here we describe the case of a patient with occupational exposure to metallic mercury, where nephrotic syndrome du
