A man aged 75 developed abdominal pain during treatment with steroids for periarteritis nodosa. Laparotomy disclosed air in the peritoneum, and a section of the sigmoid colon contained numerous airfilled cysts and the mesocolon was emphysematic. A solitary diverticulum communicated with a fistula extending into the mesocolon. Neither inflammation, nor other diverticula were seen at the histological examination.

A 52-year-old man whose fecal occult blood test was positive was found to have type 2 sigmoid colon cancer by colonoscopy. On enhanced barium enema study, the cecum was in the pelvis, and the ascending colon was running medially in the abdomen. Enhanced CT scan of the abdomen revealed rotation of the superior mesenteric vein(SMV). We diagnosed the case as sigmoid colon cancer(cT3N0M0, StageⅡA)with non rotation-type intestinal malrotation, and performed laparoscopic surgery. We confirmed the small intestine to be located on the right side of the abdomen, the cecum to be located in the pelvis, a

We report a 48-year-old-man with gastric carcinoma presenting with an unusual extraluminal growth. The patient underwent a barium meal examination and gastrofiberscopy because of progressive anemia over 6 months. These examinations revealed a Borrmann type 3 advanced gastric carcinoma of the greater curvature of the antrum. Biopsies showed moderately differentiated tubular adenocarcinoma. The intraoperative findings showed gastric carcinoma associated with extensive extraluminal invasion into the adjacent organs, i.e., the transverse colon and mesocolon. A palliative distal gastrectomy with a

The clue of the mobilization of the splenic flexure is a complete division of the root of the transverse mesocolon along the pancreas. After the section of the terminal part of the inferior mesenteric vein, the retrogastric cavity is opened and the root of the mesocolon is cut from the right to the left onto the parieto-colic attachment. Thereafter the coloepiploic attachments are divided. The retromesocolic dissection is continued from top to bottom without opening the Gerota's fascia.

BACKGROUND Although recurrence after surgery for esophagogastric junction (EGJ) adenocarcinoma frequently develops in the mediastinal and para-aortic lymph nodes (LN), distant LN recurrence in the mesocolon is rare. We report a rare case of ileocecal LN metastasis in the ascending mesocolon after radical surgery for an EGJ adenocarcinoma. CASE REPORT We performed subtotal esophagectomy with mediastinal and para-gastric LN dissection in a patient with an advanced EGJ adenocarcinoma. Clinicopathologically, the patient was diagnosed with type I EGJ adenocarcinoma based on Siewert's classification

BACKGROUND: Complete mesocolic excision with central vascular ligation is a standard method for managing colon cancer. However, there is no consensus on its procedure, especially for cancer in the splenic flexure of the transverse colon. This is because various types of variational arteries are distributed to the region, and their running course below and near the pancreas leads to difficulty in ligating the artery.
OBJECTIVE: To clarify the arterial distribution to the splenic flexure of the transverse colon using cadavers.
DESIGN: The arteries in the transverse mesocolon distributed to the

Left paraduodenal hernia is an entrapment of the small bowel into the Landzert fossa, an unusual congenital peritoneal defect behind the descending mesocolon that results from failure of part of the descending mesocolon to fuse with the posterior parietal peritoneum (Doishita et al. in Radiographics, 36(1): 88-106, 2016). This fossa is reported to be present in approximately 2% of autopsy bodies. The authors present a case of a left paraduodenal hernia in a young woman.

Walthard cell nest (WCN) is a rare incidental finding in the intestine including the vermiform appendix and the mesocolon. This article describes a case of WNCs in the vermiform appendix and the mesocolon. The location in the mesocolon is reported for the first time. A review of the seven published intestinal cases including our report is presented. The lesion is seen in children, adults, and both sexes. The number of present cell nests varied between one and three per case. The diagnosis rests on histopathological examination. A potential source for histopathological diagnostic error includes

BACKGROUND: The traditional approach to a Roux-en-Y gastric bypass is to construct the biliopancreatic and Roux limbs within the inframesocolic space. This requires reflecting the greater omentum, transverse colon, and mesocolon superiorly. This maneuver can be challenging in patients with a robust omentum, hepatomegaly, or significant omental adhesions. The inability to fully reflect the omentum superiorly prevents an appropriate view of the posterior transverse mesocolon and the inframesocolic space from being obtained. This can make it difficult to construct the biliopancreatic and Roux lim

Behçet's disease (BD) is a multisystem autoimmune disorder characterized by recurrent orogenital ulcers, uveitis, and skin lesions. The vascular manifestations include thrombophlebitis, stenosis, occlusion, and pseudoaneurysm. BD infrequently precipitates aortic pseudoaneurysm rupture into the sigmoid mesocolon and lumen of the adjacent colon. Here we report an extremely rare case of spontaneous abdominal aortic pseudoaneurysm rupture via the sigmoid mesocolon into the lumen of the sigmoid colon in a 37-year-old patient with BD.

Splenic flexure mobilization is a challenging surgical procedure, but is it necessary to safely perform left colon and rectal resections. This paper is a technical focus detailing the four ways to mobilize splenic flexure in robotic surgery. The medial approach involves an extensive dissection of the medial plane separating descending mesocolon form Toldt fascia; the sovramesocolic approach starts with gastrocolic ligament section; the lateral approach starts with coloparietal detachment and the "one inch-one inch" approach starts with section of transverse mesocolon.

INTRODUCTION: Inadequate lymphadenectomy is still a major concern in colon cancer surgery. The superior mesenteric vein (SMV)-first approach is a novel, standardized, reproducible method for robotic complete mesocolic excision surgery.
OBJECTIVE: Our aim was to present the application of the SMV-first approach principles to facilitate robotic salvage surgery for recurrent disease within the mesocolon.
METHODS: A 78-year-old female presented with a malignant lymph node deposit within residual right mesocolonic tissue, approximately 3 months following a laparoscopic right hemicolectomy for col

Seven cases of extraretroperitoneal abdominal malignant fibrous histiocytoma (MFH) are reported. The anatomic distribution was as follows: liver (2 cases), spleen with gastric and pancreatic involvement (1 case), mesentery (1 case), right mesocolon (1 case), left mesocolon (1 case), and urachus (1 case). These bulky lesions were often extensively necrotic; calcifications were seen in 2 cases. All 7 cases were investigated by ultrasonography (US) and computed tomography (CT). Barium studies were performed in 4 cases, and angiography in 3 cases.

INTRODUCTION: We propose a novel technique to close Petersen's defect using barbed sutures and evaluate the safety and usefulness of this technique by assessing postoperative complications and measuring the time required to close Petersen's defect.
MATERIALS AND SURGICAL TECHNIQUE: Petersen's defect was closed laparoscopically with running non-absorbable barbed sutures (V-loc®) after a nodal dissection and reconstruction procedure. First, the transverse colon was elevated cranially, making the dorsal side of the transverse mesocolon a flattened surface. The intersection of the transverse meso

Sigmoid mesocolon hernia is an uncommon type of internal hernia with only a few cases reported to date. This disease entity can progress rapidly to cause vascular disturbance, necrosis, and perforation of the bowel wall; therefore, early diagnosis and surgical treatment are essential. We describe the case of an intra-mesosigmoid hernia in a 60-year-old man without history of previous abdominal surgery who presented with sudden acute abdominal pain and vomiting. Based on computed tomography, which showed ascites and small bowel obstruction, we diagnosed him as having strangulation of the small

A coliky and depressed four year old Arabian mare was examined three days after replacement of a rectal prolapse that had occurred during parturition. Fresh blood was present in the rectum and a septic peritonitis was diagnosed based upon the clinical pathological examination of the peritoneal fluid. At laparotomy the terminal 1.3 meters of small colon was found to be necrotic as a result of rupture of the terminal mesocolon and associated blood vessels. Based upon the poor prognosis the mare was euthanized. Rupture of the mesocolon was thought to be a sequel to the rectal prolapse. The anatom

Spinal ganglia L5-L6 together with mesocolon (innervated from these ganlia) and tongue epithelium were transplanted into the anterior chamber of the cat eye. In 2 months survival of some of the sensory neurons and intensive regeneration of their nerve fibers was observed. At the same time the mesocolon tissues with the mechanoreceptors (Pacinian corpuscles) were found to survive. Destruction of the tongue epithelium was revealed. The trophic effect of definite sensory ganglia on the tissue corresponding to them provided structural intactness of this tissue and of its receptor apparatus. It is

BACKGROUND: Laparoscopic right hemicolectomy with complete mesocolic excision (CME) ensures an oncologically sound resection and aims to optimize long-term outcomes. The D2 and D3 lymphadenectomy techniques remain controversial, with unclear additional benefits from extended lymphadenectomy. This procedure's complexity is heightened by significant vascular variability. We describe the bottom-up approach for laparoscopic right hemicolectomy with CME.
TECHNIQUE: The patient is positioned in lithotomy, and trocars are placed in the suprapubic region. The peritoneum is incised near the ileocaecal

The root of the small-bowel mesentery (SBM) is an important peritoneal fold that is contiguous to other peritoneal ligaments and mesocolons. Several pathologic conditions can occur in the SBM itself, and diseases that spread through the connections from adjacent organs frequently involve it. The root of the SBM is contiguous to the hepatoduodenal ligament around the superior mesenteric vein (SMV) and contiguous to the right side of the transverse mesocolon around the gastrocolic trunk. The inferior mesenteric vein, which is a landmark of the descending mesocolon, runs along the left side of th
