[Post-traumatic metatarsalgia. Early results of treatment with a new insole].

Injuries of the forefoot are a challenge for orthopaedic surgeons. After healing of bone and soft tissue damage, posttraumatic metatarsalgia can persist. The paper presents a therapy algorithm with newly designed insoles, which consist of the common orthopaedic adaptations like a rocker bottom and a stiffening of the sole. The therapy is shown on two cases. With these orthotics adequate is possible and pain relief realistic.

Iatrogenic transfer metatarsalgia after hallux valgus surgery: a comprehensive treatment algorithm.

Current literature has described many of the complications following hallux valgus surgery and their treatment options. Iatrogenic transfer metatarsalgia is a distinctive and challenging complication that has not been addressed in a comprehensive fashion yet. Iatrogenic transfer metatarsalgia may result from poor preoperative assessment, planning and/or surgical technique. We have classified the causes of iatrogenic transfer metatarsalgia based on a multiplanar assessment of the malalignment(s) and are recommending a comprehensive treatment algorithm to guide surgeons in addressing this compli

[Clinical results of treatment of mechanical metatarsalgia without first metatarsal involvement].

OBJECTIVE: To analyze the clinical results of metatarsalgia treated with percutaneous distal metatarsal metaphyseal osteotomies (DMMO). MATERIAL AND METHODS: Prospective and continuous study of 29 patients with isolated unilateral mechanical metatarsalgia, operated on with DMMO, with a mean follow-up of 38 months. The mean age of the patients was 58 (range: 50-67) years and only two men. We use the AOFAS score, we obtained the digital and metatarsal formulas on the X-rays. We record the subjective assessment of the surgeon and the patient at the end of the treatment. RESULTS: The improvement

Asymmetric "V" osteotomy: a predictable surgical approach for chronic central metatarsalgia.

Chronic metatarsalgia is a condition found through the years to present many treatment difficulties. These difficulties began with a lack of understanding, not only concerning the etiologic nature of this condition, but failure to appreciate the surgical treatment ramifications inherent to alterations in the metatarsal parabola. These failures have subsequently led to lesions under the adjacent metatarsal heads, stress fractures, flail toes, and other postoperative pathology. This often leads to further surgical intervention or accommodative modifications. The authors offer an analysis of surg

[Weil's cervicocapital osteotomy for median metatarsalgia. Report of 70 cases].

The authors report a series of Weil's cervicocapital metatarsal osteotomies which were performed to treat central metatarsalgias. The series included 70 central metatarsalgias treated by osteotomy of one to four metatarsals. There was an excess of length of one or several metatarsals in all cases; there were 30 metatarsophalangeal dislocations. The results were evaluated according to Kitaoka's criteria: 20 were quoted very good, 26 good, 9 fair and 3 poor. The osteotomy gave an overall improvement regarding pain and shoe fitting but the mobility of the MP joint was reduced in all cases. The re

Sensory conduction velocity of plantar digital nerves in Morton's metatarsalgia.

The orthodromic sensory conduction velocity of the plantar interdigital nerves were measured with needle electrodes in 28 healthy subjects. The stimulating needle electrodes were placed close to the shaft of the proximal phalanx of the toe, and the recording needle electrode was placed behind the medial malleolus. We examined six subjects with clinical symptoms of entrapment of the common plantar digital nerve (Morton's metatarsalgia). Five of the subjects later had surgery. The conduction velocities of the affected interdigital nerves were abnormally slow. The method is technically simple, bu

Gait biomechanics in individuals with chronic metatarsalgia: A case-control study.

Most treatments for chronic metatarsalgia (CM) focus on reducing forefoot plantar pressure. However, no study has yet identified a link between CM and metatarsal heads overload during level walking. Characterizing the differences in plantar pressure, and lower limb angles and moments in individuals with CM would help better understand the underlying biomechanical deficits associated with CM and developing a more deficit-targeted treatment. The main objectives were to determine whether individuals with CM exhibit greater pressure under the metatarsal heads, reduced ankle pronation, and increase

Gastrocnemius release procedures in the treatment of mechanical metatarsalgia: a systematic review.

Mechanical metatarsalgia (MM) is a condition marked by forefoot overload, causing pain and functional limitations. While conservative treatments like insoles are often effective, surgery is considered when these fail. Traditional approaches focus on the midfoot or forefoot; however, gastrocnemius release (GR) has emerged as an alternative due to the link between equinus contractures and forefoot overload. This systematic review examines the role of GR in MM treatment, analyzing five studies that met the inclusion criteria. These studies evaluated outcomes following either minimally invasive pr

FACTORS INFLUENCING RESULTS OF SURGICAL TREATMENT OF METATARSALGIA AND THEIR PROGNOSTIC VALUE.

Objective - to determine the prognostic value of factors that affect the long-term result of surgical treatment of metatarsalgia, to develop a system for predicting the results of surgical treatment of metatarsalgia. A prospective trial of long-term results of surgical treatment of 172 patients with metatarsalgia was conducted from 2000 to 2019. Two study groups were formed: the core study group comprising of 107 patients who underwent Weil-osteotomy, metatarsals proximal osteotomy. The control group consisted of 65 patients who underwent another surgical treatment (metatarsophalangeal resecti

Plantar pressure analysis of accommodative insole in older people with metatarsalgia.

Foot pain frequently reduces physical activity and increases the risk of falls in older people. In current orthotic management of forefoot pain, metatarsal padding is the main strategy to reduce metatarsal pressure. However, pressure reductions are usually diverse and limited. The multi-step accommodative insole is fabricated by sequential foam padding on Plastazote under dynamic accommodation in daily walking. The aims of this study were to investigate the effectiveness and mechanisms of accommodative insole on plantar pressure redistribution in older people with metatarsalgia. The study was

Helal metatarsal osteotomy for the treatment of metatarsalgia: a critical analysis of results.

We reviewed the results of treatment of 114 feet in 96 patients with pressure metatarsalgia treated with a metatarsal osteotomy performed according to the technique of Helal. Clinical examination was performed according to a standardized evaluation questionnaire using the American Orthopaedic Foot and Ankle Society Lesser Metatarsophalangeal-Interphalangeal Scale. The results were correlated with number of metatarsal osteotomies, the length of the follow up, the age of the patient, and type of additional procedures performed. Sixty-one percent of the patients rated the operation as excellent o

[Metatarsalgia and modified Helal's operation. Apropos of a series of 40 cases].

The authors report a series of 40 cases of metatarsalgia operated according to a modified Helal technique. The mid-diaphyseal osteotomy was situated obliquely inferoanteriorly at an angle of 45 degrees on the metatarsal diaphysis. Twenty cases of hallux valgus were operated during the same procedure Surgery allowed a reduction in pain and hyperkeratosis in 67.5% of patients. Comparison of the results obtained with those reported in the literature confirms that the osteotomy must be metaphyseal and that weight bearing must be early and effective.

Effect of foot orthoses on the biomechanics of stair ascent and descent in chronic metatarsalgia.

In individuals with musculoskeletal disorders such as chronic metatarsalgia, foot orthoses (FOs) are frequently prescribed to reduce plantar pressure, improve foot function and alleviate pain. During walking, medially wedged FOs (MWFO + MPs) with a metatarsal pad are more effective than standard FOs (SFOs) in reducing plantar pressure under the metatarsal heads. As stair ambulation involves higher physical demands than walking and modifies plantar loading, we questioned whether FOs remain effective during more challenging tasks. This study aimed to determine whether MWFO + MPs reduce peak plan

Impact of different foot orthoses on gait biomechanics in individuals with chronic metatarsalgia.

BACKGROUND: Foot orthoses (FOs) are commonly prescribed to reduce pain and improve function in individuals with musculoskeletal disorders, including those with chronic metatarsalgia (CM). Reducing the mechanical overload under the metatarsal heads during locomotion is the central point of the treatment for CM. Medially wedged FOs (MWFOs) with a metatarsal pad could further reduce pressure loading under the metatarsal heads and modify foot and ankle biomechanics compared to standard FOs (SFOs). RESEARCH QUESTION: Do MWFOs further decrease the peak plantar pressure under the metatarsal heads in
