Coronary pathophysiology in idiopathic pulmonary arterial hypertension.

BACKGROUNDIdiopathic pulmonary arterial hypertension (IPAH) alters right ventricular size and function, curtailing life expectancy. Patients may experience angina and myocardial ischemia. However, the underlying mechanisms are poorly understood.METHODSThis study had a cross-sectional, case-control design. Patients with IPAH undergoing right heart catheterization were prospectively enrolled and underwent functional testing during coronary angiography using a dual pressure/temperature-sensitive guidewire. Cardiovascular MRI measured left and right ventricular mass and function. Right ventricular

Primary heart failure with preserved ejection fraction: The intertwined pathophysiology and treatment of some of the most relevant phenotypes.

Heart failure with preserved ejection fraction (HFpEF) is characterized by diverse underlying pathophysiological mechanisms and can be divided into two subgroups based on the identification of the specific cause: primary and secondary HFpEF. Primary HFpEF is caused by primary impairments in myocardial relaxation or compliance with the contribution of several risk factors. Therefore, we have reviewed current literature on pathophysiology and treatment in patients with primary HFpEF. Patients with primary HFpEF represent specific "phenotypes" and are usually elderly, more commonly women, and oft

Renovascular hypertension and ischemic nephropathy.

Renovascular disease remains among the most prevalent and important causes of secondary hypertension and renal dysfunction. Many lesions reduce perfusion pressure including fibromuscular diseases and renal infarction, but most are caused by atherosclerotic disease. Epidemiologic studies establish a strong association between atherosclerotic renal-artery stenosis (ARAS) and cardiovascular risk. Hypertension develops in patients with renovascular disease from a complex set of pressor signals, including activation of the renin-angiotensin system (RAS), recruitment of oxidative stress pathways, an

Metabolic reprogramming in diabetic panvascular disease: Molecular mechanism and therapeutic strategies.

Diabetes mellitus substantially increases the risk of cardiovascular, cerebrovascular, renal, retinal and peripheral vascular complications. These are collectively referred to as diabetic panvascular disease. A traditional approach that treats each complication separately is no longer sufficient. An emerging framework places metabolic reprogramming at its center. This review examines how the diabetic environment, including hyperglycemia, insulin resistance, and dyslipidemia, rewires core metabolic pathways in vascular cells. Major changes include the diversion of glucose through the polyol, he

Predicting the progression of proliferative diabetic retinopathy: Pathophysiology, imaging phenotypes, and determinants of disease persistence despite therapy.

Proliferative diabetic retinopathy (PDR) is a leading cause of severe vision loss in working-aged adults and represents the end stage of chronic neurovascular injury in diabetes. Despite advances in screening and treatment, including panretinal photocoagulation (PRP), intravitreal anti-vascular endothelial growth factor (anti-VEGF) agents, and pars plana vitrectomy (PPV), outcomes remain heterogeneous: many eyes stabilise, whereas others progress to vitreous hemorrhage, tractional retinal detachment, or neovascular glaucoma despite apparently adequate therapy. This review synthesizes current k

From AKI to CKD mechanisms of maladaptive repair and disease progression.

Acute kidney injury (AKI) and chronic kidney disease (CKD) were traditionally regarded as distinct clinical entities, largely defined by the reversibility or irreversibility of renal dysfunction. However, growing clinical and experimental evidence now supports a continuum model in which AKI serves as a major initiating event for subsequent CKD development and progression. Even when conventional indices suggest functional recovery, persistent subclinical injury may drive long-term structural, metabolic, and transcriptional alterations that predispose the kidney to chronic damage. In this review

Temporal Mapping of CSVD-Related White Matter Lesions and Concurrent Neurovascular Dysfunction in Spontaneously Hypertensive Rats.

OBJECTIVE: To define the critical onset window and mechanisms of white matter lesions (WMLs) in cerebral small vessel disease based on spontaneously hypertensive rat (SHR). METHODS: SHRs and age-matched Wistar-Kyoto (WKY) rats were assessed from 20 to 36 weeks of age. Systolic blood pressure, beam-walking performance, and magnetic resonance imaging (MRI)-based white matter changes were evaluated at 4-week intervals. At each time point, randomly selected animals were euthanized for histological analysis. Gait performance and novel object recognition were further assessed at 36 weeks of age. Ad

LRP-6 is a coreceptor for multiple fibrogenic signaling pathways in pericytes and myofibroblasts that are inhibited by DKK-1.

Fibrosis of vital organs is a major public health problem with limited therapeutic options. Mesenchymal cells including microvascular mural cells (pericytes) are major progenitors of scar-forming myofibroblasts in kidney and other organs. Here we show pericytes in healthy kidneys have active WNT/β-catenin signaling responses that are markedly up-regulated following kidney injury. Dickkopf-related protein 1 (DKK-1), a ligand for the WNT coreceptors low-density lipoprotein receptor-related proteins 5 and 6 (LRP-5 and LRP-6) and an inhibitor of WNT/β-catenin signaling, effectively inhibits pericy

Beyond Inflammation: The NLRP3 Inflammasome as a Cellular and Mechanical Hub of Vascular Remodeling in Hypertension.

Hypertension is sustained not only by abnormal vascular tone but also by structural and mechanical remodeling of resistance and conduit arteries. Although innate immune activation is recognized in hypertension, how inflammatory sensing becomes a stable vascular phenotype remains unclear. The NLR family pyrin domain-containing 3 (NLRP3) inflammasome responds to neurohumoral, ionic, metabolic, mitochondrial and mechanical stress signals and may provide this missing link. This review develops a cell-resolved model in which NLRP3 activation in endothelial cells, vascular smooth muscle cells, myelo

Endothelial-to-Mesenchymal Transition in Post-Myocardial Infarction Fibrosis: A Maladaptive but Targetable Pathway.

Myocardial infarction (MI) initiates a healing response in which fibroblasts and other cells deposit extracellular matrix to form a stabilizing scar. This scarring is essential for preventing ventricular rupture, yet when excessive or diffuse, it becomes maladaptive: fibrosis stiffens the ventricle, impairs filling, and drives progression to heart failure. Traditional antifibrotic approaches, such as broad TGF-β blockade or collagen cross-linking inhibition, have largely failed because fibroblast activity is required for early scar integrity, while established fibrosis is difficult to reverse.

Targeted VEGFA therapy in regulating early acute kidney injury and late fibrosis.

Damage to peritubular capillaries is a key process that contributes to acute kidney injury (AKI) progression. Vascular endothelial growth factor A (VEGFA) plays a critical role in maintaining the renal microvasculature. However, the physiological role of VEGFA in various AKI durations remains unclear. A severe unilateral ischemia‒reperfusion injury model was established to provide an overview of VEGFA expression and the peritubular microvascular density from acute to chronic injury in mouse kidneys. Therapeutic strategies involving early VEGFA supplementation protecting against acute injury an

Quercetin Reverses Cardiac Systolic Dysfunction in Mice Fed with a High-Fat Diet: Role of Angiogenesis.

Global consumption of high-fat diets (HFD) is associated with an increased incidence of cardiometabolic syndrome and cardiac injury, warranting identification of cardioprotective strategies. Cardioprotective effects of quercetin (Q) have mostly been evaluated in ischemic heart disease models and attributed to senolysis. We hypothesized that Q could alleviate murine cardiac damage caused by HFD by restoring the myocardial microcirculation. C57BL/6J mice were fed standard chow or HFD for 6 months and then treated with Q (50 mg/kg) or vehicle 5-day biweekly for 10 additional weeks. Left ventricul

Aberrant production of extracellular matrix proteins and dysfunction in kidney endothelial cells with a short duration of diabetes.

Diabetic nephropathy is the most common cause of end-stage renal disease and is a major risk factor for cardiovascular disease. In the United States, microvascular complications during diabetic nephropathy contribute to high morbidity and mortality rates. However, the cell-autonomous impact of diabetes on kidney endothelial cell function requires further investigation. Male Akita/+ [autosomal dominant mutation in the insulin II gene (Ins2)] mice reproducibly develop diabetes by 4 wk of age. Here, we examined the impact a short duration of diabetes had on kidney endothelial cell function. Kidne

A comprehensive and longitudinal cardiac magnetic resonance imaging study of the impact of coronary ischemia duration on myocardial damage in a highly translatable animal model.

OBJECTIVES: We performed a comprehensive assessment of the effect of myocardial ischemia duration on cardiac structural and functional parameters by serial cardiac magnetic resonance (CMR) and characterized the evolving scar. BACKGROUND: CMR follow-up on the cardiac impact of time of ischemia in a closed-chest animal model of myocardial infarction with human resemblance is missing. METHODS: Pigs underwent MI induction by occlusion of the left anterior descending (LAD) coronary artery for 30, 60, 90 or 120 min and then revascularized. Serial CMR was performed on day 3 and day 42 post-MI. CMR

Blockade of cysteine-rich protein 61 attenuates renal inflammation and fibrosis after ischemic kidney injury.

Emerging data have suggested that acute kidney injury (AKI) is often incompletely repaired and can lead to chronic kidney disease (CKD), which is characterized by tubulointerstitial inflammation and fibrosis. However, the underlying mechanisms linking AKI to CKD remain obscure. The present study aimed to investigate the role of cysteine-rich protein 61 (Cyr61) after unilateral kidney ischemia-reperfusion injury (IRI) in mice. After IRI, increased expression of Cyr61 was detected, predominately in the proximal tubular epithelium. This was confirmed by in vitro experiments, which showed that hyp

Percutaneous revascularization for ischemic nephropathy: the past, present, and future.

Occlusion of the renal arteries can threaten the viability of the kidney when severe, in addition to accelerating hypertension and circulatory congestion. Renal artery stenting procedures have evolved from a treatment mainly for renovascular hypertension to a maneuver capable of recovering threatened renal function in patients with 'ischemic nephropathy' and improving management of congestive heart failure. Improved catheter design and techniques have reduced, but not eliminated, hazards associated with renovascular stenting. Expanded use of endovascular stent grafts to treat abdominal aortic

Effect of high-frequency hearing loss on compound action potentials recorded from the intracranial portion of the human eighth nerve.

Compound action potentials (CAP) were recorded from the exposed intracranial portion of the eighth nerve to stimulation with click sounds in patients with sensorineural high-frequency hearing loss who underwent microvascular decompression (MVD) operations to treat trigeminal neuralgia (TN). In patients with normal hearing the CAP recorded in that way is characterized by a negative peak, preceded by a small positivity and followed by a positivity and sometimes a second negative peak. In patients with high-frequency hearing loss the CAP also usually had an initial sharp negative peak in response

Sirtuin 3 Alleviates Diabetic Cardiomyopathy by Regulating TIGAR and Cardiomyocyte Metabolism.

Background Impairment of glycolytic metabolism is suggested to contribute to diabetic cardiomyopathy. In this study, we explored the roles of SIRT3 (Sirtuin 3) on cardiomyocyte glucose metabolism and cardiac function. Methods and Results Exposure of H9c2 cardiomyocyte cell lines to high glucose (HG) (30 mmol/L) resulted in a gradual decrease in SIRT3 and 6-phosphofructo-2-kinase/fructose-2,6-bisphosphatase isoform 3 (PFKFB3) expression together with increases in p53 acetylation and TP53-induced glycolysis and apoptosis regulator (TIGAR) expression. Microcirculation in hypertensive patients.

Regardless of the mechanisms that initiate the increase in blood pressure, functional and structural changes in the systemic vasculature are the final result of long-standing hypertension. These changes can occur in the macro- but also in the microvasculature. The supply of the tissues with oxygen, nutrients, and metabolites occurs almost exclusively in the microcirculation (which comprises resistance arterioles, capillaries and venules), and an adequate perfusion via the microcirculatory network is essential for the integrity of tissue and organ function. This review focuses on results from c
