Risk factors for adverse health in military and law enforcement personnel; an umbrella review.

BACKGROUND: A better understanding of adverse health and underlying risk factors in the context of military and law enforcement is essential to developing future preventative strategies. We conducted an umbrella review to provide an overview of the current literature regarding risk factors for adverse health in military and law enforcement populations. METHODS: We systematically searched the literature for systematic reviews and meta-analyses regarding risk factors for adverse health in military or law enforcement populations. We critically appraised the studies that matched our inclusion cri

A scoping review of military veterans involved in the criminal justice system and their health and healthcare.

BACKGROUND: In the criminal justice system, special populations, such as older adults or patients with infectious diseases, have been identified as particularly vulnerable to poor health outcomes. Military veterans involved in the criminal justice system are also a vulnerable population warranting attention because of their unique healthcare needs. This review aims to provide an overview of existing literature on justice-involved veterans' health and healthcare to identify research gaps and inform policy and practice. METHODS: A systematic search was conducted to identify research articles re

Quality of Care for PTSD and Depression in the Military Health System: Phase I Report.

The U.S. Department of Defense (DoD) strives to maintain a physically and psychologically healthy, mission-ready force, and the care provided by the Military Health System (MHS) is critical to meeting this goal. Given the rates of posttraumatic stress disorder (PTSD) and depression among U.S. service members, attention has been directed to ensuring the quality and availability of programs and services targeting these and other psychological health (PH) conditions. Understanding the current quality of care for PTSD and depression is an important step toward improving care across the MHS. To hel

Prospective evaluation of mental health and deployment experience among women in the US military.

Previous research has shown that military women often experience potentially severe health outcomes following deployment. Data from the Millennium Cohort Study, a 21-year longitudinal study examining the health effects of military service, were used to examine this issue. In longitudinal analyses (2001-2008) carried out among US military women (n = 17,481), the authors examined positive screens for depression, anxiety, panic, and posttraumatic stress disorder in relation to deployment in support of the operations in Iraq and Afghanistan, while adjusting for relevant baseline and time-varying c

Outbreak of acute norovirus gastroenteritis in a military facility in Singapore: a public health perspective.

INTRODUCTION: Norovirus gastrointestinal disease (GID) outbreaks occur frequently in closed settings, with high attack rates. On October 16, 2008, a norovirus GID outbreak occurred at a Singapore military camp. This study describes the epidemiological investigations conducted to determine the cause of outbreak and the efficacy of the public health measures implemented. METHODS: Epidemiologic investigations included a case-control study of exposure to different food items and an environmental exposure survey. Stool samplings of patients and food handlers for common pathogens, and microbiologic

Posttraumatic mental and physical health correlates of forgiveness and religious coping in military veterans.

This study assessed mental and physical health correlates of dispositional forgiveness and religious coping responses in 213 help-seeking veterans diagnosed with PTSD. Controlling for age, socioeconomic status, ethnicity, combat exposure, and hostility, the results indicated that difficulty forgiving oneself and negative religious coping were related to depression, anxiety, and PTSD symptom severity. Difficulty forgiving others was associated with depression and PTSD symptom severity, but not anxiety. Positive religious coping was associated with PTSD symptom severity in this sample. Further i

A Novel Approach to Build Virtual Communities of Practice for Health Care Capacity Building in Military Health Systems Globally.

INTRODUCTION: The HIV/AIDS epidemic poses significant risks to military personnel, impacting operational readiness. For over 2 decades, the U.S. DoD HIV/AIDS Prevention Program (DHAPP) has partnered with international militaries to develop prevention and treatment programs. Military health systems, often in remote areas, face challenges in accessing and implementing updated guidelines, leading to suboptimal care. To address this, DHAPP adopted Project ECHO (Extension for Community Healthcare Outcomes) to establish virtual communities of practice (VCoPs), enhancing health care capacity. DoD HIV

Adversity during military service: the impact of military sexual trauma, emotional bullying and physical assault on the mental health and well-being of women veterans.

INTRODUCTION: Despite making up about 11% of the UK military, there remains limited investigation on the impact of adversity women experience during their service in the UK military. Military adversity can result in a range of well-being difficulties that may persist following transition out of military. The present study therefore examined the prevalence and correlates of different types of military adversity (defined as sexual harassment, sexual assault, emotional bullying and physical assault) within a community sample of UK women veterans. METHODS: Participants were recruited from a UK ch

Dental lessons learned from large-scale military dental records in the Israel defense forces: military dentistry as a window into national oral health trends.

Mandatory military service creates a unique opportunity to examine health patterns in a near-comprehensive national cohort of young adults. In this context, large-scale, routinely collected military health data, particularly from dental records, can serve as a powerful lens through which to explore broader public health phenomena. Recent population-based studies conducted within the Israel Defense Forces have demonstrated how clinical records originally intended to ensure operational readiness can also illuminate associations between health outcomes and social determinants. These findings high

Penetration rates of behavioral health consultants targeting cardiovascular disease and associated modifiable risk factors among military health care beneficiaries.

INTRODUCTION: Targeting cardiovascular disease (CVD) and its associated risk factors is important not only in the general population, but also among those served by the military health care system. The primary care behavioral health model of integrating behavioral health consultants (BHCs) into a primary care setting is one method for enhancing how these modifiable risk factors are addressed; however, it is unknown how often BHCs are used to target those with biopsychosocial factors contributing to CVD and those with diagnosed CVD. METHOD: The present study evaluated what percentage of primar

Relationship among adverse childhood experiences, history of active military service, and adult outcomes: homelessness, mental health, and physical health.

OBJECTIVES: We determined whether a report of adverse childhood experiences predicts adult outcomes related to homelessness, mental health, and physical health and whether participation in active military service influences the relationship between childhood and adult adversity. METHODS: Using data from the 2010 Washington State Behavioral Risk Factor Surveillance System, we tested by means of logistic regression the relationship between adverse childhood experiences and 3 adult outcomes--homelessness, mental health, and physical health--as well as differences among those with a history of ac

Common mental health disorders in military veterans: a quantitative comparison using primary healthcare data.

INTRODUCTION: UK primary healthcare medical records contain codes that define factors such as diagnosis, demographics and military veteran status. This provides an opportunity to use primary healthcare data to identify military veterans' clinical morbidity and compare these results with a non-veteran sample. METHODS: This research aimed to scrutinise UK primary healthcare data to compare the incidence of the mental health disorders of depression, anxiety, alcohol misuse, post-traumatic stress disorder, self-harm and substance misuse in veterans and non-veterans. Data were provided by 13 prima

Mapping the Fetal Alcohol Spectrum Disorder Continuum of Care Across the Military Health System.

INTRODUCTION: Although a leading cause of developmental disability in the United States, many individuals with fetal alcohol spectrum disorders (FASDs) do not receive a timely diagnosis, are misdiagnosed, or are never diagnosed. Prevention, diagnosis, and clinical management of FASD have not been previously studied in the U.S. Military Health System (MHS), where nearly 1 million of the nation's children receive health care. To address this gap, we undertook an environmental scan of the clinical guidelines, services, programs, educational resources, and policies within the MHS pertaining to alc

Differential associations of adverse childhood experiences and mental health outcomes in U.S. military veterans.

Adverse childhood experiences (ACEs) are robustly associated with adverse mental health outcomes across the lifespan. Military veterans may be particularly vulnerable to adverse mental health effects of ACEs given their potentially higher prevalence of childhood traumas and compounding effects of military service-related traumas. To date, however, scarce research has examined the differential impact of individual ACEs on mental health outcomes in population-based samples of veterans. To address this gap, we analyzed data from the National Health and Resilience in Veterans Study (NHRVS), which

Posttraumatic Stress Disorder, Military Sexual Trauma, and Birth Experiences at the Veterans Health Administration.

BACKGROUND: Women are a growing portion of the U.S. veteran population, and every year the Veterans Health Administration (VHA) serves an increasing number of women seeking obstetrics services. Women veterans experience elevated rates of anxiety, depression, posttraumatic stress disorder (PTSD), and traumatic events, including military sexual trauma, as compared with women in the general population. It is possible that mental health disorders may be associated with birth experiences. OBJECTIVES: We investigated the link between anxiety, depression, PTSD, and military sexual trauma (MST; i.e.,
