OBJECTIVE: This report summarizes a review of anaphylactic reactions to cow's milk and beef presented at a recent conference on adverse reactions to bovine proteins.
DATA SOURCES: A review of pertinent PubMed (National Library of Medicine) articles was performed. Relevant publications were critically analyzed.
STUDY SELECTION: The expert opinion of the author was used to select the relevant data for the review.
RESULTS: Although cow's milk has been recognized for many years as one of the leading causes of food allergy, beef has only recently been identified as a cause of immunoglobulin (Ig)

Clinical manifestations of cow's milk allergy rarely occur in the first days after birth. We report on a newborn presenting with hemorrhagic meconium in the first hour of life followed by bloody diarrhea in the next few days. At day 14, an elevated total IgE, specific IgE to cow's milk and an eosinophilia in peripheral blood were found. Symptoms disappeared when the milk feed was changed to an extensively hydrolyzed casein-formula. Two challenges with cow's milk formula (on day 30 and at 7 months of age) were followed by recurrence of vomiting, watery diarrhea and failure to thrive. At the age

PURPOSE OF REVIEW: To review current knowledge and recent advances in food protein-induced enterocolitis syndrome (FPIES).
RECENT FINDINGS: Rice is the most common solid food causing FPIES. Rice FPIES is associated with more severe reactions than other foods. Infants presenting acutely may be hypothermic (<36 degrees C) and have thrombocytosis. Finding of hypoalbuminemia and weight gain less than 10 g/day helps to differentiate chronic infantile cow's milk FPIES from infectious causes. Gastric juice leukocytes more than 10 cells per high-power field are found in infants with positive oral foo

Food allergy is an increasingly prevalent problem in westernized countries, and there is an unmet medical need for an effective form of therapy. A number of therapeutic strategies are under investigation targeting foods that most frequently provoke severe IgE-mediated anaphylactic reactions (peanut, tree nuts, and shellfish) or are most common in children, such as cow's milk and hen's egg. Approaches being pursued are both food allergen specific and nonspecific. Allergen-specific approaches include oral, sublingual, and epicutaneous immunotherapy (desensitization) with native food allergens an

PURPOSE OF REVIEW: To review recent advances in the area of food allergen processing and the effect on protein allergenicity.
RECENT FINDINGS: Heating generally decreases protein allergenicity by destroying conformational epitopes. In peanut and shrimp, heat-induced Maillard reaction (glycation) may increase allergenicity. The majority of milk and egg-allergic children tolerate extensively heated (baked with wheat matrix) milk and egg. Introduction of extensively heated milk and egg proteins is associated with decreasing sizes of skin prick test wheals and increasing serum food-specific IgG4

The prevalent use of antibiotics, e. g. sulphonamides, tetracyclines, beta-lactams, macrolides, aminoglycosides, etc., in dairy farming to prevent and treat microbial infections e. g. mastitis, can be a potential source of veterinary drug residues in milk. Antibiotic residues constitute a risk to human health, since they can cause allergic reactions in hypersensitive individuals or they may lead to the appearance of drug-resistant bacteria. Analysis of these residues plays a key role in ensuring food safety. Regulatory agencies in the European Union and in other countries have established maxi

Immunologically mediated hypersensitivity to foods is defined as food allergy, mainly due to immunglobulins of class E (IgE) triggering immediate reactions (type I hypersensitivity) with possible involvement of mucosa, skin, airways, intestinal tract, and the vascular system. Primary food allergy is based on (early) IgE sensitization against animal (e. g., cow's milk, hen's eggs) or plant proteins (e. g. peanut, hazelnut or wheat). In the case of secondary food allergies, IgE against pollen proteins (e. g., birch) reacts to structurally related food proteins (with cross-reactions to stone and

Background: Cow's milk protein allergy (CMPA) is well described in term infants, as opposed to preterm infants. In preterm infants, CMPA shares many gastrointestinal symptoms with necrotizing enterocolitis (NEC). Objectives: To evaluate the presentation of CMPA in preterm infants and to investigate the different diagnostic and therapeutic options. Materials and Methods: We searched for the relevant literature using the medical databases PubMed, Web of Science, and the Cochrane Library. We performed a post hoc analysis on the 25 case reports included in this study. Results: Literature was scarc

The presence of milk in meals and products consumed daily is common and at the same time the adoption of a milk-free diet increases due to milk allergy, lactose intolerance, vegan diets, and others. Therefore, there is an increasing demand for plant-based beverages, which present variable and, sometimes, unknown nutritional characteristics. This study sought to compare the nutritional aspects of plant-based beverages used as substitutes for cow's milk described in scientific studies. Therefore, we used a review of the scientific literature on PubMed, Google Scholar, Scopus, Web of Science, Goo

INTRODUCTION: Introducing baked milk into the diet of children with cow's milk protein allergy (CMPA) has been shown to potentially accelerate the development of tolerance to non-heated milk. However, there is no standardised milk ladder (ML) protocol, and different scientific societies across countries recommend varying versions. This study aims to assess the effectiveness and safety of the four-step ML (4-ML) compared with the six-step ML (6-ML) in children with IgE-mediated CMPA.
METHODS AND ANALYSIS: We will perform an open-label randomised trial with two parallel arms in two departments

BACKGROUND: Human infants develop IgG responses to dietary antigens during the first 2 years of life. Yet, the source of these antibodies is unclear. In previous studies we reported on the thymus as a unique functional niche for plasma cells (PCs) specific to environmental antigens.
OBJECTIVE: We sought to examine whether PCs specific to dietary antigens are detected in the infant thymus.
METHODS: We tested IgG reactivity to 112 food antigens and allergens in the serum of 20 neonates and infants using microarrays. The presence of PC-secreting IgG specific to the most prominent antigens was t

Cow milk (CM) allergy is a worldwide concern. Currently, few studies have been performed on the immunoreactivity of CM and fewer still on the antigenicity of CM in vivo and in vitro. In this study, we assessed the potential allergenicity of enzymatically hydrolyzed CM using in vitro ELISA and oral sensitization and challenge of BALB/c mice. Alcalase-, Protamex-, and Flavourzyme-treated CM (all from Novozymes) diminished IgE binding capacity, with greatest reductions of 56.31%, 50.62%, and 56.45%, respectively. Allergic symptoms and levels of total IgG1 were reduced, and allergic inflammation o

A 9-month-old infant with a personal history of atopic dermatitis and no family history of atopy presented to the outpatient allergy clinic for evaluation of suspected food and drug allergy. The mother breastfed the infant from birth and had introduced only fruits and vegetables into his diet. After the first dose of Ferplex® (iron protein succinylate), the infant had an immediate moderate systemic reaction that required emergency care and treatment with intramuscular epinephrine. The drug was indicated for the treatment of acute anemia. Skin prick test and specific IgE were done, confirming s

This guideline advises on the management of patients with cow's milk allergy. Cow's milk allergy presents in the first year of life with estimated population prevalence between 2% and 3%. The clinical manifestations of cow's milk allergy are very variable in type and severity making it the most difficult food allergy to diagnose. A careful age- and disease-specific history with relevant allergy tests including detection of milk-specific IgE (by skin prick test or serum assay), diagnostic elimination diet, and oral challenge will aid in diagnosis in most cases. Treatment is advice on cow's milk

BACKGROUND: Helminths induced eosinophilia is well known. Recent studies have been demonstrating that Giardia Lamblia is also able to cause eosinophilia and suggest that children with giardiasis have a higher risk of food allergy.
CASE REPORT: Newborn premature male patient, admitted to neonatology unit since birth, who presented persistent eosinophilia, associated with vomiting and abdominal distention when he ingested a not hydrolyzed milk formula. The subsequent study revealed milk allergy (grade II/III) but also of Giardia on stool. The child was treated with metronidazol with clinical im

Atopic dermatitis (AD) is a chronic, relapsing, inflammatory skin disease. Children with AD tend to have a higher prevalence of food allergies. This study investigated the clinical significance of food sensitization in AD patients. A total of 266 AD patients participated in this study. The prevalence of food sensitization and clinically relevant sensitization were compared in the subjects according to their age and AD severity. Sera from all patients were analyzed for food-specific IgE levels using the Pharmacia CAP System FEIA. The serum specific IgE levels for egg, milk, peanut and soybean w
