Acceptance and commitment therapy for moral injury (ACT-MI) and present-centered therapy for moral injury (PCT-MI): Results from a randomized controlled pilot trial with war zone deployed veterans.

Acceptance and commitment therapy for moral injury (ACT-MI) and present-centered therapy for moral injury (PCT-MI) were developed and piloted through the study. ACT-MI was created to improve psychosocial functioning among veterans experiencing moral injury. PCT-MI was developed as a transdiagnostic process-based moral injury intervention and an active comparison condition for a future ACT-MI efficacy trial. A randomized controlled pilot trial was conducted where 74 war zone-deployed veterans were randomized to ACT-MI or PCT-MI. Aims were to test the acceptability of ACT-MI and PCT-MI, evaluate

The prevalence and long-term effects of PTSD and moral injury in Swedish military veterans.

INTRODUCTION: In the context of international military operations, officers and soldiers are exposed to various service-related stressors that may have long-lasting effects on their health and daily functioning. This study explored (1) the prevalence of symptoms indicative of both post-traumatic stress disorder (PTSD) and moral injury (MI), (2) the relationship between these conditions, and (3) the relationship between these conditions and a selection of background variables in Swedish military veterans who have previously been deployed in operations. METHODS: The study was a self-report surv

Triage ethics in mass casualty incident simulation: A phenomenological exploration.

BackgroundDisaster scenarios challenge both novice and experienced nurses to navigate complex ethical dilemmas in resource-limited environments. Traditional nursing education often leaves new nurses unprepared for the ethical demands of disaster nursing. Utilitarianism must often guide triage ethics and decision-making. There is a critical need to equip nursing students with these ethical competencies.Research question/AimThis study explores nursing students' lived experiences using introductory triage ethics in mass casualty incident simulation (MCIS).Research designA qualitative, interpretiv

[Eduard Reich's "Hygiene of Culture"-A retrospective on the occasion of the 100th anniversary of his death].

In contrary to the developing significance of the bacteriology and the chemical and physical noxious influences of the environment for Eduard Reich the main cause of decision between health and illness are the functions of the "soul". Components of this as important for the normal life are virtue (i. e. solidarity, performance of duty, self-knowledge), good sense to realize correlations, morality, charity and mercifulness (social behaviour). The goal of the hygiene therefore is to promote the good and to hinder the evil, as well for a complete person or only one of his organs as for the whole

Moral discourses and pharmaceuticalised governance in households.

This article extends our understanding of the everyday practices of pharmaceuticalisation through an examination of moral concerns over medication practices in the household. Moral concerns of responsibility and discipline in relation to pharmaceutical consumption have been identified, such as passive or active medication practices, and adherence to orthodox or unorthodox accounts. This paper further delineates dimensions of the moral evaluations of pharmaceuticals. In 2010 and 2011 data were collected from 55 households across New Zealand and data collection techniques, such as photo- and dia

Moral dilemma of family caregivers of urostomy patients: a qualitative study.

BACKGROUND: The most prevalent malignant tumor of the urinary system, bladder cancer, exhibits a rising global incidence rate. However, recent diagnostic and therapeutic techniques, such as ostomy surgery, can effectively prolong patient survival. The majority of patients require support from family carers, presenting considerable challenges to these caregivers. Although existing research has primarily focused on patients' physiological adaptation, quality of life, and the burden on family carers, the moral dilemmas faced by these caregivers have not been explored yet. PURPOSE: This study aim

Moral disengagement in psychiatric inpatient care for individuals who engage in self-injury: a conceptual framework for understanding staff responses and patient experiences.

Psychiatric inpatient care for individuals who engage in self-injury involves tensions between preventing harm, respecting autonomy, sustaining relational care, and managing organizational demands. Staff may experience emotional burden and moral distress, while patients may experience care as invalidating, coercive, or insufficiently responsive to their needs. This article develops a theory-informed conceptual framework applying Albert Bandura's theory of moral disengagement to psychiatric inpatient care for self-injury. Drawing on an interpretive synthesis of Bandura's theory and clinical lit

Influence of interpersonal violence on maternal anxiety, depression, stress and parenting morale in the early postpartum: a community based pregnancy cohort study.

BACKGROUND: Research has shown that exposure to interpersonal violence is associated with poorer mental health outcomes. Understanding the impact of interpersonal violence on mental health in the early postpartum period has important implications for parenting, child development, and delivery of health services. The objective of the present study was to determine the impact of interpersonal violence on depression, anxiety, stress, and parenting morale in the early postpartum. METHODS: Women participating in a community-based prospective cohort study (n = 1319) completed questionnaires prior t

A matter of respect: a defense of the dead donor rule and of a "whole-brain" criterion for determination of death.

Many accounts of the historical development of neurological criteria for determination of death insufficiently distinguish between two strands of interpretation advanced by advocates of a "whole-brain" criterion. One strand focuses on the brain as the organ of integration. Another provides a far more complex and nuanced account, both of death and of a policy on the determination of death. Current criticisms of the whole-brain criterion are effective in refuting the first interpretation, but not the second, which is advanced in the 2008 President's Council report on the determination of death.

Between Cradles and Compassion: A Conditional Obligation to Adopt Instead of Procreating.

Such prominent authors as Tina Rulli have argued in favour of pro tanto duties to adopt instead of procreating. This article interrogates-and then subsequently extends support for-such a view. Specifically, this paper develops and defends a conditional moral obligation to adopt rather than procreate for agents who intend to become parents by drawing on antinatalist philosophy. It argues that even non-antinatalists should recognize what I call "antinatalistic pressures": reasons arising from the harms and risks of existence that shift the justificatory burden onto would-be procreators. I formul

The Long Tail of COVID-19: Implications for the Future of Emergency Nursing.

INTRODUCTION: COVID-19 has led to exacerbated levels of traumatic stress and moral distress experienced by emergency nurses. This study contributes to understanding the perspectives of emergency nurses' perception of psychological trauma during COVID-19 and protective mechanisms used to build resilience. METHOD: The primary method was qualitative analysis of semistructured interviews, with survey data on general resilience, moral resilience, and traumatic stress used to triangulate and understand qualitative findings. Analyses and theme development were guided by social identity theory and in

[Clinical everyday ethics-support in handling moral distress? : Evaluation of an ethical decision-making model for interprofessional clinical teams].

BACKGROUND: High-tech medicine and cost rationing provoke moral distress up to burnout syndromes. The consequences are severe, not only for those directly involved but also for the quality of patient care and the institutions. The multimodal model METAP (Modular, Ethical, Treatment, Allocation, Process) was developed as clinical everyday ethics to support the interprofessional ethical decision-making process. The distinctive feature of the model lays in education concerning ethics competence in dealing with difficult treatment decisions. METAP has been evaluated for quality testing. METHODS:

Managing yourself. Stop overdoing your strengths.

Although most managers can recognize an off-kilter leader (consider the highly supportive boss who cuts people too much slack), it's quite difficult to see overkill in yourself. Unfortunately, that's where leadership development tools such as 360-degree surveys fail to deliver, say Kaplan and Kaiser. Dividing qualities into "strengths" and "weaknesses" and rating them on a five-point scale will not account for strengths overplayed. The authors suggest several strategies, based on their years of consulting experience and research, for figuring out which attributes you've employed to excess and

Ethics and the proposed treatment for a 13-year-old with atypical gender identity.

The case of a 13-year-old girl given permission by the Family Court of Australia to begin a sex-change process involves complex issues. Nevertheless, the ethical justification for the decision is not complicated. In this case, it can be argued that the net benefit eclipses concerns about competence, autonomy and the appropriateness of the intervention. The debate this case generated in the media reminds us that one of the essential tasks in ethics debates is to get our facts straight.

Pharmacy staff training and development: upside-down thinking in a changing profession.

We suggest that the most fundamental change in staff development that must occur is recognition of the need for a professional belief system as the basis for any pharmaceutical care activity. Values derived from fundamental moral ideals and professional beliefs foster the development of attitudes and behaviors. It would be wrong to suggest or imply that such a change need only occur in postbaccalaureate training. The development of personal and professional value systems in existing primary professional training programs is inadequate--we do not yet do enough to develop people before they ente

Tragic Competence: A Third Structure in Physician Distress.

RATIONALE: Physician distress is conventionally described as burnout and addressed through resilience, or as moral injury and addressed through systemic reform. Both frameworks assume that distress follows from a known right action being blocked or a value betrayed. Some of the distress of clinical work has a third structure: clinicians are sometimes troubled not because a value has been lost but because several legitimate goods make claims that cannot all be met, so a defensible choice may still leave a genuine good unrealised. Neither existing framework names this structure, and medicine has

Human research ethics committees members: ethical review personal perceptions.

This study aims to characterise Human Research Ethics Committee (HREC) members' perceptions on five main themes associated with ethics reviews, namely, the nature of research, ethical/moral issues, assent, participants' risk and HREC prerogatives issues. Three hundred and sixteen HREC members from over 200 HRECs throughout Australia responded to an online questionnaire survey. The results show that in general, HREC members' beliefs are reasoned and align with sound principles of ethical reviews. There seems to be a disposition for living up to ethical/moral values, avoiding the issue of consen
