Minimally invasive biomarkers for triaging lung nodules-challenges and future perspectives.

CT chest scans are commonly performed worldwide, either in routine clinical practice for a wide range of indications or as part of lung cancer screening programs. Many of these scans detect lung nodules, which are small, rounded opacities measuring 8-30 mm. While the concern about nodules is that they may represent early lung cancer, in screening programs, only 1% of such nodules turn out to be cancer. This leads to a series of complex decisions and, at times, unnecessary biopsies for nodules that are ultimately determined to be benign. Additionally, patients may be anxious about the status of

Pulmonary nodules in children.

Clinicians and radiologist nowadays frequently encounter pulmonary nodules in children, thanks to the widespread use of computed tomography (CT) thorax. Most pulmonary nodules are benign; however, a small number of pulmonary nodules indicate pulmonary malignancy in children, requiring prompt diagnosis and treatment. Incidentally diagnosed pulmonary nodules are common and naturally cause anxiety in families and in clinicians, leading to successive examinations. For this reason, the creation of algorithms for the diagnosis and follow-up of pulmonary nodules, and the definition of advanced imagin

Reducing False-Positives in Lung Nodules Detection Using Balanced Datasets.

Malignant pulmonary nodules are one of the main manifestations of lung cancer in early CT image screening. Since lung cancer may have no early obvious symptoms, it is important to develop a computer-aided detection (CAD) system to assist doctors to detect the malignant pulmonary nodules in the early stage of lung cancer CT diagnosis. Due to the recent successful applications of deep learning in image processing, more and more researchers have been trying to apply it to the diagnosis of pulmonary nodules. However, due to the ratio of nodules and non-nodules samples used in the training and test

Computed Tomography-Guided Methylene Blue Localization: Single vs. Multiple Lung Nodules.

Background: Preoperative localization for small invisible and impalpable pulmonary nodules is important in single-port video-assisted thoracoscopic surgery (VATS). Localization of multiple pulmonary nodules during VATS resection remains challenging. The aim of our study is to elucidate the efficacy of preoperative CT-guided methylene blue localization of both single and multiple pulmonary nodules. Methods: Consecutive patients undergoing preoperative CT-guided methylene blue dye localization for lung nodules, followed by VATS resection, were retrospectively analyzed between January 2014 and No

Pulmonary hyalinizing granuloma associated with idiopathic thrombocytopenic purpura.

Pulmonary hyalinizing granuloma (PHG) is a rare, benign lung disease of unknown etiology. It manifests as discrete, rounded nodules within the lung parenchyma. A 39-year-old woman presented for investigation after pulmonary nodules were found incidentally. Chest computed tomography showed multiple, discrete, non-enhancing pulmonary nodules bilaterally. Positron emission tomography (PET) was negative. Biopsy demonstrated a non-specific lymphoplasmacytic infiltrate. Open resection yielded two nodules consistent with hyalinizing granulomas. The differential for multiple pulmonary nodules is broad

Rheumatoid arthritis with necrotic lung nodules.

Rheumatoid arthritis is a multisystemic in˜flammatory disease. Lungs are the commonest site of extra-articular involvement. Rheumatoid lung nodules occur infrequently and can undergo necrosis giving rise to necrobiotic lung nodules. Infections, malignancy and granulomatosis with polyangiitis are more common causes of cavitating lung nodules. Presence of rheumatoid factor, history of smoking and use of methotrexate increase the chances of developing rheumatoid lung nodulosis. Histopathological examination of the nodule is essential to make a correct diagnosis. We present a 74-year-old male with

CT-guided Preoperative Localization of Pulmonary Nodules Using a Glucose Test and Tissue Adhesive.

Intraoperative localization of ground-glass nodules (GGNs) is challenging. This study evaluates a CT-guided protocol that innovatively combines a glucose solution test with tissue adhesive marking to enhance safety and efficacy. The standardized procedure involves preoperative thin-slice CT (≤1 mm) with a 5 × 5 cm grid for planning, followed by percutaneous puncture to a shallow (5 mm) subpleural depth. A key safety innovation is the injection of 0.3 mL of 5% glucose solution to confirm extra-bronchial needle position via cough reflex testing before depositing 0.3 mL of 2-octyl cyanoacrylate t

Efficacy and Safety of Cone-Beam CT Augmented Electromagnetic Navigation Guided Bronchoscopic Biopsies of Indeterminate Pulmonary Nodules.

Bronchoscopic biopsy results for indeterminate pulmonary nodules remain suboptimal. Electromagnetic navigation bronchoscopy (ENB) coupled with cone beam computed tomography (CBCT) for confirmation has the potential to improve diagnostic yield. We present our experience using this multimodal approach to biopsy 17 indeterminate nodules in 14 consecutive patients from April to August 2021. Demographic information, nodule characteristics, and biopsy results were recorded. Procedures were performed in a hybrid operating room equipped with a Siemens Artis Q bi-plane CBCT (Siemens, Munich, Germany).

Multi-Modal Feature Fusion-Based Multi-Branch Classification Network for Pulmonary Nodule Malignancy Suspiciousness Diagnosis.

Detecting and identifying malignant nodules on chest computed tomography (CT) plays an important role in the early diagnosis and timely treatment of lung cancer, which can greatly reduce the number of deaths worldwide. In view of the existing methods in pulmonary nodule diagnosis, the importance of clinical radiological structured data (laboratory examination, radiological data) is ignored for the accuracy judgment of patients' condition. Hence, a multi-modal fusion multi-branch classification network is constructed to detect and classify pulmonary nodules in this work: (1) Radiological data o

A case of multiple pleural cryptococcosis without pleural effusion.

Pulmonary cryptococcosis is most likely to occur in immunocompromised patients. The radiological manifestations generally include pulmonary parenchymal lesions, namely, pulmonary nodules, cavitary lesions, and consolidation; thus, multiple pleural nodules are unusual presentation. Here, we report a woman who presented with multiple pleural cryptococcosis without pleural effusion. The patient had previously undergone surgery for stage II rectal cancer. In addition, she received 6 cycles of chemotherapy for follicular lymphoma. Computed tomography (CT) revealed multiple small nodules involving t
