Assessment of Health Care Costs and Total Baclofen Use Associated With Targeted Drug Delivery for Spasticity.

BACKGROUND: Chronic spasticity causes significant impairment and financial burden. Oral baclofen, the first-line therapy, can have intolerable, dose-dependent side effects. Targeted drug delivery (TDD) through intrathecal baclofen delivers smaller amounts of baclofen into the thecal sac via an implanted infusion system. However, the health care resource utilization of patients with spasticity receiving TDD has not been studied extensively. MATERIALS AND METHODS: Adult patients who received TDD for spasticity between 2009 and 2017 were identified using the IBM MarketScan® data bases. Patients'

Effects of intrathecal baclofen therapy in subjects with disorders of consciousness: a reappraisal.

Baclofen is a structural analogue of gamma-amino-butyric acid (GABA), which reduces spastic hypertonia of striated muscle due to a mechanism of GABAB-ergic inhibition of mono- and polysynaptic reflexes at the spinal level. There are reports of patients with severe disorders of consciousness that presented a substantial improvement following intrathecal baclofen (ITB) administration for severe spasticity. The neural mechanisms underlying the clinical recovery after ITB have not yet been clarified. Baclofen could modulate sleep-wake cycles that may be dysregulated and thus interfere with alertne

Drugs used to treat spasticity.

Spasticity is a common and disabling symptom for many patients with upper motor neuron dysfunction. It results from interruption of inhibitory descending spinal motor pathways, and although the pathophysiology of spasticity is poorly understood, the final common pathway is overactivity of the alpha motor neuron. Therapy for spasticity is symptomatic with the aim of increasing functional capacity and relieving discomfort. Any approach to treatment should be multidisciplinary, including physical therapy, and possibly surgery, as well as pharmacotherapy. It is important that treatment be tailored

Malignant Hyperthermia.

Malignant Hyperthermia (MH) is a life-threatening pharmacogenetic disorder which results from exposure to volatile anesthetic agents and depolarizing muscle relaxants. It manifests as a hypermetabolic response resulting in tachycardia, tachypnea, hyperthermia, hypercapnia, acidosis, muscle rigidity and rhabdomyolysis. An increase in the end-tidal carbon dioxide is one of the earliest diagnostic signs. Dantrolene sodium is effective in the management of MH, and should be available whenever general anesthesia is administered. This review also aims to highlight the genetics and pathology of MH, a

A pharmacological evaluation of the pull-up test for muscle relaxation in rats.

The pull-up test for muscle relaxation is described and validated. At testing, rats were evaluated for their ability to recover ('pull-up') from a fully inverted head-down position. Control animals rapidly regained position (median: 1 s). Known muscle relaxants increased latency to pull-up compared to controls. The test proved sensitive to the effects of barbiturates and benzodiazepines which produced graded dose-response functions. In general, results in the pull-up test corresponded with known potencies, with weaker muscle relaxants such as clobazam and oxazepam being less active. The test w

A Chlorzoxazone-Baclofen Combination Improves Cerebellar Impairment in Spinocerebellar Ataxia Type 1.

BACKGROUND: A combination of central muscle relaxants, chlorzoxazone and baclofen (chlorzoxazone-baclofen), has been proposed for treatment of cerebellar symptoms in human spinocerebellar ataxia. However, central muscle relaxants can worsen balance. The optimal dose for target engagement without toxicity remains unknown. Using the genetically precise Atxn1154Q/2Q model of spinocerebellar ataxia type 1, we aimed to determine the role of cerebellar dysfunction in motor impairment. We also aimed to identify appropriate concentrations of chlorzoxazone-baclofen needed for target engagement without

Effect of Different Muscle Relaxation on Motor Control: Comparison Between Ballistic and Ramp Task Methods.

The control of muscle relaxation is not simply the cessation of a muscle contraction, but a dynamic control mechanism for the next movement. Muscle relaxation is triggered by neurophysiological control of the central nervous system. Here, two relaxation strategies were compared, Ballistic and Ramp conditions, and the dynamics of excitability changes between the two relaxation strategies were analyzed. The Ballistic and Ramp conditions were established using visual triggers and response sounds from 20% of the maximum voluntary contraction output of the right wrist joint. The excitability of the

Ion Channels in Anesthesia.

Ion channels play a pivotal role in anesthesia, including general and regional anesthesia. Two main classes of general anesthetics (GAs) are inhalational anesthetics, such as isoflurane, sevoflurane, and nitrous oxide; injectable anesthetics, such as propofol, etomidate, and ketamine. Besides hypnotic agents, muscle relaxants for immobility and opioids for analgesia are needed to achieve balanced anesthesia. Although our understanding of anesthesia is far from complete, recent studies have revealed the molecular interactions between anesthetic drugs and ion channels, particularly, the ligand-g

Leading across boundaries: establishment of the UCLH community outreach intrathecal baclofen refill service.

BACKGROUND: Intrathecal baclofen (ITB) is a recognised treatment for severe spasticity. ITB users need to attend a specialist clinic regularly for their pump to be refilled and reprogrammed. Many patients travel significant distances and require hospital transport which is inconvenient, stressful and costly. These challenges inspired the University College London Hospitals (UCLH) spasticity team to explore establishing a community outreach clinic, staffed by the UCLH team, to provide care closer to home for this complex group. METHOD: By mapping the patient cohort, using their general practit

Efficacy, Indications, and Safety of Intrathecal Baclofen Pump: A Narrative Review.

PURPOSE OF REVIEW: Baclofen, a muscle relaxant that reduces the release of excitatory neurotransmitters in the pre-synaptic neurons stimulating inhibitory neuronal signals in post-synaptic neurons, has been around for over 5 decades. Baclofen is used primarily for spasticity and since 1982, has had a role as an intrathecal agent. In the present investigation, we review research trends and updates on safety and efficacy of intrathecal baclofen (ITB) pumps. RECENT FINDINGS: Evaluation of safety and efficacy of ITB pumps in spasticity and relevant conditions was evaluated in the present investig

Neurosurgical management of elevated tone in childhood: interventions, indications and uncertainties.

Elevated tone (hypertonia) is a common problem in children with physical disabilities. Medications intended to reduce tone often have limited efficacy, with use further limited by a significant side effect profile. Consequently, there has been growing interest in the application of Neurosurgical Interventions for the Management of Posture and Tone (NIMPTs). Three main procedures are now commonly used: selective dorsal rhizotomy (SDR), intrathecal baclofen (ITB) and deep brain stimulation (DBS). This review compares these interventions, along with discussion on the potential role of lesioning s

Intraventricular baclofen for palliative management of acquired generalized dystonia in pediatric patients: a case series and literature review.

Dystonia represents a significant source of disability in children. Generalized dystonia, which involves multiple body regions, leads to impaired mobility and motor function, resulting in substantial challenges in daily activities. Surgical treatments are used when medical treatments fail. Intrathecal baclofen (ITB) or deep brain stimulations (DBS) are the most employed surgical therapies. When these options are not feasible or ineffective, some authors have explored the use of intraventricular baclofen (IVB). In this report, we present four cases of pediatric patients with generalized dystoni
