Quadriceps femoris muscle voluntary force and relaxation capacity in children with spastic diplegic cerebral palsy.

Isometric voluntary force production and relaxation capacity of the quadriceps femoris (QF) muscle was compared between 12 children with spastic diplegic cerebral palsy (CP) and healthy controls, age 10-11 years. Children with CP had less (p < .05) maximal voluntary-contraction force, voluntary activation, and rate of force development than controls. Visual reaction to contraction did not differ significantly in measured groups, whereas the reaction time to relaxation and halfrelaxation time were longer (p < .05) in children with CP. The authors concluded that in children with CP, the capacity

MRT-measurements of muscle volumes of the lower extremities of youths with spastic hemiplegia caused by cerebral palsy.

After long term of studies from our gait lab, the typical muscular dysbalances by all of our patients with cerebral palsy where pointed out. Now we wanted to examine using the MRT, weather the dysbalances of the hemiparetic musculoskeletal system also show up in discrepancies of the muscle volumes. The MRT slices of the lower extremities were segmented. From this cross sections the muscles volumes were derived. These where analyzed particularly with regard to asymmetries between spastic and healthy side. Hemiparetic patients showed reduced volumes of all muscles on the paretic leg in compariso

Motion-Style Scalp Acupuncture Ameliorates Post-Stroke Muscle Spasticity Through Cerebral Blood Flow Augmentation and 5-HT2AR-Mediated Spinal KCC2 Reactivation.

BACKGROUND: Muscle spasticity, a prevalent and debilitating motor impairment after stroke, severely compromises patients' functional recovery and quality of life. Motion-style scalp acupuncture (MSSA), a novel integrative therapy combining scalp acupuncture (SA) with targeted exercise training, has emerged as a promising strategy for post-stroke spasticity (PSS). This study investigated the mechanisms underlying MSSA's therapeutic effects, focusing on 5-HT2AR-mediated KCC2 reactivation in spinal motoneurons. METHODS: Using a rat model of permanent middle cerebral artery (MCA) occlusion, we ev

Effects of dry needling on muscle spasticity of the upper limb in a survivor of traumatic brain injury: a case report.

BACKGROUND: Survivors of moderate and severe traumatic brain injury typically present with spasticity, an upper motor neuron lesion associated with hyperexcitability of the stretch reflex due to disinhibition of cortical influences on spinal cord circuits and structural and functional changes in skeletal muscle. There is growing evidence supporting the effectiveness of dry needling in abating spasticity. CASE PRESENTATION: The present case aims to quantify the effects of dry needling on upper limb spastic muscles in a survivor of severe traumatic brain injury in a 27-year-old Iranian man. The

Safety of OnabotulinumtoxinA with Concomitant Antithrombotic Therapy in Patients with Muscle Spasticity: A Retrospective Pooled Analysis of Randomized Double-Blind Studies.

BACKGROUND: OnabotulinumtoxinA is approved as a treatment across multiple indications. For the treatment of spasticity, onabotulinumtoxinA is injected directly into affected muscles. Intramuscular injections may result in local bleeding and related complications, especially in patients receiving anticoagulant therapy. Despite anticoagulants being commonly used, there is limited information in the medical literature regarding the safety of intramuscular medications in patients receiving oral anticoagulants. This retrospective analysis included pooled safety data from Allergan-sponsored studies

Effect of corticospinal and reticulospinal tract damage on spastic muscle tone and mobility: a retrospective observational MRI study.

BACKGROUND: Spastic muscle tone, often observed after spinal cord injury (SCI), is thought to contribute to body support during walking at a lower level of neural organisation. Understanding the link between damage to specific spinal tracts and the development of spastic muscle tone and mobility could enhance our knowledge of the neural structures crucial for recovery of function after SCI. METHODS: In this retrospective observational study, MRI-based assessments of descending spinal tract damage were related to the development of spastic muscle tone and mobility. Focal damage to the corticos

Botulinum Toxin Intervention in Cerebral Palsy-Induced Spasticity Management: Projected and Contradictory Effects on Skeletal Muscles.

Spasticity, following the neurological disorder of cerebral palsy (CP), describes a pathological condition, the central feature of which is involuntary and prolonged muscle contraction. The persistent resistance of spastic muscles to stretching is often followed by structural and mechanical changes in musculature. This leads to functional limitations at the respective joint. Focal injection of botulinum toxin type-A (BTX-A) is effectively used to manage spasticity and improve the quality of life of the patients. By blocking acetylcholine release at the neuromuscular junction and causing tempor

What causes increased passive stiffness of plantarflexor muscle-tendon unit in children with spastic cerebral palsy?

PURPOSE: The term 'stiffness' is commonly used in the literature to refer to various components of 'hyperresistance' by which spastic muscles oppose to their passive lengthening, especially in children with cerebral palsy (CP). Originally, stiffness consists of mechanical resistance to passive movement in the absence of any muscle activation. Increased muscle stiffness in CP therefore refers to alterations to the mechanical properties of the tissue. It is closely linked to muscle shortening, yet the two phenomena are not equivalent. Both increased stiffness and shortening are present early in

Architecture of the Triceps Surae Muscles Complex in Patients with Spastic Hemiplegia: Implication for the Limited Utility of the Silfverskiöld Test.

The Silfverskiöld test has long been used as an important tool for determining the affected muscles of the triceps surae in patients with equinus deformity. However, the test may not reflect the altered interactions between the muscles of the triceps which are affected by spasticity. The purpose of this study was to compare the architectural properties of the triceps surae muscles complex using ultrasonography, between hemiplegic patients and typically-developing children. Specifically, we wished to examine any differences in the architecture of the three muscles with various angle configurati

Effect of r-TMS over standard therapy in decreasing muscle tone of spastic cerebral palsy patients.

Spastic cerebral palsy (CP) is the one of most common neurological disorders occurring due to damage to the immature brain or any other brain lesion at the time of birth. To aid in making the life of the CP patient meaningful, several interventions such as medical, surgical and rehabilitation have been employed to date. Besides these, recently repetitive Transcranial magnetic stimulation (r-TMS) is a new found approach which is being employed for treating various neurological and psychological conditions. The aim of this study was to observe the effects of r-TMS on muscle spasticity in CP pati
