[Mycobacterium avium intracellulare infections. Treatment with a clarithromycin-clofazimine combination. 18 cases].

Infections caused by Mycobacterium avium intracellulare have become worrying because of their higher frequency, their new tendency to diffuse in all tissues (notably the blood) and the lack of curative treatment. The mortality rate remains high and the survival of patients after AIDS is diagnosed is estimated at 7.4 months. The effectiveness of new antimycobacterial drugs, observed in experiments on beige mice, has not yet been confirmed. In 18 patients suffering from this infection, either disseminated (88 percent) or localized in the lung (12 percent), a 12-week treatment with the clarithrom

Facial lesion caused by Mycobacterium gordonae infection.

We report a case of a patient in her 50s who first presented with cervical lymphadenopathy and later developed a slowly progressing facial lesion. After an initial lymph node biopsy, a diagnosis of cutaneous sarcoidosis was made and she was treated with immunosuppressive therapy. As the facial lesion continued to progress, a skin biopsy was performed which identified Mycobacterium gordonae In retrospect, this organism had been grown on liquid culture from the first lymph node biopsy but was dismissed as a contaminant. 15 years after the first onset of symptoms, she was commenced on multidrug a

BCG Revaccination for the Prevention of Mycobacterium tuberculosis Infection.

BACKGROUND: In a previous phase 2 trial, bacille Calmette-Guérin (BCG) revaccination was not shown to provide protection from primary Mycobacterium tuberculosis infection but prevented sustained M. tuberculosis infection, defined by an initial conversion on a QuantiFERON-TB (QFT) test (an interferon-γ release assay) from negative to positive, followed by two additional positive QFT tests at 3 and 6 months after the initial conversion (a secondary end point). A vaccine efficacy of 45% (95% confidence interval [CI], 6 to 68) was observed. METHODS: We performed a phase 2b, double-blind, randomiz

Mycobacterium bovis Infection in Cats: Zoonotic Transmission.

INTRODUCTION: Tuberculosis caused by Mycobacterium bovis is an infectious disease of worldwide relevance, with a growing concern for its zoonotic potential. Although cattle are the primary host, infections in companion animals have been reported, raising new public health concerns. METHODS: Four cases of M. bovis infection in two cohabiting cats and two humans, one being the pet owner and the other a veterinarian, are analysed. Microbiological and molecular diagnostic techniques were employed, including culture, PCR, and genotyping through spoligotyping. RESULTS: The presence of M. bovis was

Primary Orbital Presentation of Mycobacterium chimaera Infection.

Mycobacterium chimaera (M. chimaera ) is a slow-growing nontuberculous mycobacterium, which, in humans, is mainly associated with cardiothoracic surgical infections and rarely presents with ocular involvement. This report describes a unique case of orbital infection presenting as an inflammatory mass in a healthy 66-year-old white female, without prior trauma, surgery, or significant medical history. Imaging revealed a diffuse mass, with heterogeneous enhancement, medially in the left orbit, and CT of the trunk revealed mediastinal lymphadenopathy. Histopathology of orbital tissue and mediasti

Regulation of mitochondria functions by Mycobacterium tuberculosis infection.

Tuberculosis, a leading killer among infectious diseases worldwide, is caused by Mycobacterium tuberculosis (Mtb). Mtb has strong ability to manipulate the intracellular environment of macrophages for successful surviving. Mitochondrion is a key organelle involved in diverse physiological processes, including Ca2+ fluxes, ATP synthesis, bioenergetic metabolism, and cell death, which are pivotal to cellular and organismal homeostasis. Mitochondrion is also targeted by Mtb to control various physiological responses of the host. Mtb has evolved a series of strategies to manipulate mitochondrial f

Manganese as a Cofactor in the Immunity of Mycobacterium tuberculosis Infection.

Mycobacterium tuberculosis (Mtb), a highly infectious intracellular pathogen, is the primary causative agent of tuberculosis (TB), which ranks as the leading cause of death from individual infectious agents. In this context, we developed a model using macrophages infected with M. smegmatis mc2155 and Mtb H37Ra to elucidate the role of Mn2+ in host defense against tuberculosis infection. Treatment with a combination of anti-tuberculosis drugs and Mn2+ for 36 h markedly reduced the survival of intracellular bacteria, and the addition of Mn2+ significantly enhanced the secretion of TNF-α and IL-1

Mycobacterium Infection of Abscess with Hemoptysis as the Initial Manifestation.

BACKGROUND: Mycobacterium abscessus is a new pathogen in recent years, which belongs to non-tuberculosis mycobacterium. Mycobacterium abscessus is widely involved in many nosocomial infections and secondary aggravation of genetic respiratory diseases. Mycobacterium abscessus is naturally resistant to most antibiotics and is difficult to treat. We report a case of mycobacterium abscessus infection with hemoptysis as the first manifestation. METHODS: Bronchoscopy, next-generation sequencing (NGS). RESULTS: Acid-fast staining of bronchoscopic lavage fluid showed that a small amount of acid-fast

Mycobacterium tuberculosis infection in pregnancy: A systematic review.

Pregnancy may be associated with risk of developing tuberculosis (TB) in those infected with Mycobacterium tuberculosis (Mtb). The perinatal period could provide opportunities for targeted screening and treatment. This study aims to synthesise published literature on Mtb infection in pregnancy, relating to prevalence, natural history, test performance, cascade of care, and treatment. We searched Ovid MEDLINE, Embase+Embase Classic, Web of Science, and Cochrane Central Register of Controlled Trials (CENTRAL) on October 3, 2023, and 47 studies met the inclusion criteria. The prevalence of Mtb in

[A case of HIV combined with Mycobacterium celatum infection].

Obstructive sleep apnea (OSA) is the frequent occurrence of apnea and/or hypopnea during sleep, leading to intermittent hypoxia, hypercapnia, and disruption of sleep architecture, further resulting in multisystem damage. The pathophysiological mechanisms include abnormal anatomical structure, low arousal threshold, high loop gain, and poor muscle reactivity, etc. As there are individual differences in the underlying mechanisms of OSA (i.e. endotypes), the effectiveness of treatment and prognosis may also vary according to these characteristics. Understanding the endotype of OSA is critical to

Two Cases of Cutaneous Mycobacterium szulgai Infection.

Although nontuberculous mycobacteria (NTM) infection is often thought of as a pulmonary disease, it can manifest on the skin in rare cases. In this report, we describe two cases of cutaneous Mycobacterium szulgai that presented as nonhealing rashes that were initially thought to be caused by bacterial cellulitis but did not respond to broad-spectrum antibiotics. In both cases, the rash began on the upper extremities. We believe that these cases will be of interest as they demonstrate a rare cutaneous infection and an unusual presentation for NTM. It is important to consider NTM, and M. szulgai

Mycobacterium ulcerans lung infection in an immunocompetent patient.

Mycobacterium ulcerans has been implicated in cutaneous manifestations in humans, causing persistent wounds called Buruli ulcer. However, it has not been associated with pulmonary infections in humans to date. Herein, we report a case of an immunocompetent adult man with no underlying medical problems presenting with dyspnoea and generalised malaise and diagnosed with M. ulcerans lung infection. The patient was prescribed clarithromycin 500 mg two times per day, rifampin 300 mg two tablets daily and moxifloxacin 400 mg daily for 6 months, with complete resolution of his symptoms.

Behçet's disease with latent Mycobacterium tuberculosis infection.

OBJECTIVE: The aim of this study is to examine the clinical features of patients with Behçet's disease (BD) in the presence or absence of latent tuberculosis infection (LTBI). METHODS: This was a retrospective study of 232 consecutive patients with active BD hospitalized between October 2012 and June 2017. LTBI was diagnosed based on the positive T-SPOT.TB assay, negative clinical, and imaging examinations. RESULTS: Among the 232 patients, 68 (29.3%) had LTBI. The frequency, number, and scope of oral ulcers in the BD-LTBI group were significantly more serious than in the non-LTBI group (all

Genetic Resistance to Mycobacterium tuberculosis Infection and Disease.

Natural history studies of tuberculosis (TB) have revealed a spectrum of clinical outcomes after exposure to Mycobacterium tuberculosis, the cause of TB. Not all individuals exposed to the bacterium will become diseased and depending on the infection pressure, many will remain infection-free. Intriguingly, complete resistance to infection is observed in some individuals (termed resisters) after intense, continuing M. tuberculosis exposure. After successful infection, the majority of individuals will develop latent TB infection (LTBI). This infection state is currently (and perhaps imperfectly)

Therapy for Mycobacterium kansasii Infection: Beyond 2018.

The current standard of care therapy for pulmonary Mycobacterium kansasii infection is isoniazid (300 mg/day), rifampin (600 mg/day), and ethambutol (15 mg/kg/day) for 12 months after achieving sputum culture negativity. Rifampin is the key drug in this regimen. The contribution of isoniazid is unclear since its in vitro MICs against M. kansasii are near the peak achievable serum levels and more than 100-fold greater than the MICs for Mycobacterium tuberculosis. Ethambutol likely decreases the emergence of rifampin resistant organisms. There are several new drug classes (e.g., quinolones, macr

Mycobacterium bovis Infection of Red Fox, France.

Mycobacterium bovis infection in wild red foxes was found in southern France, where livestock and other wildlife species are infected. Foxes frequently interact with cattle but have been underestimated as a reservoir of M. bovis. Our results suggest a possible role of the red fox in the epidemiology of bovine tuberculosis.

Effect of Mycobacterium tuberculosis infection on adipocyte physiology.

Tuberculosis (TB) remains as a major threat to human health worldwide despite of the availability of standardized antibiotic therapy. One of the characteristic of pathogenic Mycobacterium tuberculosis (Mtb), the causative agent of tuberculosis is its ability to persist in the host in a dormant state and develop latent infection without clinical signs of active disease. However, the mechanisms involved in bacterial persistence and the establishment of latency is not well understood. Adipose tissue is emerging as an important niche that favors actively replicating as well as dormant Mtb during a
