Narcolepsy: a clinical review.

Despite the classic tetrad of clinical features that typify it, narcolepsy remains much under-diagnosed, in part, because of the wide spectrum of clinical phenotypes, but also because of its insidious onset, usually in a young person. The median time to diagnosis from first symptoms remains very long, around 10 years in the UK. Conversely, in the specialist setting, it is likely over-diagnosed, largely because of failure to exclude other causes of hypersomnia. There is an over-reliance on a biological marker of the condition, the multiple sleep latency test (MSLT), which, like many tests, has

[Moebius syndrome and narcolepsy].

BACKGROUND: Moebius syndrome is a rare neurological disease that has a frequent association with parasomnia. CASE REPORT: We report on a patient with Moebius syndrome and the clinical presentation of a narcolepsy cataplexy syndrome. With the hypoplasia of the brainstem in the cranial magnetic resonance imaging, we were able to show the morphological correlate of Moebius syndrome. Comorbidity was detected by cognitive tests, polysomnography and detection of hypocretin in the cerebrospinal fluid. Despite normal sleep onset latency and only one episode of sleep onset rapid eye movement (REM) in

[Narcolepsy as an autoimmune disease].

Narcolepsy is a chronic sleep disorder characterized by excessive daytime sleepiness and irresistible sleep attacks that occur during the activities of daily living. Falling asleep in the middle of essential activities such as driving or crossing the street may lead to life-threatening situations. Narcolepsy is estimated to affect 0.002% of the Israeli population. By using animal models, human autopsies and brain biopsies, it has recently been shown that the destruction of the orexin-secreting neurons underlies the pathogenicity of this disease. Orexin is a neurotransmitter involved in the sle

The autoimmune basis of narcolepsy.

Narcolepsy is a neurological disorder characterized by excessive daytime sleepiness, cataplexy, hypnagonic hallucinations, sleep paralysis, and disturbed nocturnal sleep patterns. Narcolepsy is caused by the loss of hypocretin (orexin)-producing neurons in the lateral hypothalamus. Evidence, such as a strong association with HLA DQB1*06:02, strongly suggests an autoimmune basis targeting hypocretin neurons. Genome-wide association studies have strengthened the association between narcolepsy and immune system gene polymorphisms, including the identification of polymorphisms in the T cell recept

[Neurophysiological examinations in narcolepsy].

BACKGROUND: Narcolepsy is a chronic hypersomnia of central origin, linked with dysfunction of hypocretin-containing neurons, localized in the lateral hypothalamus. Main symptoms of narcolepsy include excessive daytime sleepiness, cataplexy, hypnagogic hallucinations and sleep paralysis. Establishing the proper diagnosis is very important, because of the negative impact of narcolepsy on patients' functioning in social-life and the possibility of improvement of quality of life with adequate treatment. AIM OF THE STUDY: Present review describes clinical characteristics of narcolepsy, usefulness

Narcolepsy in Parkinson's disease.

Non-motor symptoms in Parkinson's disease (PD), such as excessive daytime sleepiness, 'sleep attacks', insomnia, restless legs syndrome and rapid eye movement sleep behavior disorder, are common and provide a challenge to treatment. These sleep symptoms are also described in patients suffering from the sleep/wake disorder, narcolepsy. The International Classification of Sleep Disorders (ICSD-2) narcolepsy criteria uses a number of markers for diagnosis, of which lack or deficiency of cerebrospinal fluid (CSF) hypocretin-1 levels is a key marker. Hypocretin neurons prominently located in the la

[Narcolepsy and other hypersomnias].

BACKGROUND: Narcolepsy is currently categorized into three groups; narcolepsy with and without emotionally triggered loss of muscle tone (cataplexy), and narcolepsy secondary to other medical conditions. Many patients with hypersomnia are diagnosed too late. The article presents a review of this field. MATERIAL AND METHODS: Sources are literature identified through a non-systematic Pub-Med search and the authors' personal experience with diagnosing and treating patients with narcolepsy and other hypersomnias. RESULTS: Sleep attacks and emotionally triggered loss of muscle tone are the most i

Antidepressant drugs for narcolepsy.

BACKGROUND: Narcolepsy is a disorder of the central nervous system, the main symptoms of which are excessive daytime sleepiness (EDS) and cataplexy (an abrupt and reversible decrease in or loss of muscle tone, affecting the limbs or trunk or both, elicited by emotional stimuli). Narcolepsy has an adverse impact on people's quality of life. Together with stimulant drugs (used to control EDS), antidepressants are usually recommended to counteract cataplexy. In addition, some antidepressants are also reported to improve EDS. OBJECTIVES: To evaluate the effects of antidepressant drugs on EDS, cat

Narcolepsy: pathophysiology and pharmacology.

Narcolepsy, which affects 1 in 2000 people in the general population, is characterized by excessive daytime sleepiness (EDS), cataplexy, and other dissociated manifestations of rapid eye movement sleep (hypnagogic hallucinations and sleep paralysis). The disease is currently treated with amphetamine-like central nervous system stimulants (for EDS) and antidepressants (for cataplexy). Some compounds from other classes, such as modafinil (a non-amphetamine wake-promoting compound for EDS) and sodium oxybate (a short-acting sedative for EDS and cataplexy, administered at night), are also employed

Cognitive deficits in narcolepsy.

The aim of the investigations was to explore the nature and the severity of cognitive deficits in narcolepsy patients. In two studies, narcolepsy patients were compared with matched control subjects on a range of attention, memory and executive control tasks. Impairments were only observed on attention and executive function tasks which involved higher demands on inhibition or task management abilities whereas relatively routine memory and attention tasks yielded intact performance in narcolepsy patients. The overall pattern of results indicates an executive control deficit in narcolepsy which

Donepezil in the treatment of narcolepsy.

Chronic excessive sleepiness impairs the daily life of patients suffering from narcolepsy. Antidepressants that enhance synaptic levels of noradrenaline and serotonin have been reported as having some therapeutic efficacy. For that reason, acetylcholinesterase inhibitors, which elevate acetylcholine levels, may have a protective function with respect to narcolepsy and cataplexy. The patient reported in this article received donepezil, 10 mg, for 3 consecutive months and showed an improvement of the Epworth Sleepiness Scale score from 20 up to 14.

Narcolepsy in the custodial setting.

A case of a detained person who suffered from narcolepsy is presented. The management of the condition in general and with particular reference to the custodial setting is discussed. The management of obstructive sleep apnoea, the commonest sleep disorder, is contrasted. Forensic physicians are often confronted with rare or unusual medical problems in the custodial setting, often having to make immediate medical decisions without access to specialist medical advice. Twenty-four hour access to Internet medical resources in the custodial setting may be helpful.

Emerging drugs for narcolepsy.

Narcolepsy is characterised by excessive daytime sleepiness, usually associated with cataplexy, hypnagogic hallucinations, sleep paralysis and fragmented nocturnal sleep. Although uncommon, it results in significant disability. Most cases occur sporadically, but genetic factors probably form a susceptibility background on which unknown environmental triggers act. The hypocretin system is strongly implicated in the development of narcolepsy. Cerebrospinal fluid levels of hypocretin-1 are significantly reduced in narcoleptic subjects with cataplexy. Despite the advances in our understanding of n

A functional autoantibody in narcolepsy.

Narcolepsy is widely believed to have an autoimmune basis, but conventional immunological approaches have failed to detect a serum autoantibody marker. Since cholinergic hyperactivity is a feature of narcolepsy-cataplexy, we transferred IgG from nine patients with narcolepsy and nine healthy controls to mice and assessed the effect on smooth muscle contractile responses to cholinergic stimulation. IgG from all narcolepsy patients significantly enhanced bladder contractile responses to the muscarinic agonist carbachol and to neuronally released acetylcholine compared with control IgG (p<0.0001)
