BACKGROUND: Provision of value-based, patient-centered care requires careful appraisal of patients’ symptoms. Understanding subjective experiences, needs, and feelings is crucial for shared decision-making, especially when clinical findings differ from individual perceptions. Natural language processing (NLP) offers new ways to understand patients’ perspectives. This exploratory pilot study aimed to exemplify the use of three popular NLP methods to analyze open-ended textual self-reports from individuals experiencing orofacial pain and dysfunction for a more comprehensive understanding of thei

New methods that capture the public's perception of controversial topics may be valuable. This study investigates public sentiments toward psychedelics and other substances through analyzes of Reddit discussions, using Google's cloud-based Natural Language Processing (NLP) infrastructure. Our findings indicate that illicit substances such as heroin and methamphetamine are associated with highly negative general sentiments, whereas psychedelics like Psilocybin, LSD, and Ayahuasca generally evoke neutral to slightly positive sentiments. This study underscores the effectiveness and cost efficienc

BACKGROUND: Subjective report of pain remains the gold standard for assessing symptoms in patients with chronic pain and their response to analgesics. This subjectivity underscores the importance of understanding patients' personal narratives, as they offer an accurate representation of the illness experience.
OBJECTIVE: In this pilot study involving 20 patients with chronic low back pain (CLBP), we applied emerging tools from natural language processing (NLP) to derive quantitative measures that captured patients' pain narratives.
METHODS: Patients' narratives were collected during recorded

BACKGROUND: Bicuspid aortic valve (BAV) is the most common congenital heart defect but often evades timely diagnosis due to variable clinical presentations. Prior to October 2024, no specific diagnosis code existed for BAV, limiting retrospective identification.
OBJECTIVES: The purpose of this study was to develop and validate a natural language processing (NLP) system for automated extraction of heart valve morphology from echocardiographic reports, with focus on BAV detection.
METHODS: We developed a rule-based NLP system using MedSpaCy to analyze echocardiographic reports from the Veteran

Physicians' letters are the optimal source of diagnoses for registries. However, most registries demand for diagnosis codes such as ICD-10. We herein describe an algorithm that infers ICD-10 codes from German ophthalmologic physicians' letters. We assess the method in three German eye hospitals. Our algorithm is based on the nearest-neighbor method as well as on a large thesaurus for ICD-10 codes. This thesaurus was embedded into a Word2Vec space created from anonymized physicians' reports of the first hospital. For evaluation, each of the three hospitals sent all diagnoses taken from 100 lett

BACKGROUND: Postoperative complication rates are often assessed through administrative data, although this method has proven to be imprecise. Recently, new developments in natural language processing have shown promise in detecting specific phenotypes from free medical text. Using the clinical challenge of extracting four specific and frequently undercoded postoperative complications (pneumonia, urinary tract infection, sepsis, and septic shock), it was hypothesized that natural language processing would capture postoperative complications on a par with human-level curation from electronic hea

AIMS: Worsening heart failure (WHF) events occurring in non-inpatient settings are becoming increasingly recognized, with implications for prognostication. We evaluate the performance of a natural language processing (NLP)-based approach compared with traditional diagnostic coding for non-inpatient clinical encounters and left ventricular ejection fraction (LVEF).
METHODS AND RESULTS: We compared characteristics for encounters that did vs. did not meet WHF criteria, stratified by care setting [i.e. emergency department (ED) and observation stay]. Overall, 8407 (22%) encounters met NLP-based c

Manual decisions regarding the timing of surveillance endoscopy for premalignant Barrett's oesophagus (BO) is error-prone. This leads to inefficient resource usage and safety risks. To automate decision-making, we fine-tuned Bidirectional Encoder Representations from Transformers (BERT) models to categorize BO length (EndoBERT) and worst histopathological grade (PathBERT) on 4,831 endoscopy and 4,581 pathology reports from Guy's and St Thomas' Hospital (GSTT). The accuracies for EndoBERT test sets from GSTT, King's College Hospital (KCH), and Sandwell and West Birmingham Hospitals (SWB) were 0

BACKGROUND: Data examining associations among social support, survival, and healthcare utilization are lacking in patients with advanced cancer.
METHODS: We conducted a cross-sectional secondary analysis using data from a prospective longitudinal cohort study of 966 hospitalized patients with advanced cancer at Massachusetts General Hospital from 2014 through 2017. We used NLP to identify extent of patients' social support (limited versus adequate as defined by NLP-aided review of the Electronic Health Record (EHR)). Two independent coders achieved a Kappa of 0.90 (95% CI: 0.84-1.00) using NL

BACKGROUND: Severe maternal morbidity and mortality remain public health priorities in the United States, given their high rates relative to other high-income countries and the notable racial and ethnic disparities that exist. In general, accurate risk stratification methods are needed to help patients, providers, hospitals, and health systems plan for and potentially avert adverse outcomes.
OBJECTIVE: Our objective was to understand if machine learning methods with natural language processing of history and physical notes could identify a group of patients at high risk of maternal morbidity

BACKGROUND: Maternal mental disorders are considered a leading complication of childbirth and a common contributor to maternal death. In addition to undermining maternal welfare, untreated postpartum psychopathology can result in child emotional and physical neglect and associated significant pediatric health costs. Some women may experience traumatic childbirth and develop posttraumatic stress disorder symptoms after delivery (childbirth-related posttraumatic stress disorder). Although women are routinely screened for postpartum depression in the United States, there is no recommended protoco

BACKGROUND: Natural language processing (NLP) is a discipline of machine learning concerned with the analysis of language and text. Although NLP has been applied to various forms of clinical text, the applications and utility of NLP in spine surgery remain poorly characterized. Here, we systematically reviewed studies that use NLP for spine surgery applications, and analyzed applications, bias, and reporting transparency of the studies.
METHODS: We performed a literature search using the PubMed, Scopus, and Embase databases. Data extraction was performed after appropriate screening. The risk

OBJECTIVE: Investigation of asymptomatic carotid stenosis treatment is hindered by the lack of a contemporary population-based disease cohort. We describe the use of natural language processing (NLP) to identify stenosis in patients undergoing carotid imaging.
METHODS: Adult patients with carotid imaging between 2008 and 2012 in a large integrated health care system were identified and followed through 2017. An NLP process was developed to characterize carotid stenosis according to the Society of Radiologists in Ultrasound (for ultrasounds) and North American Symptomatic Carotid Endarterectom

In this paper, we propose a framework for the automatic generation of natural language descriptions of healthcare processes using quantitative and qualitative data and medical expert knowledge. Inspired by the demand of novel ways of conveying process mining analysis results of healthcare processes (Rojas et al., 2016), our framework is based on the most widely used Data-To-Text (D2T) pipeline (Reiter, 2007) and on the usage of process mining techniques. Backed by a general model that handles process data, this framework is able to quantify attributes in time during a process life-span, recall
