Screening for GALC to make neonatal diagnosis and initial neonatal stem cell treatment with umbilical cord blood.

Generalized vitiligo is an autoimmune disorder characterized by acquired white patches of skin and overlying hair, the result of loss of melanocytes from involved areas. The most common disorder of pigmentation, vitiligo occurs with a frequency of 0.1-2.0% in various populations. Family clustering of cases is not uncommon, in a non-Mendelian pattern suggestive of multifactorial, polygenic inheritance. We surveyed 2624 vitiligo probands from North America and the UK regarding clinical characteristics, familial involvement, and association with other autoimmune disorders, the largest such survey

Evaluation of neonatal sepsis screening in a tropical area Part III: Neonatal sepsis in meconium stained deliveries.

Of the 6,060 consecutive live births delivered at the University Maternity Unit of Guadeloupe (French West Indies) during a 30-month period, 635 newborns (10.4%) presented with meconium stained (MS) amniotic fluid, of which 595 (94%) received bacteriological screening at birth (light MS, n = 543; thick MS, n = 52). Thirty (5%) of MS newborns had a bacteraemia (n = 13, group B streptococcus, GBS), and 128 (21.5%) a bacterial positive gastric aspirate (n = 54, GBS). Sixty-six newborns among MS babies needed tracheal suctioning (11%) in the delivery room for meconium inhalation. Among these 595 s

Progress in screening strategies for neonatal developmental dysplasia of the hip.

Developmental dysplasia of the hip (DDH) is the most common congenital disease of the musculoskeletal system in newborns and encompasses a disease spectrum ranging from a stable hip with a mildly dysplastic acetabulum to complete hip dislocation. Systematic screening for infant DDH has been performed for several decades all over the world and has contributed greatly to the early detection, diagnosis and treatment of DDH. However, some cases of delayed diagnosis still occur among the screened population, or conversely, overdiagnosis or overtreatment occasionally occurs. Furthermore, screening s

The utility of cranial ultrasound as a screening tool for neonatal ECMO.

Background Neurologic complications including hemorrhage, ischemia, and infarction are often identified in neonates undergoing extracorporeal membrane oxygenation (ECMO) and may contribute to the high morbidity observed in ECMO survivors. Screening for intracranial complications is reliant on bedside transcranial ultrasound (CUS) prior to and during ECMO therapy, and advanced imaging [i.e. computed tomography (CT)/magnetic resonance imaging (MRI)] is recommended after completion of ECMO support. The goal of this study is to describe the correlation of intracranial complications identified on C

Improving the rigour of VACTERL screening for neonates with anorectal malformations.

PURPOSE: Screening investigations for the vertebral, anorectal, cardiac, tracheo-oesophageal, renal and limb (VACTERL) association form an important part of the management of neonates with anorectal malformations (ARMs). We developed a proforma to define investigations and indications for referral. The aim of the current study was to determine if the proforma could improve screening rigour. METHODS: Four centres performed a 3-year retrospective audit of neonates with ARM. Following introduction of a proforma, the same data were collected prospectively for consecutive neonates over a further 2

Routine TORCH screening is not warranted in neonates with subependymal cysts.

BACKGROUND: Congenital infections are associated with a wide variety of clinical symptoms, including subependymal cysts (SEC). OBJECTIVE: To determine the co-occurrence of SEC and congenital infections, as diagnosed by TORCH serologic tests and/or cytomegalovirus (CMV) urine culture. METHODS: We performed a retrospective study of all neonates admitted to our neonatal intensive care unit from 1998 to 2009 in whom SEC were detected on cranial ultrasound and TORCH serologic tests and/or CMV urine cultures were performed. RESULTS: Fifty-nine neonates fulfilled the inclusion criteria. TORCH sero

Lenticular subluxation in a patient with homocystinuria undetected by neonatal screening.

A case of homocystinuria with lenticular subluxation was misdiagnosed as Marfan syndrome since the patient had no apparent mental impairment and had had a negative neonatal screen for homocystinuria. The delayed diagnosis of homocystinuria was due to a negative prior neonatal screen which was checked when he was a breastfed healthy newborn. In the absence of an autosomal dominant family history, and because of prior poor school performance, amino acid analysis and mutational analysis of the cystathionine beta-synthase gene were performed, which revealed the presence of homocystinuria. Low meth

Economic evaluation of neonatal screening for congenital dislocation of the hip.

A study was undertaken to determine the cost benefit of neonatal screening for dislocatable and subluxable hips. As a follow-up to retrospective and prospective studies that established the safety and efficacy of early treatment to the newborn, this study discusses the direct and significant economic benefit of a neonatal hip screening program to the participating institutions. British Columbia's Grace and Children's Hospitals experienced an economic benefit of $15,143.71 (Canadian) per 1,000 infants screened.

[Thyroid ectopia, a cause of error in neonatal screening for hypothyroidism].

The authors report the case of a boy presenting with hypothyroidism due to ectopic thyroid, diagnosed at age 3 years. The results of neonatal screening for hypothyroidism (T4 + TSH) had been normal. A large ectopic thyroid might have been responsible for normal levels of thyroid hormone at birth.

The effect of antenatal screening for depression on neonatal birthweight and gestation.

BACKGROUND: Antenatal depression has been associated with poor perinatal outcomes such as preterm birth and low birth weight. Universal screening of perinatal depression has been recommended to improve maternal and pregnancy outcomes. HYPOTHESIS AND AIM: We hypothesise that screening for antenatal depressive symptoms may reduce the risk of preterm birth and low birth weight. To assess the hypothesis, we explore the association between antenatal depression screening using the Edinburgh Postnatal Depression Scale and neonatal outcomes. METHODS: A retrospective analysis of state-wide population

Contingent Screening for Small by Weight for Gestational Age Neonates.

Effective screening for small for gestational age neonates (SGA), in the absence of preeclampsia, can be accomplished using a contingent screening method. The basis for the contingent model is a combined assessment at 19-24 weeks gestation to stratify patients according to their risk. We can then identify prenatally over 90% of SGA neonates for a false positive rate of 10%.

[Mass screening of deafness in neonates using otoacoustic emissions].

There is no clinical sign of hearing loss in the infant. Defective language acquisition and altered voice quality always occur late making early diagnosis essential for therapeutic management of deaf children. Subjective assessment of behavioral is highly dependent on the experience of the examinator and cannot be used as an effective routine method. Electrophysiological recordings of otoacoustic emissions have the advantage of allowing objective measurement. Otoacoustic emissions originate in the inner ear. A quantity of energy is transmitted via the tympanic membrane which acts as a micropho

Comparison of auditory brainstem response and behavioral screening in neonates.

This paper reports on two studies, one in Halifax and the other in Ottawa, which compared behavioral methods and BERA in the screening of hearing loss in neonates. The Halifax study used BERA as the screening procedure for infants of a neonatal intensive care unit (NICU) and as a supplementary procedure to behavioral screening test for non-NICU but "at risk" infants. The results of this study indicate false positive rates with the behavioral test of 86.1 and 50.5% for NICU and at risk groups respectively. The Ottawa study evaluated the Crib-o-gram as a screening test for NICU infants using BER

Neonatal screening for prenatal alcohol exposure: assessment of voluntary maternal participation in an open meconium screening program.

Meconium fatty acid ethyl esters (FAEEs) are validated biomarkers of fetal alcohol exposure. Meconium FAEE testing can potentially be used as a screen by health-care professionals to identify neonates at-risk for Fetal Alcohol Spectrum Disorder, thereby permitting diagnostic follow-up of these children and early intervention in those who develop disabilities. The purpose of this study was to assess whether women would willingly partake in a screening program of this nature. This was determined by launching a pilot screening program for prenatal alcohol exposure in a high-risk obstetric unit pr
