Considerations in the pharmacologic treatment and prevention of neonatal sepsis.

The management of neonatal sepsis is challenging owing to complex developmental and environmental factors that contribute to inter-individual variability in the pharmacokinetics and pharmacodynamics of many antimicrobial agents. In this review, we describe (i) the changing epidemiology of early- and late-onset neonatal sepsis; (ii) the pharmacologic considerations that influence the safety and efficacy of antibacterials, antifungals, and immunomodulatory adjuvants; and (iii) the recommended dosing regimens for pharmacologic agents commonly used in the treatment and prevention of neonatal sepsi

The effects of ibuprofen on sepsis parameters in preterm neonates.

AIM: To examine the effects of ibuprofen used for patent ductus arteriosus (PDA) treatment on the production of the proinflammatory cytokines C-reactive protein (CRP) and interleukin 6 (IL-6) in preterm septic newborns. METHODS: Patients with acute phase reactant elevation were divided into two groups according to receiving ibuprofen (Group I, n=51) or not (Group II, n=38). Course of sepsis was evaluated by CRP and IL-6 levels. RESULTS: CRP and IL-6 levels at the time of diagnosis were not different between two groups [16±9.1 vs 16.4±13.2mg/dL (p=0.43) for CRP and 124±82 vs 119±73mg/dL (p=0.

Neonatal sepsis: causative bacteria and their resistance to antibiotics.

BACKGROUND: Neonatal sepsis is one of the major causes of neonatal morbidity and mortality, particularly in developing countries. The objective of this study was to determine the causative bacteria and level of their resistance to commonly used antibiotics. METHODS: This descriptive study was carried out at Ayub Teaching Hospital, Abbottabad from April 2009 to January 2010. All neonates of either gender admitted in neonatology unit with clinical sepsis and positive blood culture were included in the study. Neonatal period was defined as 28 days of life at term and up to 44 weeks of gestationa

Neonatal sepsis due to Salmonella Typhi and Paratyphi A.

Sepsis due to Salmonella enterica serovar Typhi and Paratyphi A is rare in neonates. Though described as a mild and unrecognized illness in infants, life-threatening complications and even deaths have been reported. We present five cases of neonatal septicaemia due to S. Typhi and S. Paratyphi A. The cases were presented because of their interesting clinical presentations and possible modes of transmission. Salmonella infections should be considered in the differential diagnosis of sepsis neonatorum, especially in endemic areas.

Expression of CD64 as a potential marker of neonatal sepsis.

The aim of this study was to identify a novel immunological indicator useful for the early diagnosis (through a rapid and single determination) of neonatal sepsis (NS). Peripheral blood samples were taken from 63 neonates, who were classified into four groups: proven NS (n = 17); clinical NS (n = 14); disease without infection (n = 17); and healthy newborns (n = 15). Neutrophil expression of CD64, CD43, CD44, CD50, CD62L and Mac-1, and plasma levels of interleukin (IL)-1beta, IL-6, tumor necrosis factor-alpha (TNF-alpha) and soluble L-selectin (sCD62L), were determined. Expression of CD64 was

Evaluation of sepsis screen for diagnosis of neonatal septicemia.

Fifty clinically suspected cases of neonatal septicemia were studied for evaluating the role of sepsis screen. Sensitivity and specificity of C-reactive protein test, micro-ESR, gastric aspirate cytology for polymorphs and toxic granules in neutrophils were studied singly and in combinations of two and three tests. Positive blood culture was obtained in only 20% cases, thereby underlying the need for a sepsis screen in the diagnosis of neonatal septicemia, especially in areas where adequate micro-biological facilities are lacking.

Association of Escherichia coli sepsis and galactosemia in neonates.

Galactosemia in newborns and infants is associated with the following symptoms: jaundice, hepatomegaly, failure to thrive, feeding difficulties, hypoglycemia, convulsions, lethargy, amino-aciduria, cataracts, hepatic cirrhosis, ascites, and mental retardation. If the preliminary evaluation indicates galactosemia, there is high risk for E. coli sepsis and death. Strong consideration should therefore be given for early antibiotic therapy in infants with suspected galactosemia in spite of the absence of clinical signs or symptoms of sepsis.

Incidence of sepsis in neonates with clinical respiratory distress.

Respiratory distress is the most frequent symptom initiating referral to neonatal intensive care centers. Previous reports have indicated that 40% of infants with sepsis proved by blood culture had respiratory symptoms as their clinical presentation. In this prospective study there were 145 consecutive infants referred because of respiratory distress. Two-site blood cultures were collated with all other clinical and laboratory data to confirm a diagnosis of septicemia. There was a 4.8% incidence of bacteremia, and clinical and laboratory parameters confirmed septicemia in 3.5% of infants studi

TRIC agent as a cause of neonatal eye sepsis.

Among 2,700 babies born in a maternity hospital there were 71 (2.6%) cases of eye sepsis, mostly mild. Forty-four of these cases were screened for inclusions of TRIC agent. Four certain positives and a further four very suggestive cases were found-an incidence of 18% of the cases of eye sepsis investigated. The true incidence is probably somewhat higher.Follow-up showed that even minimal and transitory eye sepsis from TRIC infection in the neonate can lead to permanent scarring of the conjunctiva and cornea, with or without pannus.

Antibiotic Therapy for the Prevention and Treatment of Neonatal Sepsis.

Antibiotics are among the most commonly prescribed medications in neonatal intensive care units. Preterm and term infants are commonly exposed to antibiotics: before birth via transplacental intrapartum maternal exposures, at birth due to suspected early-onset sepsis, or due to suspected late-onset sepsis in the first weeks to months of a neonatal intensive care unit hospitalization. Antibiotic selection requires assessment of infection timing and associated microbiology patterns, patient-specific factors (eg, colonization status, source of infection), and antibiotic resistance concerns. Neonatal sepsis in a tertiary unit in South Africa.

BACKGROUND: Antimicrobial resistance (AMR) has emerged as a global threat to healthcare resulting in an increase in morbidity and mortality. Neonatal sepsis is ranked as the third highest cause of neonatal demise globally, in which AMR accounted for 31.0% of deaths. AMR in neonates has been poorly characterised in Durban, South Africa. Thus, the resultant effect of AMR on empiric regimens for neonatal sepsis is uncertain in this setting. Therefore, this study analysed the aetiology and antimicrobial susceptibility patterns of bloodstream infections within the neonatal intensive care unit at a

Clinical and immunological aspects of microRNAs in neonatal sepsis.

Neonatal sepsis constitutes a highly relevant public health challenge and is the most common cause of infant morbidity and mortality worldwide. Recent studies have demonstrated that during infection epigenetic changes may occur leading to reprogramming of gene expression. Post-transcriptional regulation by short non-coding RNAs (e.g., microRNAs) have recently acquired special relevance because of their role in the regulation of the pathophysiology of sepsis and their potential clinical use as biomarkers. ~22-nucleotide of microRNAs are not only involved in regulating multiple relevant cellular

Evaluation of procalcitonin as a diagnostic marker in neonatal sepsis.

CONTEXT: Neonatal sepsis is an early infection occurring within 28 days of the postnatal life. It has nonspecific signs and symptoms which make the diagnosis cumbersome. It inflicts an increase in morbidity and mortality among neonates. Procalcitonin (PCT) is yet another acute phase reactant, which is synthesized by the C-cells of thyroid gland. AIMS: The aim of our study is to evaluate PCT as a diagnostic marker of neonatal sepsis in comparison with C-reactive protein (CRP). SUBJECTS AND METHODS: A prospective cross-sectional study was conducted at our tertiary care hospital in Puducherry.

Incidence of fever in labor and risk of neonatal sepsis.

BACKGROUND: The current recommendation regarding the management of a term newborn delivered of a mother with an intrapartum fever or a diagnosis of clinical chorioamnionitis is that the neonate should have baseline laboratory work drawn along with blood cultures and be universally treated with antibiotics until culture results return. These guidelines report that the rate of intrapartum fever is about 3%; however, a few large studies suggest that the rate is higher at about 7%. OBJECTIVE: We sought to prospectively evaluate the rate of fever during labor in a large number of deliveries and de

Pentoxifylline for treatment of sepsis and necrotizing enterocolitis in neonates.

BACKGROUND: Mortality and morbidity due to neonatal sepsis and necrotizing enterocolitis (NEC) remain high despite the use of potent antimicrobial agents. Agents that modulate inflammation may improve outcomes. Pentoxifylline, a phosphodiesterase inhibitor, is one such agent. OBJECTIVES: Our primary objectives were :1.To assess the effect of intravenous pentoxifylline as an adjunct to antibiotic therapy on mortality and morbidity in neonates with suspected or confirmed sepsis.2.To assess the effect of intravenous pentoxifylline as an adjunct to antibiotic therapy on mortality and morbidity in
