Is the root entry/exit zone important in microvascular compression syndromes?

OBJECTIVE: Microvascular compression syndromes such as trigeminal neuralgia, hemifacial spasm, and disabling positional vertigo involve an artery or vein compressing a cranial nerve. A cranial nerve is composed of a central nervous system (CNS) segment and a peripheral nervous system (PNS) segment separated by the root entry/exit zone (REZ). Although vascular compression can occur at any point along the cranial nerve, it has been generally assumed that only vascular contact at the REZ of the affected cranial nerve can cause symptoms. On the basis of personal surgical experience, we propose tha

The clinical triad: a structured approach to diagnosing peripheral nerve compressions.

PURPOSE: Peripheral nerve compression syndromes are a common cause of pain, weakness, and functional limitations, yet they often remain underdiagnosed due to the limitations of traditional diagnostic methods such as electromyography and imaging. This article describes the clinical triad-manual muscle testing (MMT), sensory-collapse testing (SCT), and pain evaluation-as a structured, integrative approach to improving the diagnosis of nerve compressions. METHODS: This narrative review examines the anatomical basis and diagnostic application of the clinical triad across common peripheral nerve c

Entrapment neuropathies of the upper extremities.

The three syndromes described in this article were chosen from about a dozen nerve-compression disorders that affect the arm from scapula to digits. They illustrate the range of problems encountered. None of the other syndromes are as common, and in some instances (pronator syndrome and anterior interosseous syndrome), localized inflammation is at least as common a cause of neuropathy as is compression. Some syndromes of nerve compression have important implications for musicians, athletes, and those who place stress on the upper limbs through heavy or repeated use. Well-designed prospective t

The humeral origin of the brachioradialis muscle: an unusual site of high radial nerve compression.

Radial nerve compression is seldom encountered in the upper arm, and most commonly described compression syndromes have their anatomical cause in the forearm. The teres major, the triceps muscle, the intermuscular septum region and the space between the brachialis and brachioradialis muscles have all been identified as radial nerve compression sites above the elbow. We describe the case of a 38-year-old male patient who presented with dorso-lateral forearm pain and paraesthesias without neurological deficit. Surgical exploration revealed radial nerve compression at the humeral origin of the br

Therapy of Vestibular Paroxysmia, Superior Oblique Myokymia, and Ocular Neuromyotonia.

Neurovascular compression syndromes are characterized by recurrent attacks of neurological symptoms and clinical signs depending on the cranial nerve affected. It is assumed that pulsatile compression of the nerve is caused mainly by an artery. The result is segmental demyelination of the transition zone or the central part of the cranial nerve, which is covered by oligodendrocytes, and subsequent ephaptic axonal transmission. Compression of the vestibular nerve can cause attacks of spinning or non-spinning vertigo: vestibular paroxysmia. Compression of the trochlear nerve is characterized by

[Electromyography and electroneurography in peripheral nerve lesions. Part II].

Electromyographical and electroneurographical examinations are indicated in the forefield of the surgical treatment for radicular syndromes, traumatic plexus and nerve lesions as well as compression or bottle-neck syndromes. The electrophysiological findings point out the neurogenic nature of the disturbance. They furthermore furnish statements with respect to the localisation, the extent, the kind and the prognosis of the lesions. The electromyographical and electroneurographical examination programme is determined by the anatomical innervation conditions and the questions that are of clinica

Extradural corticosteroid injection in management of lumbar nerve root compression.

The effect of extradural corticosteroid injection in patients with nerve root compression syndromes associated with degenerative disease of the lumbar intervertebral discs was assessed in a double-blind controlled trial on 100 consecutive inpatients assigned by random allocation to treatment and control groups. Assessment during admission and at three months revealed statistically highly significant differences in respect of relief of pain and resumption of normal occupation in favour of the group treated by extradural injection. This treatment seems to be a valuable adjunct to the management

The silent scream: Unraveling vagal nerve neuralgia; VANCOUVER syndrome and HeLPS.

Cranial nerves are mechanically irritated by vascular compression in neurovascular compression syndromes. Vagus nerve neuralgia is one of the less prevalent types of Neurovascular Compression Syndromes. It can cause disorders like Hemilaryngopharyngeal Spasm (HeLPS) and Vagus Associated Neurogenic Cough (VANCOUVER syndrome). HeLPS presents with unilateral spasms of the throat and coughing, whereas VANCOUVER syndrome is typified by a chronic cough brought on by vascular encroachment on the rootlet of the vagus nerve. The diseases' clinical manifestations, diagnosis, differential diagnoses, and

[Posttraumatic nerve entrapment syndromes in the upper extremities].

Nerve entrapment syndromes in the upper extremities are common clinical disease patterns, less often as direct results of accidents. The most frequent compression syndrome is the carpal tunnel syndrome followed by the cubital tunnel syndrome. If the cause of the compression cannot be eliminated by conservative treatment options, an operative therapy is necessary. As the prognosis becomes worse with the duration of the nerve compression, it is important to initiate therapy at an early stage.

Ulnar Tunnel Syndrome, Radial Tunnel Syndrome, Anterior Interosseous Nerve Syndrome, and Pronator Syndrome.

In addition to the more common carpal tunnel and cubital tunnel syndromes, orthopaedic surgeons must recognize and manage other potential sites of peripheral nerve compression. The distal ulnar nerve may become compressed as it travels through the wrist, which is known as ulnar tunnel or Guyon canal syndrome. The posterior interosseous nerve may become entrapped in the proximal forearm as it travels through the radial tunnel, which results in a pain syndrome without motor weakness. The median nerve may become entrapped in the proximal forearm, which can result in a variety of symptoms. Spontan

The Reversed Palmaris Longus: Sonographic Findings and Anatomical Correlation With Implications for Carpal Tunnel Syndrome Diagnosis and Management.

Anomalous muscles of the forearm, including a reversed palmaris longus, may present as masses or median nerve compression syndromes, such as carpal tunnel syndrome. In both cases, ultrasound may be used as a first-line diagnostic test. This report presents a case of a reversed palmaris longus with sonographic findings and anatomic correlation. The diagnosis of these muscular anomalies, their relationship to compression syndromes, and implications for surgical planning are discussed.

CRANIAL NERVE HYPERFUNCTION SYNDROMES. MODERN APPROACHES TO DIAGNOSIS AND TREATMENT (REVIEW).

In this article, we reviewed neurovascular conflicts of most cranial nerves with characteristic clinical syndromes and analyzed preferred treatment strategies, including surgical methods. We conducted literature review using the Pubmed database, selecting articles on cranial nerves compression syndromes, published for 10 years (from 2010 to 2020). The analysis covered all articles with information on the etiology, pathogenesis, clinical findings, diagnosis, differential diagnosis, neuroimaging, and pathomorphological assessment, as well as treatment strategies for such pathology, including sur

Vertigo caused by basilar artery compression of the eighth nerve.

Vascular compression syndromes in the posterior cranial fossa have become well described clinical entities, especially for the fifth and seventh cranial nerves. Jannetta has proposed vascular compression of the eighth nerve as the etiology of hearing loss, tinnitus, and vertigo in some patients. The case that follows illustrates a clear-cut example of vestibular symptoms arising from vascular compression of the eighth nerve. The patient involved had disabling peripheral vertigo refractory to medical management. Magnetic resonance imaging documented a tortuous basilar artery compressing the eig

[Neurovascular compression (conflict)]].

Since Dandy first reported vascular compression of the trigeminal nerve, the concept of neurovascular compression syndrome for trigeminal neuralgia and hemifacial spasm (HFS) has been accepted, and neurovascular decompression has been performed for this condition. The further investigations indicated that some other clinical syndromes such as glossopharyngeal neuralgia, disabling positional vertigo, tinnitus, geniculate neuarlgia, spasmodic torticolis, essential hypertension, cyclic oculomotor spasm with paresis and superior oblique myokymia also may be initiated by vascular compression of the

Nerve Entrapments.

There are more than 2 dozen nerve entrapment syndromes in the body. Generally, these occur at sites of fibroosseous or fibromuscular tunnels. Any insult that leads to an increase in the size of the nerve or a decrease in the volume of the tunnel will cause compression. Resultant nerve ischemia sets off a cascade of events that lead to predictable clinical signs and symptoms. Here, we review the most common nerve entrapment syndromes and highlight their assessment and management. Specific clinical scenarios that require a high suspicion for nerve entrapment are highlighted.

Recognizing specific characteristics of nonspecific low back pain.

A retrospective review of 1293 cases of low back pain treated over a 12-year period revealed that sacroiliac joint syndrome and posterior joint syndromes were the most common referred-pain syndromes, whereas herniated nucleus pulposus and lateral spinal stenosis were the most common nerve root compression lesions. Referred pain syndromes occur nearly twice as often and frequently mimic the clinical presentation of nerve root compression syndromes. Combined lesions occurred in 33.5% of cases. Lateral spinal stenosis and herniated nucleus pulposus coexisted in 17.7%. In 30% of the cases of spond

[Ulnar nerve compression in Guyon's canal by an accessory abductor digiti minimi brevis muscle].

Compression syndromes of the ulnar nerve are more often located proximally in the cubital tunnel than distally in Guyon's canal. In many cases anatomic structures can be found that induce compression. These may be ganglia, tumours, vascular abnormalities, crossing fibrous bands or accessory muscles. The ulnar nerve has therefore to be revised all along its passage through Guyon's canal. When other causes are excluded, it might be necessary to excise an accessory muscle to decompress the ulnar nerve sufficiently.
