Complete Foot Drop With Normal Electrodiagnostic Studies: Sunderland "Zero" Ischemic Conduction Block of the Common Peroneal Nerve.

Common peroneal neuropathy is a peripheral neuropathy of multifactorial etiology often left undiagnosed until foot drop manifests and electrodiagnostic abnormalities are detected. However, reliance on such striking symptoms and electrodiagnostic findings for diagnosis stands in contrast to other commonly treated neuropathies, such as carpal tunnel and cubital tunnel syndrome. Poor recognition of common peroneal neuropathy without foot drop or the presence of foot drop with normal electrodiagnostic studies thus often results in delayed or no surgical treatment. Our cases document 2 patients pre

Changes in Nerve Conduction Studies After Distal Radius Fracture Fixation Using a Volar Approach and Locked Plate.

PURPOSE: To determine whether there are changes in nerve conduction studies (NCS) of the median nerve after distal radius fracture (DRF) and to determine how operative fixation through a volar approach with a locking plate contributes to nerve conduction changes. We hypothesized that a considerable percentage of patients would have electrodiagnostic evidence of median neuropathy at the wrist after fracture, but fixation with a volar locked plate would not worsen the electrodiagnostic findings. METHODS: This was a prospective cohort study of 14 neurologically asymptomatic patients who underwen

The Sensitivity and Specificity of Nerve Conduction Studies for Diagnosis of Carpal Tunnel Syndrome: A Systematic Review.

Background: The utility of nerve conduction studies (NCS) for diagnosis of carpal tunnel syndrome (CTS) has continued to be a subject of debate. Proponents of NCS assume a high sensitivity and specificity; however, many are unaware of the actual literature on this topic and the cutoff values commonly used for diagnosis. The purpose of this systematic review of the literature is to report the sensitivity and specificity of NCS for diagnosis of CTS in various studies. Methods: A literature review of PubMed and EMBASE databases was performed for all articles on NCS for diagnosis of CTS. The outco

Tibial nerve motor conduction with recording from the first dorsal interosseous: a comparison with standard tibial studies.

PURPOSE: The lateral plantar nerve (LPN) branch of the tibial nerve provides the primary supply to the first dorsal interosseous (FDI) muscle of the foot. A consistent clear response has been observed with recording of the LPN from the FDI muscle. This clarity is not always found with the standard recording site for the LPN. As there are no published normative values for the FDI recording site, the purpose of this study is to describe these values and compare them with standard tibial values. METHODS: Fifty four healthy adult volunteers with 102 limbs were studied to determine the nerve condu

Levels of agreement of nerve conduction studies and symptoms in workers at risk of carpal tunnel syndrome.

OBJECTIVE: Information is required on levels of agreement between different sets of median nerve conduction studies (NCS) and symptoms of carpal tunnel syndrome (CTS) in at-risk subjects. METHODS: We performed a reanalysis of an extensive data set from a population of 114 "highly exposed" meat workers (without prior diagnosis of CTS), who were interviewed for the presence of current symptoms typical of CTS and systematically submitted to median NCS. RESULTS: Agreement between presence/absence of symptoms and NCS normal/abnormal findings ranged from 81% for wrist sensory latency (WSL) to 49%

Clinical trials for polyneuropathy: the role of nerve conduction studies, quantitative sensory testing, and autonomic function testing.

Clinical trials to assess the treatment of diabetic and other forms of polyneuropathy are becoming increasingly common. Nerve conduction studies, quantitative sensory testing, and autonomic function testing are often used in these trials. This article reviews the sensitivity and reproducibility of these measures to detect change in peripheral nerve function during long-term trials. The attributes of nerve conduction studies that are likely to be most useful are summated or averaged sensory nerve action potential amplitudes and averaged motor nerve conduction velocities. Summated or averaged co

Intracutaneous Amyloid Deposition is Associated With Nerve Conduction Studies Deterioration in Presumed Asymptomatic Pathogenic Variant TTR Carriers.

BACKGROUND: Hereditary transthyretin amyloidosis (ATTRv amyloidosis) is an autosomal dominant systemic disease, with an overall poor prognosis. Markers of disease onset are urgently required to optimize the timing of treatment initiation. Nerve conduction studies (NCS) are an objective, reproducible, and non-invasive tool for following large nerve fiber involvement. Our objective was to determine whether the presence of intracutaneous amyloid deposition (ICAD) was associated with a higher risk of developing the disease, defined as a decline observed on nerve conduction studies, in a population

Utility of phrenic nerve conduction studies for identification of patients with neuromuscular diseases requiring invasive mechanical ventilation.

Источник пометил публикацию как отозванную

INTRODUCTION/AIMS: Predicting when a patient will require invasive mechanical ventilation (IMV) is a major challenge in routine care for some neuromuscular diseases. In this study, we prospectively investigated whether phrenic nerve conduction studies (PNCS) can predict when IMV will be required in patients with amyotrophic lateral sclerosis (ALS), Guillain-Barré syndrome (GBS), chronic inflammatory demyelinating polyneuropathy (CIDP), and myotonic dystrophy (DM). METHODS: PNCS amplitude (avAMP) and latency (avLAT) were compared between patients who required IMV (IMV group) and those who did

Comparison of Different Sequences of Magnetic Resonance Imaging and Ultrasonography with Nerve Conduction Studies in Peripheral Neuropathies.

BACKGROUND: Peripheral neuropathies refer to a group of disorders in which there is damage to the nerves of the peripheral nervous system. Electrophysiologic studies are the main stay for the diagnosis of peripheral neuropathies. However, direct visualization of the nerves is possible with exact localization of site of disease with high-resolution ultrasonography (USG) and 3-Tesla magnetic resonance imaging (MRI) scanner, and newer magnetic resonance (MR) sequences. METHODS: We performed a cross-sectional study including 55 patients and 64 nerves with upper limb peripheral neuropathies. All p

Two sets of nerve conduction studies may suffice in reaching a reliable electrodiagnosis in Guillain-Barré syndrome.

OBJECTIVE: Recent studies have advocated the use of serial nerve conduction studies (NCS) in the electrodiagnosis of Guillain-Barré syndrome (GBS). The current study aims to elucidate when and how frequent NCS can be performed to reflect the disease pathophysiology. METHODS: A prospective study of GBS patients documenting the initial and final electrodiagnoses following serial NCS performed at three time intervals: 1-2 weeks, 3-8 weeks and 8-12 weeks. RESULTS: Twenty-one patients were recruited over a period of 2 years. Electrodiagnosis within 2 weeks revealed 17 acute inflammatory demyelina

Improvement of diagnostic rate of carpal tunnel syndrome with additional median-to-ulnar comparative nerve conduction studies.

PURPOSE: The aim of this study is to assess whether additional median -to-ulnar comparative tests will improve the diagnostic rate of carpal tunnel syndrome (CTS). METHODS: We recruited 248 hands of 162 CTS patients, and 166 hands of 83 controls. One hundred and sixty-eight (68%) symptomatic hands had abnormal median distal latencies or palm-wrist latencies. We performed three additional comparative tests in the remaining symptomatic hands and the non-CTS hands. The first test compared median distal motor latency (MDL) recorded from the second lumbrical muscle (2L) and ulnar distal latency re

Dorsal caudal tail and sciatic motor nerve conduction studies in adult mice: technical aspects and normative data.

Mice provide an important tool to investigate human neuromuscular disorders. The variability of electrophysiological techniques limits direct comparison between studies. The purpose of this study was to establish normative motor nerve conduction data in adult mice. The dorsal caudal tail nerve and sciatic nerve motor conduction studies were performed bilaterally on restrained anesthetized adult mice. The means and standard deviations for each electrophysiological parameter were determined in normal mice. Data were compared with inflammatory demyelinating polyneuropathy mice to determine whethe

Motor nerve-conduction studies in obstetric brachial plexopathy for a selection of patients with a poor outcome.

BACKGROUND: The criteria and timing for nerve surgery in infants with obstetric brachial plexopathy remain controversial. Our aim was to develop a new method for early prognostic assessment to assist this decision process. METHODS: Fifty-four patients with unilateral obstetric brachial plexopathy who were ten to sixty days old underwent bilateral motor-nerve-conduction studies of the axillary, musculocutaneous, proximal radial, distal radial, median, and ulnar nerves. The ratio between the amplitude of the compound muscle action potential of the affected limb and that of the healthy side was
