Multidisciplinary approach to Fabry disease: from the eye of a neurologist.

Fabry Disease (FD) is an X-linked lysosomal storage disease that emerges as a result of the mutations in the galactosidase A gene encoding alpha-galactosidase. The peripheral nervous system (PNS) involvement manifests itself as acroparesthetic complaints due to the small-fiber involvement. Our goal was to assess the PNS involvement of 14 patients with FD both clinically and electrophysiologically besides the other systemic features. 14 patients (11 female and 3 male) of the same family whose enzyme level and genetic mutation analysis confirmed the FD diagnosis were evaluated retrospectively in

Constantin Tsiminakis (1875-1942): Neurologist, Neuropathologist, Statesman.

The article is a 75-year memorial tribute to the Greek neurologist, Constantin Tsiminakis (1875-1942). Coming from a family of physicians, Tsiminakis graduated from the University of Athens in 1897, and trained in Vienna under Nothnagel, Frankl-Hochwart, and Obersteiner. In 1905, he was appointed Reader in Neurology and Psychiatry at his alma mater. He published over 40 articles on topics of neurology and neuropathology, including megalencephaly, hydrocephalus, progressive paralysis, epidemic encephalitis, dengue fever, and narcolepsy. However, his main focus of interest was epilepsy, includin

A survey of neurologists on the likeability of headaches and other neurological disorders.

BACKGROUND: A pilot survey of 94 neurologists attending a continuing medical education meeting was performed to assess whether neurologists like to treat headaches and other common disorders and evaluate their personal prevalence of the disorders. METHODS: Physicians were asked to respond to the following statement using a 5-point Likert scale (from 1, strongly disagree to 5, strongly agree): "I like to treat patients with this disease or symptom." RESULTS: The response rate was 46% with a mean age of 52.5 years. The respondents liked to treat migraine (mean = 4.32) similarly to carpal tunne

[Stendhal syndrome: origin, characteristics and presentation in a group of neurologists].

INTRODUCTION: Travelling, when searching for knowledge and emotion, can cause psychic discomfort that occasionally leads the traveller to seek medical attention. The psychiatrist Graziella Magherini described, in tourists visiting Florence, acute attacks including disorders of thought and affects, and even including, anxiety attack. She named it the Stendhal syndrome (SS) remembering the experience of the writer when visiting the Basilica of Santa Croce in Florence. METHODS: We attempt to investigate the incidence of SS or isolated symptoms related to it, in a homogeneous group of travellers.

Management of optic neuritis in Canada: survey of ophthalmologists and neurologists.

BACKGROUND: Acute isolated optic neuritis is often the first manifestation of multiple sclerosis (MS), and its management remains controversial. Over the past decade, with the advent of new disease-modifying agents, management of isolated optic neuritis has become more complicated. OBJECTIVES: To evaluate the current practice patterns of Canadian ophthalmologists and neurologists in the management of acute optic neuritis, and to evaluate the impact of recently published randomized clinical trials. DESIGN: Mail survey. METHODS: All practicing ophthalmologists and neurologists in Canada were

[Diagnosis and treatment of carcinoid meningitis: a challenge to the neurologist and oncologist].

BACKGROUND: Two clinical types of leptomeningeal metastases from solid tumors are observed: local and disseminated. The former (meningeal carcinomatosis) consists in nodular infiltration of leptomeninges, while the latter (carcinomatous meningitis)--in tumor cells free floating in the cerebrospinal fluid and adhering as a monolayer to the surface of neural structures. Despite the same etiology, the two types of metastasis differ in their clinical manifestation and prognosis. Meningeal carcinomatosis is more frequently diagnosed nowadays due to advances in neuroimaging techniques and the long s

Neurological aspects of biological and chemical terrorism: a review for neurologists.

The Centers for Disease Control and Prevention urge physicians to become familiar with chemical and biological weapons. Preparedness among neurologists is especially important because several of these agents affect the nervous system. This article reviews 4 agents that have a history of military or terrorist use: cyanide poisons, organophosphate poisons, botulinum toxin, and anthrax. Cyanide and organophosphate poisons are characterized by dose-dependent impairment of neurological function with nonspecific symptoms such as headache or dizziness at one end of the spectrum and convulsions and co

[Thrombosis in the bifurcation of the abdominal aorta through the eyes of a neurologist].

We described a case of the malign course of the abdominal aorta bifurcation in a 75-year old woman with the case history of a successfully removed neurinom of the eleventh thoracic root on the left side. Despite a timely diagnosis with the recurrence of a spinal tumor being ruled out and followed by the streptokinase therapy, the course of the disease was lethal. Another six lethal cases of the thrombosis of the abdominal aorta studied retrospectively proved the necessity to improve both the diagnostics and the therapy of this disease. The possibilities of modern angiosurgery are, however, bei

Artificial intelligence versus neurologists: A comparative study on multiple sclerosis expertise.

INTRODUCTION: Multiple sclerosis (MS) is an autoimmune neurodegenerative disease affecting the central nervous system. MS diagnosis is complex, requiring magnetic resonance imaging and cerebrospinal fluid analysis due to the lack of definitive biomarkers. Although treatment advancements have reduced disability, diagnostic and therapeutic challenges remain, even among MS-specialized neurologists. Artificial Intelligence (AI) tools, which analyze large datasets, are increasingly used in healthcare, especially for diagnosis and treatment. OBJECTIVES: This study aims to assess the accuracy and sc

Chronic glucocorticoid management in neuromuscular disease: A survey of neuromuscular neurologists.

INTRODUCTION/AIMS: Glucocorticoids (GC) are first-line therapy for many neuromuscular diseases. There is a lack of guidelines regarding the prevention and management of GC complications in the context of neuromuscular disease, introducing the potential for practice variation, that may compromise quality of care. Our aim was to evaluate the practice patterns among Canadian adult neuromuscular neurologists on the screening, management, and treatment of GC-related complications and to identify variances in practice. METHODS: A web-based anonymous questionnaire was disseminated to 99 Canadian adu

Acute psychomotor agitation - challenges for psychiatrists and neurologists: a case study.

PURPOSE: Bilateral thalamic ischemia can, paradoxically, manifest itself with various non-sensory clinical symptoms, thereby complicating diagnosis. CASE DESCRIPTION: A 59-year-old woman was admitted to an Emergency Department about and hour and a half after she was found with altered mental status. Her initial symptoms were psychomotor agitation and slight confusion. No evident focal neurological deficit was observed at that time and computed tomography was negative. After acute drug intoxication was excluded, a transfer to a psychiatric unit was considered. Simultaneously, short stenosis of

Coordination of Care Among Physical Therapists and Neurologists in Huntington Disease.

This study presents a framework for physical therapy through the course of Huntington disease (HD) which includes coordinated care plans with neurologists. HD is an inherited neurodegenerative disorder that leads to impaired strength and coordination and ultimately progressive loss of function. Interdisciplinary HD care teams provide patient-centered, comprehensive evaluations and make recommendations for pharmacologic, healthcare, and lifestyle interventions based on best available evidence. Physical therapists work to improve movement and mobility using specific therapeutic interventions and

[Persecuted and forgotten? The neurologists Kurt Goldstein and Friedrich Heinrich Lewy].

This paper commemorates the careers and the scientific influence of the clinical neurologists Kurt Goldstein and Friedrich Heinrich Lewy including their forced migration in the mid-1930s. Goldstein (1878-1965) set up independent neurological departments in Frankfurt/Main and Berlin, adopting a decidedly holistic approach in medical care, research and teaching. He is therefore considered a co-founder of modern neuropsychology and neurorehabilitation. Goldstein came into the focus of the National Socialists as a Jew, socialist and adherent of psychotherapeutic methods. After a short incarceratio

Treatment of post-stroke aphasia: A narrative review for stroke neurologists.

This review is intended to help physicians guide patients to optimal management of post-stroke aphasia. We review literature on post-stroke aphasia treatment, focusing on: (1) when and for whom language therapy is most effective, (2) the variety of approaches that can be effective for different individuals, and (3) the extent to which behavioral therapy might be augmented by non-invasive brain stimulation and/or medications.

Risk perception in natalizumab-treated multiple sclerosis patients and their neurologists.

BACKGROUND: Natalizumab is associated with the potentially life-threatening side-effect progressive multifocal leukoencephalopathy (PML). Little is known about patients' and physicians' risk estimates and attitudes towards natalizumab treatment. METHODS: Consecutive natalizumab-treated patients (n = 69) and neurologists (n = 66) in two centres and cooperating private practices received an evidence-based three-page information leaflet about natalizumab-associated PML and an evaluation sheet. RESULTS: After reading the information, patients were significantly more likely than physicians to int

What should neurologists expect to observe in relapsing polychondritis and VEXAS?

Relapsing polychondritis (RP) and VEXAS syndrome (Vacuoles, E1 enzyme, X-linked, Autoinflammatory, Somatic) are two clinically overlapping conditions in which neurological complications remain uncommon. Based on the existing literature, neurological involvement in RP most frequently comprises meningitis, encephalitis, cranial nerve palsies, and stroke, whereas VEXAS is additionally characterized by peripheral nerve and muscle involvement. This scoping review summarizes the neurological manifestations associated with RP and VEXAS and highlights the current gaps in knowledge regarding these comp
