Primary percutaneous coronary intervention in nonagenarians: six-month outcomes from a single-center registry.

Little is known about the efficacy and medium-term outcomes of primary percutaneous coronary intervention (PCI) in very old patients. We evaluated in-hospital and 6-month outcomes in a retrospective cohort of nonagenarian patients presenting at our hospital with ST-segment elevation myocardial infarction (STEMI) and treated by primary PCI from January 2003 to May 2012. During this period, primary PCI was performed in 1598 consecutive patients; twenty-seven patients (age, 92.5 ± 2.5 years) were enrolled in the study. Four patients (15%) were in advanced Killip class at presentation. STEMI locat

Radiotherapy at the limits of age: evolving fractionation strategies, symptom palliation, and survival in nonagenarians.

BACKGROUND AND PURPOSE: Advances in radiotherapy (RT) techniques have facilitated the increasing use of hypofractionated treatment schedules, which may be particularly advantageous for elderly patients by reducing treatment burden while maintaining acceptable toxicity. This study aimed to evaluate survival and symptom palliation and to investigate whether different fractionation approaches affected toxicity and clinical outcomes in nonagenarian patients. MATERIALS AND METHODS: This single-center retrospective cohort included 199 nonagenarians treated with RT between 2005 and 2023. Fractionati

Feasibility and Safety of Left Bundle Branch Pacing in Nonagenarians: A Single Center Observational Study.

BACKGROUND: Left bundle branch pacing (LBBP) is emerging as a physiological pacing strategy. However, data on its feasibility and safety in nonagenarians (≥90 years) with symptomatic bradycardia are limited. This study aims to evaluate the outcomes of LBBP in this frail, high-risk population. METHODS: A retrospective analysis was conducted on 8 patients aged 90 and older who underwent LBBP between December 2023 and December 2024. Procedural outcomes, complications, and pacing parameters were assessed. RESULTS: LBBP was successfully performed in 8 patients (mean age 93 ± 2 years, mean Clinica

Evaluating the Efficacy and Safety of Pluvicto in Chemotherapy-Ineligible Nonagenarians: A Descriptive Case Series.

Nonagenarians with metastatic castration-resistant prostate cancer (mCRPC) are often underrepresented in prostate cancer research due to their limited numbers among treated populations. For this group, traditional chemotherapy carries significant side effects that can severely impact quality of life. Targeted therapy designed to selectively bind to prostate cancer cells expressing the prostate-specific membrane antigen (PSMA) presents a promising alternative, particularly for those with widespread metastatic disease and contraindications to chemotherapy. This retrospective case series evaluate

Impact of surgical intervention on ankle fractures in octogenarians and nonagenarians: a comprehensive outcome analysis.

This study assesses clinical outcomes and quality of life (QoL) following surgical treatment of fragility ankle fractures in individuals aged over 80 years. We conducted a retrospective study involving 45 elderly patients who underwent surgery for fragility ankle fractures between 2015 and 2023. Pre-operative mobility was assessed using the Parker Mobility Score (PMS), and nutritional status was evaluated using a short-form mini-nutritional assessment (MNA-SF). The ASA classification and the age-adjusted Charlson Comorbidity Index (aCCI) were used to assess comorbidities affecting outcomes com

Impact of frailty on endoscopic retrograde cholangiopancreatography outcomes in nonagenarians: A United States national experience.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is an essential therapeutic tool for biliary and pancreatic diseases. Frail and elderly patients, especially those aged ≥ 90 years are generally considered a higher-risk population for ERCP-related complications. AIM: To investigate outcomes of ERCP in the Non-agenarian population (≥ 90 years) concerning Frailty. METHODS: This is a cohort study using the 2018-2020 National Readmission Database. Patients aged ≥ 90 were identified who underwent ERCP, using the international classification of diseases-10 code with clinical modifi

Risk Factors and Long-Term Survival of Octogenarians and Nonagenarians Undergoing Transvenous Lead Extraction Procedures.

INTRODUCTION: Transvenous lead extraction (TLE) has become a frequently used tool for the management of complications related to pacemakers, implantable cardiac defibrillators and cardiac resynchronization therapy devices. However, it is still a matter of debate whether the lead extraction procedure is a safe treatment choice in the elderly. METHODS: We collected the clinical information from 3,810 patients undergoing TLE in 2 high-volume centers (Poland and Italy) between 2006 and 2017. We tested risk factors, effectiveness, safety and long-term survival in 3 groups of patients: those aged 8

Acral Plantar Melanoma Mimicking a Diabetic Foot Ulcer in a Nonagenarian: A Case Report.

Acral melanoma on the plantar surface may be clinically overlooked and can resemble common causes of plantar ulceration. In patients with diabetes, chronic plantar lesions are often managed as diabetic foot ulcers, which can delay consideration of alternative diagnoses. We report the case of a very elderly woman with diabetes whose plantar lesion was initially treated as a diabetic foot ulcer but later evolved with features concerning for malignancy. Histopathology confirmed deeply invasive ulcerated acral melanoma. This case highlights the importance of maintaining suspicion for malignant mim

Non-traumatic Intramural Duodenal Hematoma With Acute Pancreatitis in a Nonagenarian: A Case Report.

Intramural duodenal hematoma (IDH) is a relatively rare condition typically managed conservatively. Although IDH frequently causes duodenal obstruction, the development of acute pancreatitis (AP) due to ampulla of Vater compression is uncommon. We report the case of non-traumatic IDH in a 90-year-old man receiving anticoagulant therapy, complicated by AP and persistent duodenal obstruction. Imaging studies revealed an intramural hematoma in the descending portion of the duodenum, without evidence of active bleeding. Despite five weeks of conservative management, the obstruction did not improve

Gallbladder volvulus discovered during laparoscopic cholecystectomy in a nonagenarian patient: a case report.

Gallbladder volvulus is a rare and frequently underdiagnosed condition, particularly in elderly patients. This case demonstrates a 91-year-old female patient with a clinical presentation of cholecystitis, in which, during the laparoscopy, a gallbladder volvulus was revealed. A detorsion of the gallbladder and cholecystectomy were conducted laparoscopically. Besides minor postoperative complications, the patient was discharged home in the same condition as before surgery. This case highlights that surgical management should be considered in elderly patients with operable cholecystitis to reduce

Anesthetic Management of an Urgent Hip Fracture Repair in a Nonagenarian With Critical Aortic Stenosis.

Severe aortic stenosis presents substantial anesthetic risk because fixed left ventricular outflow obstruction limits the ability to compensate for perioperative hemodynamic changes. These risks are further amplified in elderly patients with additional valvular disease and limited physiologic reserve who require urgent noncardiac surgery. We report the anesthetic management of a 96-year-old woman with critical aortic stenosis and concomitant mitral valve disease who required urgent hip fracture repair. Preoperative echocardiography demonstrated severe valvular calcification with a markedly red

Outcomes and Predictors of In-hospital Mortality in Nonagenarians with NSTEMI: A Comparison of PCI and Medical Management.

Nonagenarians, the fastest-growing U.S. age group, face a high burden of Non-ST-Elevation Myocardial Infarction (NSTEMI), yet the utilization and outcomes of percutaneous coronary intervention (PCI) in this population remain poorly understood. The aims of this study were to assess patient characteristics, comorbidities, and in-hospital outcomes in nonagenarians and identify predictors of in-hospital mortality. We analyzed 122,845 hospitalizations with a principal discharge diagnosis of NSTEMI among nonagenarians using the National Inpatient Sample (2015-2019) to compare PCI (8%) versus medical

A Rare Case of Right-Sided Chylothorax Induced by Vertebral Osteophytosis in a Nonagenarian Patient.

UNLABELLED: The present study presents a new approach to boost citric acid production by combining genetic improvement and enzyme inhibition in the fungus Aspergillus niger. A mutant strain, named NA-CYS3, was developed by chemically treating the wild-type strain to make it resistant to L-cysteine HCl, a chemical that normally limits growth. This mutant showed better tolerance and higher productivity. Optimal fermentation conditions including medium volume (50 mL), acidity (pH: 4.5), inoculum size (10%), incubation time (144 h), and temperature (30 °C) were optimized to maximize citric acid yi
