Successful nutritional therapy for superior mesenteric artery syndrome.

Superior mesenteric artery (SMA) syndrome is an uncommon cause of duodenal outlet obstruction. Symptoms and signs suggestive of this condition are nonspecific, and a high index of suspicion coupled with appropriate imaging studies are necessary for diagnosis. We present the case of a 70-year-old man who developed SMA syndrome following prolonged hospitalisation for a surgically treated bleeding duodenal ulcer. His SMA syndrome resolved after successful nonoperative management based on accepted guidelines for nutritional therapy, thus avoiding the need for reoperation and its attendant risks in

Enteral nutritional therapy for induction of remission in Crohn's disease.

BACKGROUND: The role of enteral nutrition in Crohn's disease is controversial. Increasing research on the mechanisms by which nutritional therapy improves the clinical well being of patients with Crohn's disease has led to novel formula design and trials comparing two different forms of enteral nutrition. This meta-analysis aims to provide an update on the existing effectiveness data for both corticosteroids versus enteral nutrition and for one form of enteral nutrition versus another for inducing remission of active Crohn's disease. OBJECTIVES: To evaluate the effectiveness of exclusive ente

Systematic review: nutritional therapy in paediatric Crohn's disease.

BACKGROUND: At least 25% of individuals diagnosed with Crohn's disease (CD) have onset of disease in childhood. Almost all children with CD have nutritional impairments, such as weight loss or stunting, at diagnosis or subsequently. Nutritional therapy (exclusive enteral nutrition) is established as a valid and effective treatment in paediatric CD. The advantages of this approach are induction of remission and control of inflammatory changes, mucosal healing, positive benefits to growth and overall nutritional status, and avoidance of other medical therapies. AIM: To provide a comprehensive u

[Nutritional therapy in gastrointestinal diseases. Diets--necessary or superfluous?].

Diets form a part of the treatment concept in numerous gastrointestinal diseases. Their effectiveness, however, varies considerably from one disease to another. Thus, for example, diet is of decisive importance in celiac disease and lactose intolerance. In contrast, dietary measures are ineffective in the treatment of gallstones, and uncertain as a prophylactic measure against biliary colic. While dietetic measures are an important temporary measure in acute pancreatitis, in chronic pancreatitis such an approach is often not complied with, since it includes abstinence from alcohol. In chronic

Cell culture as a tool for identifying nutritional disease therapies.

Cell culture methodologies can be used to help develop therapies for the treatment of polytrauma. An overview is presented of research with the L6 myoblast cell line that led to the discovery of the truncated insulin-like growth factor (IGF) variant, des(1-3)IGF-I and that gave an explanation of the enhanced potency of this growth factor. Subsequent efforts to develop an even more potent variant also utilized cultured cells as indicator bioassays. Such experiments not only provided mechanistic information on IGF action but also directed attention towards IGF variants that were later shown to b

Malnutrition and the role of nutritional support for radiation therapy patients.

The nutritional status of a tumor patient can be negatively influenced by the local and systemic effects of the malignant tumor (tumor cachexia, anorexia, difficult oral food intake), by the effects of the various antitumoral therapy modalities (surgery, radiotherapy, chemotherapy), and by the complications associated with such modalities (anorexia, nausea, vomiting, mucositis, xerostomia, alterations of the smell and taste sensations, odynophagia, dysphagia, maldigestion, malabsorption, diarrhea, steatorrhea, conditioned aversions, radiogenic late effects), as well as by the psychological rea

Drug therapy and nutritional management of patients with gastrointestinal fistulas.

The drug therapy and nutritional management of patients with gastrointestinal fistulas are reviewed. The goal of therapy is to compensate for loss of electrolytes, nutrients, and water in the fistula drainage and to monitor the acid-base status of the patient. Positive nitrogen balance is essential for anabolism and closure of the fistula. Usually it is not feasible to analyze the fistula drainage for nitrogen content, and nitrogen requirements must be estimated. To prevent folic acid deficiencies, patients should receive daily doses of folic acid 1 mg orally or 0.5 mg in parenteral nutrient s

Effects of therapy on nutritional status of the pediatric cancer patient.

Children with cancer are at high risk for major nutrition problems both from the tumor itself and from the treatment administered. Overt malnutrition is seen in as many as 17% of children with newly diagnosed localized tumors and 37% of those with metastatic disease. Weight loss in children with cancer is directly correlated with a poor nutritional status at the time of diagnosis and with a low serum albumin. Massive surgical resections are a common source of nutritional problems and are usually not indicated as primary therapy in children with cancer. Both radiotherapy and chemotherapy are as

Alterations of nutritional status: impact of chemotherapy and radiation therapy.

The nutritional status of a cancer patient may be affected by the tumor, the chemotherapy and/or radiation therapy directed against the tumor, and by complications associated with that therapy. Chemotherapy-radiotherapy is not confined exclusively to malignant cell populations; thus, normal tissues may also be affected by the therapy and may contribute to specific nutritional problems. Impaired nutrition due to anorexia, mucositis, nausea, vomiting, and diarrhea may be dependent upon the specific chemotherapeutic agent, dose, or schedule utilized. Similar side effects from radiation therapy de

Personalized Medical Nutrition Therapy and Physical Exercise: The Future of Diabesity Care.

PURPOSE OF THE REVIEW: Diabesity, the coexistence of type 2 diabetes mellitus (T2DM) and obesity, represents one of the major global health challenges. This review aims to synthesize current evidence on personalized medical nutrition therapy (MNT) and structured physical exercise as cornerstones of diabesity management, with a particular focus on very-low-energy ketogenic therapy (VLEKT). RECENT FINDINGS: Conventional pharmacotherapies improve glycemic control and promote weight reduction but often fail to fully address the multifactorial pathophysiology of diabesity. MNT has demonstrated sig

Nutrition delivery in infants with kidney failure on extracorporeal kidney replacement therapy.

BACKGROUND: Infants with kidney failure who initiate kidney replacement therapy (KRT) within 3 months of life require optimal fluid and nutritional management to support growth. Continuous or prolonged intermittent KRT (CKRT or PIKRT) is often used as a bridge to peritoneal dialysis facilitating earlier nutrition provision but is associated with protein losses. Current guidelines recommend ≥ 3-3.5 g/kg/day of protein during KRT. We aimed to assess nutritional prescriptions and growth in these infants. METHODS: We conducted a retrospective review of infants with kidney failure managed with ext

Evolving nutrition therapy in cystic fibrosis: Adapting to the CFTR modulator era.

Cystic fibrosis transmembrane regulator (CFTR)-directed therapies, such as modulators, have transformed the medical management of people with CF, resulting in better lung function, weight, and body mass index in recent years. With improved nutrition status in people on CFTR modulators, the emphasis on a high-energy, high-fat diet (the legacy CF diet) is declining, with an increased focus on a healthy diet. The increased survival and median predicted age of people with CF have created a need for more attention to metabolic diseases, including hypertension, dyslipidemia, and cardiovascular disea

Nutrition therapy in a pediatric intensive care unit: indications, monitoring, and complications.

BACKGROUND: Nutrition therapy (NT) is essential for the care of critically ill children. Inadequate feeding leads to malnutrition and may increase the patient's risk of morbidity and mortality. The aim of this study was to describe the NT used in a tertiary pediatric intensive care unit (PICU). METHODS: The authors evaluated NT administered to 90 consecutive patients who were hospitalized for 7 days in the PICU of Instituto da Criança, Hospital das Clínicas, Universidade de São Paulo, Brazil. NT was established according to the protocol provided by the institution's NT team. NT provided a bal

Nutrition support therapy in acute kidney injury: distinguishing dogma from good practice.

Acute kidney injury (AKI) is a frequently observed complication in critically ill patients. Its presentation may range from the early risk of renal dysfunction to complete renal failure. Morbidity and mortality in the AKI patient increase with the decline of renal function. Appropriate nutrition therapy is essential in the medical management of the AKI patient. Assessment of nutritional requirements should take into account the patient's underlying complication, comorbid medical conditions, and severity of the renal dysfunction. Various stages of AKI determine the direction of nutrition therap

[Life-threatening lactic acidosis during total parenteral nutrition. Successful therapy with thiamine].

Two patients, 37 and 44 years old, respectively, developed severe metabolic acidosis after abdominal surgery which was followed by three weeks of total parenteral nutrition. Septicaemia, peritonitis or hypoxia were excluded as possible causes. Both patients had very high serum lactate concentrations (24.3 and 22.8 mmol/l, respectively). Conventional treatment with buffer agents was unsuccessful. Because vitamin B1 deficiency was suspected, two doses of 400 mg thiamine were administered. In both patients the extreme lactic acidosis disappeared immediately after the injections. Both patients wer

Advanced nutrition support therapy practices of registered dietitian nutritionists: A survey of the Academy of Nutrition and Dietetics' Dietitians in Nutrition Support Dietetics Practice Group and the American Society for Parenteral and Enteral Nutrition Dietetics Practice Section.

Nutrition support therapy (NST) requires continuous monitoring and interdisciplinary collaboration to optimize nutrition status and prevent complications. Registered dietitian nutritionists (RDNs) are pivotal in managing NST, using their expertise to assess malnutrition risk, evaluate suitable nutrition routes, and lead teams in evidence-based nutrition care. However, not all RDNs operate at an expert level owing to varying education, training, and clinical experiences. A survey to determine expert-level RDN NST practices was created and distributed through the Dietitians in Nutrition Support

Nutrition-Based, High-Protein Adjunct Therapy for Hashimoto's Thyroiditis: A Case Report.

Hashimoto's thyroiditis (HT) can cause symptoms as weight gain and fatigue, or with more drastic ones like lowered blood pressure, low resting heart rate, or manifestation of nutritional deficiencies. While diagnostic criteria for HT are aimed at detecting abnormalities in thyroid hormones, there is scarce data on nutritional markers for concerns related to the manifestation of thyroid disease symptoms. We report the case of a 21-year-old overweight female with a 10-year history of HT. The patient developed severe bilateral lower extremity muscle weakness and fatigue, severe enough to restrict
