Obsessive compulsive disorder in children and adolescents.

During 1992, further progress has been made in determining the incidence of obsessive compulsive disorder and change in symptoms over time. Advances have been made in determining what category of mood disorder obsessive compulsive disorders belong to; contributions have been made in the theoretic understanding of the cause of obsessive compulsive disorder, and why it may be associated with trichotillomania and Tourette's syndrome. Finally, the treatment of children with both medication and behavior therapy has been explored.

A family study of obsessive-compulsive disorder.

First-degree relatives of probands with obsessive-compulsive disorder (OCD) (n = 32) and psychiatrically normal controls (n = 33) were blindly interviewed with the use of the Diagnostic Interview Schedule. The morbidity risk for anxiety disorders was increased among the relatives of obsessional subjects compared with that for the relatives of controls, but the risk for OCD was not. Risk for a more broadly defined OCD (including relatives with obsessions and compulsions not meeting criteria for OCD) was increased among the parents of obsessional subjects but not among the parents of controls (1

Monoamine oxidase inhibitors in obsessive-compulsive disorder.

The authors describe the use of monoamine oxidase inhibitors (MAOIs) in eight patients with severe obsessive-compulsive disorder. In four cases, the MAOIs produced rapid and sustained remission of symptoms; no response was seen in four other patients. All patients who responded to MAOIs, but none of the nonresponders, had phobic anxiety and/or panic attacks. Thus, a trial of MAOIs is indicated in obsessive-compulsive disorder, especially when phobic anxiety or panic attacks form part of the clinical picture.

Symptom provocation in obsessive-compulsive disorder: Validation of the Braga Obsessive Compulsive image set (BOCIS).

Symptom provocation paradigms are paramount to understand a heterogeneous disorder as obsessive-compulsive disorder (OCD). The main aim of our work was to develop and validate an open-access set of OCD-related images comprising three main subtypes: washing, checking, and symmetry. Twenty-six OCD patients and 25 controls provided valence and arousal ratings for a set of OCD-related, aversive, and neutral images. Linear mixed model analyses were used to estimate the main effects of group, image category, and group-image category interaction in image ratings. All main effects were found to be sig

Mental Disorders: Obsessive-Compulsive Disorder and Body Dysmorphic Disorder.

Obsessive-compulsive disorder (OCD) is characterized by recurrent intrusive thoughts, images, or urges (ie, obsessions) and repetitive mental acts or behaviors (ie, compulsions). Body dysmorphic disorder (BDD) is a chronic preoccupation with perceived flaws in physical appearance that leads to repetitive behaviors (ie, rituals). The Diagnostic and Statistical Manual of Mental Disorders (Fifth Edition) (DSM-5) places both disorders under a new category, obsessive-compulsive and related disorders, along with hoarding disorder, excoriation disorder, and trichotillomania. Both OCD and BDD cause ma

Obsessive-compulsive disorder comorbid with schizophrenia and bipolar disorder.

Obsessive Compulsive Disorder (OCD) and Obsessive Compulsive Symptoms (OCS) are known to be highly comorbid with bipolar disorder and schizophrenia. Comorbid OCD/OCS influences the course of schizophrenia and bipolar disorder. There is also some evidence to suggest that a diagnosis of OCD may be associated with elevated risk for later development of psychosis and bipolar disorder. Comorbid OCD/OCS is associated with a greater severity of schizophrenia phenotype and poorer prognosis. In addition, certain atypical antipsychotics, clozapine in particular are known to induce or worsen OCS in schiz

Thought dismissability in obsessive-compulsive disorder versus panic disorder.

This study sought to better understand the persistence of obsessive-compulsive disorder (OCD) by studying the dismissability of obsessional thoughts in individuals with OCD relative to the dismissability of panic-relevant thoughts in individuals with panic disorder with or without agoraphobia (PD/A). Individuals with a principal diagnosis of OCD (n = 25) or PD/A (n = 25) completed a thought replacement task in which they replaced a primary obsession (target thought for the OCD group) or a primary panic-relevant thought (target thought for the PD/A group), with a neutral thought, signaling when

Sensory phenomena in obsessive-compulsive disorder and Tourette's disorder.

BACKGROUND: Recent studies have suggested that obsessive-compulsive disorder (OCD) is a heterogeneous disorder with some forms related to tics and Tourette's disorder. The present study was undertaken to investigate the sensory phenomena in patients with OCD and/or Tourette's disorder to determine if these phenotypic features represent valid clinical indices for differentiating tic-related OCD from non-tic-related OCD. METHOD: We evaluated 20 adult outpatients with OCD, 20 with OCD plus Tourette's disorder, and 21 with Tourette's disorder, using a semistructured interview designed to assess s

Eating disorder history in women with obsessive compulsive disorder.

This study examined the frequency of eating disorders in women with obsessive compulsive disorder (OCD). Letters were sent to area psychiatrists asking them to refer patients diagnosed with OCD to a research study. Subjects completed the Yale Brown Obsessive Compulsive Scale and a survey including history of eating disorders. Thirty-one women participated in the study. Forty-two percent (N = 13) had a past or current history of an eating disorder: 26% anorexia nervosa alone (N = 8); 3% bulimia nervosa alone (N = 1); and 13% both anorexia and bulimia (N = 4). Our study found a higher percentage

Management of obsessive-compulsive disorder in adults.

Obsessive-compulsive disorder (OCD) is a chronic, often debilitating psychiatric condition characterized by intrusive thoughts and behavioral or mental rituals. Although exposure and response prevention (ERP) remains the first line treatment, many individuals do not experience full remission, highlighting the need for innovation. This review provides a comprehensive and up-to-date synthesis of evidence based treatments for OCD in adults, as well as emerging psychological and biological approaches. It first describes established psychological and pharmacological therapies, highlighting recent f

[Diagnosis and treatment of obsessive compulsive disorder].

Obsessive-compulsive disorders (OCD) are common and often chronic mental illnesses, characterized by recurring, intrusive thoughts, ideas and impulses (obsessions) and repetitive, stereotypical behaviors (compulsions). Etiopathogenetically, OCD is explained by a complex interplay of genetics and epigenetics, neurotransmission, imbalances in cortico-striato-thalamo-cortical circuits, as well as learning mechanism and environmental factors. In ICD-11, OCD is now classified in the chapter "Obsessive-compulsive and related disorders", which share compulsive and impulsive behavioral patterns. This

Machine learning in obsessive-compulsive disorder medications.

Obsessive-compulsive disorder (OCD) is the fourth most common psychiatric disorder with a significant morbidity rate. Despite various treatment modalities and medications, some patients show no definitive response. The aim of this study is to classify the medications of OCD with machine learning (ML) methods and to compare the classification performances of the decision tree (DT), chi-square automatic interaction detection (CHAID) algorithm, and linear model in ML methods. This research is a descriptive analytical study based on co-word and artificial intelligence methods. The DT models were c

Gamma Knife radiosurgery for obsessive compulsive disorder.

Gamma Knife radiosurgical capsulotomy has been performed for over 40 years as a rarely used surgical intervention for the treatment of obsessive-compulsive disorder. Over time, the procedure has evolved in many ways with most significant modifications being made in target location, number of isocenters and prescribed dose, subsequently producing changes in lesion size and geometry. Long-term clinical response data and adverse outcomes to the earlier empiric treatment parameters have resulted in shifting the target from its initial location within the midpoint of the anterior limb of internal c

Cognitive-Behavioral Therapy for Obsessive-Compulsive Disorder.

Obsessive-compulsive disorder (OCD) is characterized by the presence of debilitating obsessions and compulsions. Cognitive and behavioral models of OCD provide a strong theoretic and empirical foundation for informing effective psychotherapeutic treatment. Cognitive-behavioral therapy (CBT) for OCD, which includes a deliberate emphasis on exposure and response/ritual prevention, has consistently demonstrated robust efficacy for the treatment of pediatric and adult OCD and is the front-line psychotherapeutic treatment for OCD. Two case vignettes describing CBT for OCD in practice as well as rec
