Updosed Omalizumab in Chronic Spontaneous Urticaria With or Without Symptomatic Dermographism.

BACKGROUND: One-third of patients with chronic spontaneous urticaria (CSU) require treatment beyond the licensed dose of omalizumab, but real-world data on response and predictors of updosed omalizumab remain limited. OBJECTIVES: We evaluated response rates to updosed omalizumab in CSU patients with or without symptomatic dermographism (SD) and explored predictors of response to 450 mg every 4 weeks. METHODS: We retrospectively analyzed 57 patients (27 CSU, 30 CSU + SD) who were poorly responsive to or lost response at licensed-dose omalizumab (300 mg every 4 weeks). All received omalizumab

The Expanding Role of Omalizumab: From Food Allergy to Drug Desensitization.

Although omalizumab is currently approved for a limited number of indications-such as asthma, chronic rhinosinusitis with nasal polyps, and chronic spontaneous urticaria-its potential applications are expanding each year. Owing to its diverse and not yet fully elucidated mechanism of action, including effects on both IgE- and non-IgE-mediated hypersensitivity reactions with delayed onset, this monoclonal antibody may be beneficial in a wide range of allergic and non-allergic conditions. To date, numerous clinical trials and case reports have documented the successful off-label use of omalizuma

Real-Life Effects of Omalizumab on Chronic Rhinosinusitis with Nasal Polyposis.

Chronic rhinosinusitis with nasal polyposis (CRSwNP) is an inflammatory disease of the nasal and sinus mucosa. This inflammatory process is supported by a multitude of cytokines, including IL-4, IL-5, and IL-13 produced by Th2 cells, as well as by IgE produced by B lymphocytes in response to a stimulus. Omalizumab is an anti-IgE monoclonal antibody with well-recognized roles in allergic asthma and chronic spontaneous urticaria. The aim of this study was to evaluate the clinical efficacy of omalizumab in a cohort of 13 patients suffering from chronic rhinosinusitis with CRSwNP. The inclusion cr

Successful treatment of urticarial vasculitis with omalizumab in children: a case series.

Urticarial vasculitis (UV) is a small vessel leucocytoclastic vasculitis, which often needs to be distinguished from urticaria and other dermatoses. Treatment of UV in children is challenging because of the unsatisfying efficacy of antihistamines and the safety concern of long-term systemic corticosteroids or immunosuppressive agents. As a classic biological agent widely used in chronic spontaneous urticaria, omalizumab might also be a potential therapeutic option in the treatment of children with UV. This report presented four children, aged 4-6 years, with glucocorticoid-unresponsive UV succ

Case Report: Combination of Omalizumab and Dupilumab for Recalcitrant Bullous Pemphigoid.

Bullous pemphigoid (BP) is a blistering autoimmune skin disease. Omalizumab, a monoclonal antibody directed to IgE, showed a beneficial effect in treatment of recalcitrant BP in case series. More recently, dupilumab, an interleukin (IL)-4-receptor alpha antagonist, also showed promising preliminary results. We describe a patient with refractory BP who showed a complete response to a combination therapy with omalizumab and dupilumab.

A case of omalizumab as a successful treatment for telangiectasia macularis eruptiva perstans.

Treatment for telangiectasia macularis eruptiva perstans (TMEP) is often challenging due to lack of an established first-line therapy and as such is primarily focused on symptomatic relief. Omalizumab shows promise as a potential therapy for mast cell disorders; however, its efficacy in TMEP is yet to be established. This case describes a 72-year-old woman with chronic refractory TMEP achieving symptomatic remission within 4 months of commencing omalizumab therapy.

Successful treatment of refractory status asthmaticus with omalizumab: a case report.

Refractory status asthmaticus is the cause of rare cases of in-hospital death due to acute bronchial asthma. The most severe cases unresponsive to first, second and next line treatment may be fatal despite aggressive organ support with invasive ventilation and extracorporeal membrane oxygenation. Omalizumab, a humanized recombinant monoclonal anti-IgE antibody, is an approved add-on biological treatment for severe asthma. However, it is not indicated in an acute setting. Here, we report the case of a young patient with status asthmaticus fully dependent on extracorporeal membrane oxygenation r

Long term treatment with omalizumab in adolescent with refractory solar urticaria.

BACKGROUND: Solar urticaria represents an uncomfortable form of chronic inducible urticaria. First and second-line treatments are ineffective in some patients, leading to an impairment in their quality of life. Omalizumab represents a safe therapeutic option in case of refractory solar urticaria. CASE PRESENTATION: We update a case of a 21-year-old Caucasian girl affected by solar urticaria from the age of 14. Poor disease control was achieved with standard or high-dose of H1-antihistamines. Several omalizumab courses, including a 1-year-long course, were practiced resulting in clinical remis

Crusted Scabies Infection in the Setting of Chronic Steroid and Omalizumab Use.

Scabies infection is a very common skin disease that occurs due to infestation with the Sarcoptes scabei mite. Typically, it results in intensely pruritic papules and excoriations in the webs of the hand, groin, or axilla, and remains limited in its spread. In rare cases, the disease can become diffuse and progress to crusted or nodular subtypes. Here, we report the case of crusted scabies infestation in a 69-year-old male who presented with a diffuse pruritic, erythematous, and petechial rash. His medical history was significant for severe idiopathic urticaria treated with omalizumab. Before

A Case of Serum Sickness-Like Reaction in an Adult Treated with Omalizumab.

Omalizumab has been safely used to treat asthma and urticaria. We report a case of serum sickness-like reaction in a patient treated with omalizumab for chronic idiopathic urticaria/angioedema. An adult female experienced episodic urticaria/angioedema without repeatable trigger, ultimately receiving diagnosis of chronic idiopathic urticaria/angioedema. After initial treatment, attempts with escalating cetirizine and montelukast doses were unsuccessful due to sedation; she began treatment with subcutaneous omalizumab 150 mg monthly. Urticaria frequency partially improved after two injections; t

Acute Interstitial Nephritis: A Rare and Unusual Side Effect of Omalizumab.

Acute interstitial nephritis (AIN) is a relatively common cause of acute kidney injury with etiologies that include drug therapy, infections, and systemic diseases. Of these etiologies, drug therapy accounts for ~70% of AIN cases. Although any drug can cause AIN, there are no reported cases of AIN caused by omalizumab, a humanized monoclonal antibody that binds to and inhibits circulating immunoglobulin E. In this article, we share the first reported case of AIN following administration of omalizumab for the treatment of moderate to severe persistent asthma.
