BACKGROUND: Costello syndrome (CS) is a multisystem developmental disorder caused by germline pathogenic variants in HRAS resulting in dysregulation of the Ras pathway. A systematic characterization of ophthalmic manifestations provides a unique opportunity to understand the role of Ras signal transduction in ocular development and guide optimal ophthalmic care in CS individuals.
METHODS: Visual function, ocular features and genotype/phenotype correlations were evaluated in CS individuals harboring HRAS pathogenic variants, by cross-sectional and retrospective studies, and were recruited thro

OBJECTIVE: Giant cell arteritis (GCA) and polymyalgia rheumatica (PMR) are considered as manifestations of a shared disease spectrum (GCA-PMR spectrum disease, GPSD). Data on orbital involvement in isolated PMR and across the spectrum remain limited. This study evaluated inflammatory intraorbital MRI findings across the GCA-PMR spectrum.
METHODS: This retrospective observational study included patients with isolated PMR and combined GCA-PMR (presence of both clinical diagnoses and fulfilling classification criteria of GCA plus PMR). Patients with GCA and no signs and symptoms of PMR were not

IMPORTANCE: The ocular manifestations and sequelae of Zika virus infection are not well known. Recently, the World Health Organization changed the declaration of Zika as a public health emergency and designated the viral outbreak and related microcephaly clusters as a long-term program of work. This change indicates the urgent need to evaluate and document ophthalmic manifestations in patients for timely management of this disease. In addition, confirmation whether the public health problem in Brazil extends to other regions in South America is needed.
OBJECTIVE: To report the ocular manifest

Carotid cavernous fistula (CCF) is a condition with abnormal flow from the carotid artery to the cavernous sinus. The characteristic clinical findings are ophthalmic monoplegia, ptosis, exophthalmos, chemosis, Horner's syndrome, facial pain, loss of sensation in the distribution of the trigeminal nerve, and headache. Digital subtraction angiography (DSA) is the gold standard modality for evaluating CCF. An 18-year-old patient came with decreased bilateral visual acuity after a traffic accident. CT angiography was performed on the patient and found a dilated right superior ophthalmic vein. CCF

The oculocardiac reflex has been well described in the literature and was first defined in 1908 by Aschner. The phenomenon involves the afferent limb of the ophthalmic division of the trigeminal nerve as well as the efferent pathway involving the vagal nerve leading to negative chronotropic effects. It results in a decrease in heart rate and oftentimes a decrease in blood pressure associated with compression of the eye or traction of the extraocular muscles. This reflex has clinical significance in both the operating room during ophthalmic procedures and in the emergency department in patients

A case of mucormycosis in a 45 year-old woman with uncontrolled diabetes is reported. Following dental extractions, the patient presented with total ophthalmoplegia, chemosis and complete sensory loss of the right fifth cranial nerve. Exophthalmus was not present. Although a fungal infection was suspected, administration of Amphotericin B was avoided because of renal insufficiency. After temporary clinical improvement with high doses of antibiotics, signs of right seventh and eighth cranial nerve paralysis and of thrombosis of the right ophthalmic artery developed. The patient expired after si

Sturge-Weber syndrome (SWS), also known as encephalotrigeminal angiomatosis, is a rare congenital neurocutaneous syndrome characterised by a triad of cutaneous, ocular and central nervous system findings. In the majority of the patients, it manifests as unilateral facial port-wine stain (PWS), which is a capillary malformation frequently affecting the first branch (ophthalmic division) of the trigeminal nerve. The ocular manifestations involve glaucoma and choroidal haemangiomas, whereas intracranial leptomeningeal venous angiomas form part of the central nervous system involvement. There are

Orbital apex syndrome (OAS) is an optic nerve dysfunction with palsy of the third, fourth and sixth cranial nerves and ophthalmic division of the fifth cranial nerve. Causes can be infective, inflammatory, traumatic, neoplastic or vasculitic. We describe the first case in British literature and second worldwide of OAS presenting as granulomatosis with polyangiitis (GPA). A 38-year-old patient presented with left periorbital swelling and pain. An examination revealed left eye proptosis, tenderness in the ophthalmic distribution of the trigeminal nerve, unilateral ophthalmoplegia and reduced vis
