BACKGROUND: Acute isolated optic neuritis is often the first manifestation of multiple sclerosis (MS). Despite the results of several clinical trials its management remains controversial. With the advent of new disease-modifying agents for the treatment of MS, management of isolated optic neuritis has become more complicated. The goal of this study was to evaluate the current clinical practice of French ophthalmologists and neurologists in the management of acute isolated optic neuritis, and to evaluate the impact of recently published randomized clinical trials on their practice.

BACKGROUND/PURPOSE: Ocular toxoplasmosis is the most frequent cause of posterior uveitis in Germany. The purpose of this survey was to evaluate current strategies in the management of ocular toxoplasmosis by uveitis specialists in Germany.
METHODS: An itemized questionnaire including clinical case reports with authentic photographs was distributed to physician members (n=40) of the German Uveitis Society. The term 'functional nasolacrimal duct obstruction' describes patients with epiphora where the tear duct is partially or completely patent to syringing. Delayed tear drainage is demonstrated with a fluorescein dye disappearance test (FDDT) or lacrimal scintigraphy. A dacrocystogram is also frequently abnormal. Studies show that selected patients with epiphora because of functional nasolacrimal duct obstruction have an 80-90% success rate with open or endonasal dacryocystorhinostomy. A perinatally, somatically, and neurologically aggravated child born at 26 weeks weighing 860 g with preterm neuropathy is described. Typical difficulties of neonatologists and pediatric ophthalmologists in the treatment of preterm babies with this grave ocular disease are described: the diagnosis cannot be confirmed by ophthalmoscopy because the optic media are not transparent, cryo- or surgical treatment has to be delayed because of severe somatic and neurological status, and intra- and postoperative complications develop despite thorough preoperative preparation.
