Microbiological aspects of extended-wear soft contact lenses.

Corneal ulcers are the most devastating complication associated with extended-wear soft contact lenses. The increasing prevalence of microbial keratitis associated with the wear of these lenses is a major concern for the optometrist. Studies of extended-wear and daily-wear contact lens patients strongly suggest that extended-wear patients face a greater risk of ulcerative keratitis than other cosmetic contact lens wearers. This discussion addresses the potential risks of extended-wear, with emphasis on ulcerative keratitis, the likely causes for these increased risks, and what the optometrist

An automated microaneurysm detector as a tool for identification of diabetic retinopathy in rural optometric practice.

BACKGROUND: With the increase in the prevalence of diabetes, rural optometric clinics stand to increase their patient load and assessment of diabetic eye disease. This study aimed to assess whether automated identification of diabetic retinopathy based on the presence of microaneurysms is an effective tool in clinical practice. METHODS: We analysed 758 fundal images of 385 patients with diabetes attending the clinic obtained using a Canon CR5 with an EOS10 digital camera through a dilated pupil. Five optometrists employed in the clinic assessed the diabetic retinopathy using binocular indirec

Results of a Glaucoma Shared Care Model Using the Enhanced Glaucoma Staging System and Disc Damage Likelihood Scale with a Novel Scoring Scheme in New Zealand.

PURPOSE: To report the results of a scheme sharing follow-up glaucoma consultations between community optometrists and secondary care in a hospital ophthalmic clinic. PATIENTS AND METHODS: Optometrists measured intraocular pressure (IOP) with Goldmann Tonometry, graded the optic disc with the Disc Damage Likelihood Score (DDLS) and graded visual field tests with the enhanced Glaucoma Staging Score (eGSS). A scoring system from these tests was applied, and patients were either seen routinely 6 months later within the scheme or returned non-routinely to secondary care. Appointments within the s

Exploring myopia management practices among eye care professionals in India: A knowledge, attitudes, practices, and barriers study.

CLINICAL RELEVANCE: Rising myopia prevalence in Indian children necessitates an increase in the number of myopia practitioners across all states to effectively counteract its progression and prevent long-term visual impairment. PURPOSE: There is limited information on how eye care practitioners perceive and practice myopia control treatments in India. This study aimed to assess the current knowledge, attitudes, practices, and barriers to initiating myopia management among eye care practitioners in India. METHODS: A self-administered online questionnaire was distributed through social media p

Focus on eye care in schizophrenia.

Schizophrenia, a neurodevelopmental mental disorder with heterogeneous features, poses major social and health-care challenges in Australia and worldwide. Ophthalmic findings in patients with schizophrenia are varied and include conditions that result from psychotropic complications such as sun gazing, heritable genetic endophenotypes such as oculomotor abnormalities and strabismus, treatment-related complications such as chlorpromazine lenticular and corneal deposits, and co-morbid health problems such as poor compliance. This report reviews special considerations for eye care in schizophreni

Smallpox and smallpox vaccine: ocular and systemic risks and ethical uncertainties.

BACKGROUND: The threat of bioterrorism and use of biological weapons has drawn renewed attention to smallpox, and smallpox vaccinations have been resumed in the United States. Both smallpox and smallpox vaccine carry risk of potentially debilitating or fatal adverse effects. The optometrist must be familiar with the signs and symptoms of smallpox disease and complications of smallpox vaccine for proper management and preservation of vision. OVERVIEW: The literature on the ocular and systemic effects of smallpox and smallpox vaccination is reviewed to provide the practicing optometrist with an

Real-World Evaluation of the Eye+Dot Online Triage Support Tool in Community Optometry Practices: Mixed Methods Evaluation Study.

BACKGROUND: Many patients attending hospital emergency services with recent-onset eye symptoms could have been managed in the community. This may reflect a lack of specialist experience or triage capacity among primary care providers. Online triage tools collect patient symptomatology and relevant ophthalmic and medical history virtually, compile a report, and suggest an outcome decision which can support the streaming and direction of patients to a suitable service within an appropriate timeframe. OBJECTIVE: Our objectives were to implement and analyze the real-world use of a novel online tr

Dyslexia and learning disabilities: an overview.

The concept and definition of dyslexia have been evolving for more than a century. Although it is certain that dyslexia is not a unitary syndrome, a comprehensive, testable definition has eluded us. Its characteristics have been differentially related to age; maturation of the brain and nervous system; auditory, visual, and intersensory processing; genetics; nutrition; and the environment. That we are dealing with an interdisciplinary disorder is beyond question. Optometrists do not treat dyslexia. They treat reading-disabled (RD) children who manifest some type of visual dysfunction that impa

Consensus Statement on Visual Rehabilitation in Mild Traumatic Brain Injury.

Optometric visual rehabilitation therapy has been used for a variety of visual disorders. Descriptively named entities such as posttrauma visual syndrome, visual midline shift syndrome, and vertical heterophoria syndrome are frequently diagnosed by neuro-optometrists and/or behavioral optometrists in patients after stroke or head injury or in the setting of dizziness and/or headache. The scientific underpinnings of these diagnoses and treatments are weak, and published clinical studies comprise case reports and case series without comparison to control populations. Neuro-ophthalmologists are f

Glaucoma referral refinement in Ireland: managing the sensitivity-specificity paradox in optometric practice.

PURPOSE: Glaucoma referral refinement (GRR) has proven a successful demand management strategy for glaucoma suspect cases in the United Kingdom (UK). A GRR clinic was established in Dublin, Ireland to investigate the clinical viability of this pathway outside the UK's National Health Service (NHS) structures, and away from the influence of National Institute for Clinical Excellence (NICE) guidance. METHODS: Glaucoma suspect patients were recruited into the scheme following referral from community optometrists in the greater Dublin area. The GRR exam protocol was designed in consultation with

Clinical testing of mild traumatic brain injury using computerised eye-tracking tests.

Traumatic brain injury (TBI) refers to the alteration of typical brain function that occurs following a blow to the head. Even a mild case of traumatic brain injury (mTBI) can lead to long-term impairment, so accurate and timely detection is vital. Visual symptoms are common following mTBI, so while it may seem to fall outside their typical scope of practice, optometrists are ideally qualified to assess the visual impacts and help with the diagnosis of mTBI. Given that mTBI is challenging to objectively diagnose and has no universally accepted diagnostic criteria, clinicians can lack confidenc

Effect of delisting routine eye exams on optometric glaucoma diagnosis billings in Ontario.

OBJECTIVE: Routine eye examinations (REEs) are critical for glaucoma detection. In 2004, Ontario discontinued coverage for REEs for healthy individuals aged 20-64. We assessed whether this "delisting" policy affected the rate of new glaucoma diagnoses identified through REE billings. DESIGN: A population-level intervention study PARTICIPANTS: Ontarians aged 20+ insured under Ontario's universal health coverage. METHODS: Using Ontario health administrative data and segmented regression analysis, we assessed changes in the rate (per 10,000 population) of new glaucoma diagnoses billed by optome

Impact of an email advice service on filtering and refining ophthalmology referrals in England.

PURPOSE: The growing capacity-demand imbalance has necessitated the accelerated digital transformation of eye care services. The role of Oxford Eye Hospital's (OEH) email advice service has become even more relevant in the post-Covid era. We sought to evaluate its impact on referrals to secondary care. METHODS: The consultant-led OEH email advice service primarily targets primary eye care personnel (optometrists and GPs) requiring clinical advice on patient referral. Emails received between September and November 2020 were analysed for demographic data, contents, characteristics, and outcomes

A prospective evaluation of the clinical safety and effectiveness of a COVID-19 Urgent Eyecare Service across five areas in England.

PURPOSE: Although urgent primary eye care schemes exist in some areas of England, their current safety is unknown. Accordingly, the aim of the present study was to quantify the clinical safety and effectiveness of a COVID-19 Urgent Eyecare Service (CUES) across Luton, Bedford, Hull, East Riding of Yorkshire and Harrogate. METHODS: Consenting patients with acute onset eye problems who had accessed the service were contacted to ascertain what the optometrist's recommendation was, whether this worked, if they had to present elsewhere and how satisfied they were with the CUES. RESULTS: A total o
