Epidemiology of Symptomatic Human Metapneumovirus Infection in the CASCADIA Community-Based Cohort - Oregon and Washington, 2022-2024.

Human metapneumovirus (hMPV) is an important cause of respiratory illness. However, information about hMPV incidence, patient characteristics, and symptoms outside hospital settings is limited. During June 2022-March 2024, participants aged 6 months-49 years who were enrolled in the CASCADIA community-based cohort study submitted weekly illness surveys and nasal swabs, and completed follow-up illness surveys. Swabs collected 0-3 days before reporting new or worsening symptoms were tested for hMPV and other respiratory viruses by multiplex polymerase chain reaction. Incidence was analyzed using

Heavy precipitation as a risk factor for shigellosis among homeless persons during an outbreak - Oregon, 2015-2016.

OBJECTIVES: Shigella species are the third most common cause of bacterial gastroenteritis in the United States. During a Shigella sonnei outbreak in Oregon from July 2015 through June 2016, Shigella cases spread among homeless persons with onset of the wettest rainy season on record. METHODS: We conducted time series analyses using Poisson regression to determine if a temporal association between precipitation and shigellosis incidence existed. Models were stratified by housing status. RESULTS: Among 105 infections identified, 45 (43%) occurred in homeless persons. With increasing precipitat

The Oregon physician orders for life-sustaining treatment registry: a preliminary study of emergency medical services utilization.

BACKGROUND: The Physician Orders for Life-Sustaining Treatment (POLST) form translates patient treatment preferences into medical orders. The Oregon POLST Registry provides emergency personnel 24-h access to POLST forms. OBJECTIVE: To determine if Emergency Medical Technicians (EMTs) can use the Oregon POLST Registry to honor patient preferences. METHODS: Two telephone surveys were developed: one for the EMT who made a call to the Registry and one for the patient or the surrogate. The EMT survey was designed to determine if the POLST form accessed through the Registry changed the care of the

Oregon primary care providers as a frontline defense in the War on Melanoma™: improving access to melanoma education.

Melanoma is one of the deadliest forms of skin cancer but is typically cured with surgical excision when detected early. As an access point to medical care, primary care providers (PCP) play an integral role in early skin cancer detection. However, limited time for examinations and dermatologic training may present barriers to effective skin examination in the primary care setting. As a facet of Oregon Health & Science University's War on Melanoma™ (WoM), our multi-pronged outreach initiative aims to provide PCPs across Oregon with free, convenient, and effective melanoma education. The WoM PC

Delayed Plasmodium falciparum Malaria in Pregnant Patient with Sickle Cell Trait 11 Years after Exposure, Oregon, USA.

Delayed Plasmodium falciparum malaria in immigrants from disease-endemic countries is rare. Such cases pose a challenge for public health because mosquitoborne transmission must be rigorously investigated. We report a case of delayed P. falciparum malaria in a pregnant woman with sickle cell trait 11 years after immigration to the United States.

Postpartum Hospital Readmissions With and Without Severe Maternal Morbidity Within 1 Year of Birth, Oregon, 2012-2017.

Postpartum readmissions (PPRs) represent a critical marker of maternal morbidity after hospital childbirth. Most severe maternal morbidity (SMM) events result in a hospital admission, but most PPRs do not have evidence of SMM. Little is known about PPR and SMM beyond the first 6 weeks postpartum. We examined the associations of maternal demographic and clinical factors with PPR within 12 months postpartum. We categorized PPR as being with or without evidence of SMM to assess whether risk factors and timing differed. Using the Oregon All Payer All Claims database, we analyzed hospital births fr

Accuracy of behavioral health variables in Oregon national violent death reporting system data: a linked cohort study.

BACKGROUND: The National Violent Death Reporting System (NVDRS) collects data on the circumstances of violent deaths, and all firearm-related deaths, across states and territories in the USA. This surveillance system is critical to understanding patterns and risk factors for these fatalities, thereby informing targets for prevention. NVDRS variables include behavioral health conditions among decedents, but the validity of the reported behavioral health data is unknown. Using Department of Veterans Affairs (VA) healthcare records as a criterion standard, we examined the accuracy of NVDRS-report

Who uses a prescription drug monitoring program and how? Insights from a statewide survey of Oregon clinicians.

UNLABELLED: Prescription drug monitoring programs (PDMPs) are relatively new but potentially useful tools to enhance prudent prescribing of controlled substances. However, little is known about the types of clinicians who make the most use of PDMPs, how these programs are incorporated into clinicians' work flow, or how clinicians and patients respond to the information. We therefore surveyed a random sample of Oregon providers, with 1,065 respondents. Clinicians in emergency medicine, primary care, and pain and addiction specialties were the largest number of registrants, but many frequent pre

Hepatitis C treatment eligibility among HIV-hepatitis C virus coinfected patients in Oregon: a population-based sample.

Approximately 287,000 individuals in the USA are coinfected with HIV and hepatitis C. Recently, new hepatitis C regimens have become available, increasing rates of sustained virologic response in the monoinfected, with studies evaluating their success in the coinfected under way. Previous investigators estimated eligibility for hepatitis C therapy among the coinfected patients, but all had significant methodological limitations. Our study is the first to use a multi-year, statewide, population-based sample to estimate treatment eligibility, and the first to estimate eligibility in the setting

Extreme precipitation appears a key driver of mercury transport from the watershed to Cottage Grove Reservoir, Oregon.

An abandoned cinnabar mining and roasting site is in the major sub-basin of the watershed for Cottage Grove Reservoir, Oregon. Average surface sediment total mercury concentration in the river draining this sub-basin (0.61 ± 0.52 μg/g) was about ten-fold higher than three smaller tributaries to the reservoir. Total mercury in reservoir surface sediments averaged 1.66 ± 0.70 μg/g. Stratigraphy for two sediment cores indicated generally decreased reservoir mercury loading from 1963 to 2002 but two pronounced peaks in mercury deposition. Years of extreme precipitation immediately prior to these p

Rising congenital syphilis amid declining perinatal HIV: a 7-year study from a tertiary medical center in Oregon.

We analyzed laboratory testing and positivity trends for syphilis and human immunodeficiency virus (HIV) to evaluate the effectiveness of public health policies and clinical practices. Laboratory data from unique patients were reviewed for syphilis (2018-2024) and HIV (2021-2024), stratified by age, gender, prenatal or newborn status, and county of residence in Oregon. Syphilis testing volumes reflected shifts in healthcare utilization before, during, and after the COVID-19 pandemic. HIV testing increased steadily from 6,627 patients in 2021 to more than 18,600 in 2024. Despite an average of ~

Represent: A community engagement roadmap to improve participant representation in cancer early detection research: An Oregon case study.

INTRODUCTION: While authentic and sustained community involvement in the research process is critically important to making new technologies and interventions effective and socially acceptable, there is uneven participation across sociodemographic, racial, and ethnic communities in many research areas, including cancer early detection research. Currently, 18% of cancer in the United States impacts Hispanics and Latinos, this population accounts for < 10% of research participants. Understanding barriers and facilitators to cancer early detection research is imperative to the ultimate success of

Resistome characterization of extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli isolated from wastewater treatment utilities in Oregon.

Infections resistant to broad spectrum antibiotics due to the emergence of extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae is of global concern. This study characterizes the resistome (i.e., entire ecology of resistance determinants) of 11 ESBL-producing Escherichia coli isolates collected from eight wastewater treatment utilities across Oregon. Whole genome sequencing was performed to identify the most abundant antibiotic resistance genes including ESBL-associated genes, virulence factors, as well as their sequence types. Moreover, the phenotypes of antibiotic resistance
