Ossicular chain reconstruction (OCR) is the gold standard for repairing conductive hearing loss (CHL) using prosthetic devices. However, few techniques can validate prosthesis performance before translational implementation. This study not only validates a Finite Element (FE) model for examining (reconstructed) middle ear biomechanics, but also evaluates the potential of personalised prosthetic devices for OCR. Additionally, this research examines tricalcium phosphate (TCP), as a potential biomaterial, besides titanium and hydroxyapatite (HA), as commonly used biomaterials for use in (personal

OBJECTIVE: To evaluate electromechanical excitation as an alternative excitation mode for middle ear transfer function (METF) measurements as well as real-time feedback in prosthetic ossicular reconstruction.
METHOD: In eight human cadaveric temporal bones, the ossicular chain was excited using acoustic and mechanical (floating mass transducer, FMT) stimulation to determine the METF. After disconnecting the ossicular chain and reconstruction with partial or total prosthesis the METFs were measured again. Continuous FMT stimulation was then applied to improve the prosthesis' position using rea

Knowledge of the vascularization of the malleus may help to better understand some unwanted effects occurred in those surgical techniques using the handle as an attachment site of ossicle-replacement prostheses. Our hypothesis is that vascular damage occurred in the long process of the malleus during tympanic membrane detachment before fastening a malleovestibulopexy (MVP) prosthesis promotes the lysis of this ossicle at the attachment site. To confirm this premise, we carried out an evolutive and descriptive study of ten MVP procedures as well as a morphological analysis of ten cadaveric mall

INTRODUCTION AND OBJECTIVE: Many designs of prostheses are available for middle ear surgery. In this study we propose a design for a new prosthesis, which optimises mechanical performance in the human middle ear and improves some deficiencies in the prostheses currently available. Our objective was to design and assess the theoretical acoustic-mechanical behaviour of this new total ossicular replacement prosthesis.
METHODS: The design of this new prosthesis was based on an animal model (an iguana). For the modelling and mechanical analysis of the new prosthesis, we used a dynamic 3D computer

HYPOTHESIS: Custom prostheses could be used to recreate the ossicular chain and improve hearing.
BACKGROUND: Ossicular discontinuity or fixation occurs in 55% of cases of conductive hearing loss, with most cases involving the incus. Reconstruction has been achieved by a variety of methods; however, there has been little improvement in hearing outcomes in decades.
METHODS: Precise measurements of anatomic dimensions, weight, and center of gravity were taken from 19 cadaveric incudes. These measurements were combined with measurements from the medical literature and micro-computed tomography (

BACKGROUND: Fully implantable active middle ear implants require implantable sensors to replace external microphones. A recently proposed piezoelectric sensor for ossicular coupling is promising, but data on anatomical feasibility and the impact of coupling mechanics on sensor performance and feedback are limited.
OBJECTIVE: To assess the anatomical feasibility of ossicular coupling sites and evaluate sensor sensitivity and feedback for alternative coupling strategies using a piezoelectric sensor.
METHODS: Ear morphology was assessed in a retrospective, single center analysis of 101 ears fro

Revision stapedectomy can be a technically demanding operation. The surgeon must be prepared for many pathologic conditions before revision stapes surgery. Appropriate preoperative patient counseling is a must. The best chance for hearing improvement is in those cases that have a delayed conductive hearing loss after primary stapedectomy. This article serves as a guideline for discussing the myriad possibilities that may be encountered during this type of revision stapedectomy. Indications for revision stapedectomy and general surgical guidelines for management of specific pathologic condition

OBJECTIVES: The aim of this study is to describe a method to repair small defects (1 to 2 mm) of the ossicular chain by means of glass ionomer cement (GIC) alone. Larger defects can be repaired with a combination of GIC and a platinum wire prosthesis. GIC is a two-component hybrid material consisting of inorganic glass particles surrounded by an insoluble hydrogel matrix. GIC has been used by dentists for many years, but only a few reports describe the use of GIC in human middle ear surgery. Many investigators have proved that most GICs are biocompatible, biostable, and well tolerated by bone

Creation of the middle ear cavity in primary tympanoplasty in patients with chronic purulent epi- and epimesotympanitis is made in the end of the operation after the sanative measures. The destroyed chain of the auditory ossicles is repaired using tephlon daisy-shaped graft or its modifications. The graft is covered with a thin cartilaginous plate while the membraneous defect is filled with the temporal fascia. Lateral attic wall and posterior wall of the external acoustic meatus are made of cartilaginous plates, the cavity in the mastoid process is filled with fragments of the allogenic carti

OBJECTIVES/HYPOTHESIS: In conductive hearing loss (HL), even though preoperative examinations including audiometry, tympanometry, and computed tomography (CT) scan are applied, exploratory tympanotomy is necessary to make an exact diagnosis of ossicular interruption and stapes fixation. Precise preoperative diagnosis would be useful for patient counseling, surgical indications, and planning. The advantages of office-based OtoScan laser-assisted myringotomy (LAM), which makes a circular and non-hemorrhagic perforation, prompted us to investigate other uses of LAM in addition to ventilation and

BACKGROUND: Total ossicular replacement prostheses (TORP) are often used to re-establish ossicular coupling of sound in an ear lacking a stapes supra-structure. The use of TORPs, however, is associated with a 2/3 five year failure rate due to their anatomic instability over time in the middle ear. The use of autologous fat to try and stabilize TORPs may improve long-term results with this challenging ossicular reconstruction technique.
METHODS: A cadaveric temporal bone model was developed and laser Doppler vibrometry was used to measure and record round window membrane vibration in response
