Reconstruction of the mandibular condyle using transport distraction osteogenesis.

Transport distraction osteogenesis was used to reconstruct the articulation of the temporomandibular joint in two patients. Patient 1 underwent a gap arthroplasty for release of a bony ankylosis. Patient 2 had degenerative joint disease after tumor resection. Both patients had painful, limited mouth opening and facial asymmetry as a result of condylar shortening. A reverse-L osteotomy was performed from the sigmoid notch to the posterior border of the mandible. An external transport distraction device was applied, and the segment was advanced superiorly, 1.0 mm per day, until contact was made

Mandibular distraction osteogenesis: effects on articulation and velopharyngeal function.

Mandibular distraction osteogenesis has gained popularity for the treatment of mandibular hypoplasia and asymmetry. With this technique, significant skeletal and soft tissue changes in the lower face can be produced in a relatively short time period without the need for extensive surgery. Despite general acceptance, the long-term sequelae of the procedure have not been well documented. In this report, we describe two patients who experienced changes in their articulation and velopharyngeal function after unilateral mandibular distraction. The first patient underwent 35 mm and the second patien

Le Fort III distraction osteogenesis: modification of the intraoral splint in the rigid external distraction.

Le Fort III distraction osteogenesis (DO) is a useful procedure for treating midface hypoplasia. The external distraction system has the advantage of allowing modification of the distraction vector during the activation period. This system needs an intraoral splint for the anchorage points. The intraoral splint has an important role in conducting the distraction force to the midface and in forming the advancement vector. The authors developed a modified type of splint to improve the outcome of Le Fort III DO.The intraoral splint is composed of a modified headgear face bow, molar bands, and a p

The "FAD" (Floating Alveolar Device): a bidirectional distraction system for distraction osteogenesis of the alveolar process.

PURPOSE: The following study illustrates preoperative and perioperative vector management in alveolar distraction using a new distraction system--the "Floating Alveolar Device" (FAD). The FAD is a bidirectional alveolar distractor that allows augmentation of an atrophic alveolar process in several planes, assures easy intraoperative positioning of the planned vector of distraction, and provides correction of the horizontal position of the transported segment during and after vertical distraction. PATIENTS AND METHODS: The FAD is composed entirely of stainless steel and has the following basic

Dissolving Doubts: Resorbable Fixation in Posterior Vault Distraction Osteogenesis.

Posterior cranial vault distraction osteogenesis (PVDO) traditionally requires titanium screw fixation of metallic distractor footplates. Cho and colleagues recently described an off-label application of the Synthes RapidSorb Injectable Polymer System for footplate fixation in PVDO, reporting reduced distractor removal operative time in an initial single-institution series. This study expands that cohort and further evaluates operative outcomes across a multisurgeon institutional experience. Retrospective review identified consecutive PVDO patients treated between 2013 and 2025. The most recen

Treatment Experience With Midfacial Distraction Osteogenesis for Down Syndrome.

Patients with Down syndrome have special characteristics, such as a round flat face, epicanthal fold, up-slanting palpebral fissures, flat nasal bridge, relative macroglossia, midfacial hypoplasia, short stature, short neck with extra skin at the nape, muscle hypotonia, ligament laxity, and joint hypermobility. Some of these characteristics can lead to obstructive sleep apnea. Epicanthal fold, up-slanting palpebral fissures, flat nasal bridge, and relative macroglossia are candidates for plastic surgery treatment. However, to the best of the authors' knowledge, there have been no reports of mi

Hemi one-piece distraction osteogenesis for unilateral coronal craniosynostosis.

INTRODUCTION: It was the aim of the study to assess the efficacy of the hemi one-piece distraction osteogenesis and to compare it to the traditional one-piece distraction osteogenesis technique. METHODS: Two different surgical techniques were used; the one-piece distraction and the hemi one-piece distraction. The principal distinction between the two techniques is that in the hemi style approach, the intact sutures on the contralateral side were left undisturbed, with no osteotomy performed. RESULTS: The hemi one-piece group had a significantly lower median value of plastic surgery time, tot

The Radiographic "Air Sign" in Neonatal Mandibular Distraction Osteogenesis.

Timing of extubation on post-mandibular distraction osteogenesis (MDO) surgery is critical, given that at baseline these infants have difficult airways and failed extubation requires either re-intubation of an already complex airway with a fragile, recently osteotomized mandible, or adjunctive airway measures such as CPAP that may apply unwanted pressure to the surgical site. Thus, the goal is to plan extubation when the risk of failure is minimal. Currently, there is a void in the literature addressing the timing of extubation post-MDO and no objective sign of extubation readiness has been el

Distraction osteogenesis with temporal bone remodeling for unicoronal craniosynostosis.

Unicoronal craniosynostosis correction with fronto-orbital advancement and cranial vault remodeling has traditionally been the gold standard. Distraction osteogenesis has the advantage of increased size of movement without constriction of the scalp and decreased morbidity. Although fronto-orbital advancement and cranial vault remodeling are usually performed at 6 months of age or later, distraction osteogenesis is performed at a younger age, between 3 and 6 months, to take advantage of the infant bony physiology. Herein, the authors demonstrate a case of distraction osteogenesis for unicoronal

Posterior cranial vault distraction osteogenesis in the immunocompromised patient.

PURPOSE: The treatment of patients with multisuture craniosynostosis is complex and patient-dependent. Cranial distraction osteogenesis is a relatively new procedure for treatment of these patients, with its use increasing in many centers. With this increased use comes an expanding range of indications. Surgical management of multisuture craniosynostosis in therapeutically immunosuppressed patients following a solid organ transplant presents unique challenges. We describe our experience with posterior cranial vault distraction in two patients with multisuture craniosynostosis that had previous

Sema3A accelerates bone formation during distraction osteogenesis in mice.

Distraction osteogenesis (DO) is a bone regeneration technique used to treat maxillofacial disorders, fracture nonunion, and large bone defects. It is well known for its amazing regenerative potential, but an extended consolidation period limits its clinical use. The interaction between the nervous system and bone regeneration has attracted great attention in recent years. Sema3A is a key axonal chemorepellent which has been proved to have bone-protective effects. In this article, we try to improve DO by local administration of Sema3A and explore the possible mechanisms. Forty wildtype, male,

Hemifacial microsomia managed by distraction osteogenesis: A clinicoradiological report.

Hemifacial microsomia (HFM) is a congenital malformation in which there is hypoplasia of hard and soft tissues on one side of the face. It is considered the second most common congenital syndrome of the head-and-neck region. The hypoplasia manifests in all the structures derived from the first and second branchial arches and therefore, growth and development of these structures is affected in this condition. The present case is a variant of HFM in a 5-year-old Indian male child, which has been diagnosed based on clinical and radiological findings and managed successfully by surgical technique

Rehabilitation post-distraction osteogenesis for brachymetacarpia: a case report.

UNLABELLED: Distraction osteogenesis for brachymetacarpia has been described in several small case series and single case reports, but the rehabilitation required to optimize outcomes has not been reported. We present a case report describing the hand rehabilitation program of a 13-year-old girl with congenital brachymetacarpia who underwent distraction osteogenesis of the third metacarpal. Intense weekly hand therapy including desensitization, scar massage, range of motion exercises and splinting was essential up to 28 weeks postoperatively to address the progressive changes in the anatomical

Abducens Nerve Palsy Following Expansion Cranioplasty with Distraction Osteogenesis.

Childhood abducens nerve palsy can occur as a result of trauma, tumour, vasculopathic disease, elevated intracranial pressure, infection, inflammation, and congenital or idiopathic causes. The authors present two cases of unilateral abduction deficit secondary to a recent trans-sutural distraction osteogenesis (TSuDO) operation for craniosynostosis. After distractor removal, the problem resolved spontaneously over 2-4 months in both cases. This is a first reported case of sixth nerve palsy as a complication of TSuDO operation.

A craniometric analysis of posterior cranial vault distraction osteogenesis.

BACKGROUND: Posterior cranial vault distraction osteogenesis has replaced fronto-orbital advancement in some centers as the first-line treatment in patients with syndromic craniosynostosis. Despite this fact, little has been written about its craniometric effects on children with syndromic craniosynostosis. METHODS: A retrospective review of all patients who underwent posterior distraction was performed. Patient demographic, perioperative data, and preoperative/postoperative computed tomographic scans were reviewed. Volumetric and craniometric indices were calculated and measured using commer

Maxillomandibular tandem osteotomy with distraction osteogenesis for hemifacial microsomia.

Currently, simultaneous maxillomandibular distraction osteogenesis is a standard procedure for the correction of hemifacial microsomia. Many variations of the procedure have been reported. However, a common problem has been the extended duration of the procedure. In particular, the lengthy intermaxillary fixation period includes a prolonged distraction series that requires the restriction of oral food intake and creates significant stress for the patient, which has a profound effect on patient satisfaction. To avoid this stress, we have developed tandem osteotomy with distraction osteogenesis,

Complications in transpalatal distraction osteogenesis: a retrospective clinical study.

PURPOSE: Transpalatal distraction osteogenesis is a bone-borne technique to expand the maxilla and has become a routine method in treating patients with transverse maxillary hypoplasia. Limited reports concerning treatment difficulties have been published. The purpose of this study was to investigate and categorize the short- and middle-term incidence of peri- and postoperative difficulties (categorized by the classification of Paley in problems, obstacles, and complications). The signs of adverse dental and periodontal effects were established at least 1 year after removal of the distractor.

Expansion of the posterior cranial vault using distraction osteogenesis.

BACKGROUND: Expansion of the posterior cranial vault more profoundly enlarges intracranial volume compared with the anterior region. Conventional vault remodeling techniques are limited by scalp forces and may relapse with supine positioning. The purpose of this study was to demonstrate the efficacy of posterior vault distraction and evaluate perioperative variables compared with conventional methods in syndromic children. METHODS: This was a retrospective analysis of consecutive children who underwent posterior vault expansion using distraction osteogenesis. Information was compiled regardin
