The effect on the ear of late closure of the cleft hard palate.

Thirty-three patients with cleft palate are reviewed. The late effects on the ear of delayed closure of the hard palate after early soft palate repair are compared with the results of simultaneous early closure of both hard and soft palate clefts. Delayed closure of the hard palate does not affect the incidence of otitis media, the number of insertions of ventilation tubes or the findings on otoscopy. The prevalence of sensorineural hearing loss is significantly raised by late closure of the hard palate. The reason for this is unknown.

Endoscopy in otolaryngology utilising smartphone as the capturing device.

Mobile technology has been a significant development. With the advent of smartphones and many applications, it is possible to integrate endoscopic instruments. This is a study where the authors were able to use a smartphone as the capturing device for endoscopic examinations and were able to get satisfactory results on all but two of the 52 patients, doing four endoscopic examinations commonly being performed in otolaryngology clinics: rigid nasal endoscopy, rigid laryngoscopy, otoscopy and flexible fiberoptic naso-laryngoscopy. This article reveals their experience.

[Hearing disorders in 3- to 12-year-olds].

The hearing disorders of the child between 3 and 12 years are dominated by the conductive hearing loss. The questioning, looking for risk factors, as well as the otoscopy are essential to detect and diagnose a hearing disorder. Screening tests help to identify the children with risk of hearing loss. Then, the child is sent for diagnosis towards an ENT specialist or a specialized structure. In spite of the frequency of the conductive hearing loss, in case of the slightest doubt, a sensorineural hearing loss must be always looked for by a puretone audiometry examination via the headphones.

[Temporo-mandibular joint pain dysfunction syndrome in children].

Otalgia is a common symptom in children. Otological examination usually reveals a local pathology as the causative agent. However, it does occur that otoscopy reveals normal ear drums and canals. The otologist must then review the causes of referred otalgia. We know that the ear is a nervous crossroad: vagus, trigeminal and third cervical nerve. The most common causes of referred otalgia are the teeth, the sinus, the mouth, the pharynx, and the larynx. If none of these factors is involved, one must then think about the painful temporomandibular dysfunction syndrome.

Digital otoscopic examination of tympanic membranes in confirmed suicidal hanging deaths: Preliminary autopsy study.

A crucial factor in cases of apparent hanging deaths is establishing whether an antemortem hanging or postmortem suspension has occurred. Hanging diagnosis relies upon the history, scene investigation, and gross examination of the body including the ligature furrow and possible resultant injuries including hemorrhages, fractures, and petechiae. However, pathognomonic changes are lacking. Examination of the tympanic membrane may provide additional information to assist in distinguishing antemortem hanging from postmortem suspension. The changes in the tympanic membrane in cases of hanging death

New Approaches and Technologies to Improve Accuracy of Acute Otitis Media Diagnosis.

Several studies have shown that in recent years incidence of acute otitis media (AOM) has declined worldwide. However, related medical, social, and economic problems for patients, their families, and society remain very high. Better knowledge of potential risk factors for AOM development and more effective preventive interventions, particularly in AOM-prone children, can further reduce disease incidence. However, a more accurate AOM diagnosis seems essential to achieve this goal. Diagnostic uncertainty is common, and to avoid risks related to a disease caused mainly by bacteria, several childr

Comparison of spectral gradient acoustic reflectometry and tympanometry for detection of middle-ear effusion in children.

BACKGROUND: Spectral gradient acoustic reflectometry (SGAR) and tympanometry are handy methods to aid in the diagnostics of middle-ear diseases, but no prior studies have directly assessed the correlation between SGAR and tympanometry readings in children. METHODS: We compared SGAR and tympanometry in 2152 otoscopic examinations among 600 children aged 0.6-14 years, who were examined for signs and symptoms of respiratory infection in a primary care setting; 677 (31.5%) of the ear examinations were in children <3 years of age. We further determined the test characteristics of these methods for

Pressure-equalizing earplugs do not prevent barotrauma on descent from 8000 ft cabin altitude.

INTRODUCTION: The aim of this study was to evaluate the effect of pressure-equalizing earplugs available in major airports and drugstores. No previous study has focused on preventing barotrauma using these earplugs. METHODS: Blinded and double-blinded, one type of pressure-equalizing earplugs (JetEars) was studied in 27 volunteers disposed to ear barotrauma. They acted as their own controls with an active earplug in one ear and a placebo earplug in the other ear at random. All were exposed to the same well-defined pressure profile for 1 h at 8000 ft, comparable to the environment in civil com

Feasibility of a hearing screening programme using DPOAEs in 3-year-old children in South Auckland.

OBJECTIVES: In New Zealand (NZ), permanent hearing loss is associated with higher levels of socioeconomic deprivation, and is more prevalent amongst Māori and Pacific than NZ European children. Many of these hearing losses are detected through newborn hearing screening, however there is a need to screen children again later, to look for childhood hearing losses that are either late-onset, progressive, or acquired. This study evaluated the feasibility of implementing an objective screening protocol that includes otoscopy, distortion product otoacoustic emission screening (DPOAEs), and tympanome

Otoendoscopy: Tricks and tips for dealing with bleeding.

For pediatric myringoplasty, otoendoscopy is less invasive than traditional microscopic surgery, enabling a wide-angle view and precise identification of anatomic and pathological structures. It has significant advantages, with high rates of tympanic closure, shorter operating time and simpler postoperative course. Bleeding, however, notably in children's narrow auditory canals, is a major problem, impairing visibility and complicating surgery. There are various techniques for dealing with it: cautious infiltration of the external auditory canal, and adrenaline-soaked cotton pledgets help redu

Optimized fine-tuned ensemble classifier using Bayesian optimization for the detection of ear diseases.

External and middle ear diseases are common disorders, especially in children, and can be examined using a digital otoscope. Hearing loss can result from delayed diagnosis and treatment which is subjective and error-prone depending on the expertise of the otolaryngologist. For these reasons, deep learning-based automated diagnostic systems are highly needed. In this study, a novel weighted average voting ensemble classifier between MobileNet and DenseNet169 has been developed to diagnose and detect different ear conditions. Bayesian optimization was used to select hyperparameters that gave the

Effects of freediving on middle ear and eustachian tube function.

INTRODUCTION: During descent in freediving there is exposure to rapidly increasing pressure. Inability to quickly equalise middle ear pressure may cause trauma to the ear. This study aimed to evaluate the occurrence of pressure-related damage to the middle ear and the Eustachian tube during freediving and to identify possible risk factors. METHODS: Sixteen free divers performed diving sessions in an indoor pool 20 metres' freshwater (mfw) deep. During each session, each diver performed four own free dives and up to four safety dives. Naso- and oto-endoscopy and Eustachian tube function tests

Using the shortwave infrared to image middle ear pathologies.

Visualizing structures deep inside opaque biological tissues is one of the central challenges in biomedical imaging. Optical imaging with visible light provides high resolution and sensitivity; however, scattering and absorption of light by tissue limits the imaging depth to superficial features. Imaging with shortwave infrared light (SWIR, 1-2 μm) shares many advantages of visible imaging, but light scattering in tissue is reduced, providing sufficient optical penetration depth to noninvasively interrogate subsurface tissue features. However, the clinical potential of this approach has been l

[Use of tympanometry in children].

Tympanometry provides useful informations about the presence of fluid in the middle ear, mobility of the tympanossicular system, and ear canal volume. Its use is recommended in conjunction with other informations as history, otoscopy and general signs in the evaluation of an otologic pathology. Tympanometry can also indicate abnormal pressure in the middle ear, patency of tympanostomy tubes, retracted or perforated tympanic membrane or impaired function of the eustachian tube. It is not reliable in infants younger than seven months because of their highly compliant ear canals.

Acute otitis media in adults: many unknowns.

(1) Acute otitis media is likely in adults with recent-onset otalgia, fever, and a bulging eardrum on otoscopy. Management is similar to that in children: no antibiotics in the first instance, then amoxicillin later if antibiotic therapy is needed. (2) Otitis media with a perforated eardrum, and evidence of pus must be distinguished from external otitis. In patients with otitis media and a perforated eardrum, the commonest bacterial isolates are staphylococci, pseudomonas and pneumococci. (3) If antibiotics are prescribed the choice of agent is based on individual clinical findings and underly

Acute otitis media in children.

Acute otitis media (AOM) is the most common reason parents bring children into the primary care physician's office. It is diagnosed by clinical symptoms of otalgia and often fever and irritability and by clinical otoscopy. Organisms common to the nasopharynx usually cause AOM. Many episodes of AOM resolve spontaneously. Because it is uncertain how many and which children will resolve without therapy, it is reasonable to treat all children with AOM with antibiotics. The first choice of antibiotics is amoxicillin. Other choices rest on a variety of factors.
