Overdiagnosis of thyroid cancer in the United States: Improved estimates based on the SEER registries during 2000-2019.

The rising thyroid cancer (TC) incidence is largely attributed to overdiagnosis, primarily due to widespread use of sensitive imaging technologies. Prior estimates of overdiagnosis relied on generalized assumptions that may not account for population-specific risk exposures and diagnostic trends. This study aims to provide more accurate estimates by addressing previous methodological limitations. In this population-based study, we obtained aggregated data on the incidence and mortality of TC during 2000 to 2019 from the Surveillance, Epidemiology, and End Results 22 (SEER-22) and SEER-17. Beyond Overdiagnosis: Evaluating the Drivers of Increasing Thyroid Cancer Incidence in the Era of Obesity, Environmental Exposure, and Advanced Diagnostics.

The global incidence of thyroid cancer (TC) has increased significantly in recent decades. Traditionally, this trend has been attributed to overdiagnosis resulting from the widespread use of imaging and improved diagnostic techniques, but increasing evidence suggests that current epidemiological patterns may not be fully explained by diagnostic factors alone. Most importantly, increasing rates of larger and more advanced tumors have been reported in several populations. This narrative review discusses the evidence on possible drivers of rising TC incidence: obesity and metabolic dysfunction, e

Preoperative urodynamics and the risk of overdiagnosis and overtreatment of occult urinary incontinence in continent women with advanced pelvic organ prolapse.

PURPOSE: This study aimed to investigate the impact of preoperative urodynamics (UDS) on the surgical treatment of pelvic organ prolapse (POP). It assessed whether UDS provided additional diagnoses, modified surgical planning, and influenced postoperative outcomes in continent women with advanced POP. METHODS: This retrospective observational study included women with POP stages III-IV, without symptoms of stress urinary incontinence (SUI), who underwent surgical intervention between May 2015 and January 2020. RESULTS: A total of 226 patients met the inclusion criteria, of whom 102 (45.1%) u

Evidence of potential overdiagnosis and overtreatment of attention deficit hyperactivity disorder (ADHD) in children and adolescents: protocol for a scoping review.

INTRODUCTION: Worldwide, attention deficit hyperactivity disorder (ADHD) diagnosis rates in children and adolescents have been increasing consistently over the past decades, fuelling a debate about the underlying reasons for this trend. While many hypothesise that a substantial number of these additional cases are overdiagnosed, to date there has been no comprehensive evaluation of evidence for or against this hypothesis. Thus, with this scoping review we aim to synthesise published evidence on the topic in order to investigate whether existing literature is consistent with the occurrence of o

The distribution of ductal carcinoma in situ (DCIS) grade in 4232 women and its impact on overdiagnosis in breast cancer screening.

BACKGROUND: The incidence of ductal carcinoma in situ (DCIS) has rapidly increased over time. The malignant potential of DCIS is dependent on its differentiation grade. METHODS: Our aim is to determine the distribution of different grades of DCIS among women screened in the mass screening programme, and women not screened in the mass screening programme, and to estimate the amount of overdiagnosis by grade of DCIS. We retrospectively included a population-based sample of 4232 women with a diagnosis of DCIS in the years 2007-2009 from the Nationwide network and registry of histopathology and c

Higher breast cancer prevalence associated with higher socioeconomic status in the South Korean population; Has it resulted from overdiagnosis?

Recently, breast cancer prevalence has increased in South Korea. In this study, we investigated the correlation between breast cancer prevalence and socioeconomic status. This study enrolled 27,331 people who participated in the Korean National Health and Nutrition Examination Survey (KNHANES) IV-VI (2007-2015). In addition, we obtained data from the Korean Statistical Information Service (KSIS) on the breast cancer age-standardized incidence rate (AIR), the age-standardized mortality rate (AMR), the number of women screened, and the number of newly diagnosed patients. The KHANES data showed t

Individualized prostate-specific antigen threshold values to avoid overdiagnosis of prostate cancer and reduce unnecessary biopsy in elderly men.

OBJECTIVE: To individualize prostate-specific antigen threshold values to avoid overdiagnosis of prostate cancer and reduce unnecessary biopsy in elderly men. METHODS: A total of 406 men aged over 70 years old with prostate-specific antigen levels between 4.0 and 20.0 ng/ml, normal digital rectal examination results and diagnosed by transrectal needle biopsy were retrospectively analyzed. The patients were divided into a no/favorable-risk cancer group or an unfavorable-risk cancer group based on their Gleason score and the number of positive cores. Prostate-specific antigen levels, percent fr

Evaluating the impact of age on prostate cancer overdiagnosis using long-term follow-up from a randomised trial.

Prostate cancer overdiagnosis is detection of prostate cancer through PSA testing that otherwise would not have been diagnosed within the patient's lifetime. It is a major concern to policymakers due to its impact on quality of life. We used long-term followup data from the CAP randomised trial of a one-off screen, and English male competing mortality rates (2021-23), to estimate the impact of age on excess prostate cancer incidence within 15 years ('overdiagnosis') using competing risks methods. In total, 2249 (1.19%) of 189,386 men invited for a PSA test in CAP had cancer detected at the one

Revised free light chain reference intervals enhance risk stratification in monoclonal gammopathy of undetermined significance and reduce overdiagnosis.

The free light chain (FLC) ratio is a critical part of risk stratification for monoclonal gammopathy of undetermined significance (MGUS). Recently, revised FLC reference intervals developed using the iStopMM cohort, accounting for age and renal function, have reduced the rate of abnormal findings. Here, we examine the implications of the revision in an independent Danish MGUS cohort. Of 6993 MGUS individuals, 2641 had an abnormal FLC ratio by the original intervals, of whom 844 (32%) were reclassified as normal using the revised intervals. Reclassified individuals had no significantly increase

Increasing melanoma incidence with unchanged mortality: more sunshine, better treatment, increased diagnostic activity, overdiagnosis or lowered diagnostic threshold?

BACKGROUND: Increasing melanoma incidence with less increasing mortality is observed in several countries. This discrepancy is not well understood. OBJECTIVES: In this study, our aim was to discuss factors [ultraviolet radiation (UVR) exposure, melanoma treatment, diagnostic activity, overdiagnosis, pathologists' diagnostic threshold and clinicians' propensity to remove suspect skin lesions] that might influence melanoma incidence and mortality in Denmark. METHODS: This was a register study with the number of melanocyte-related lesions and melanoma mortality based on comprehensive national p

Influence of follow-up, screening age, interval, and compliance on overdiagnosis of ductal carcinoma in situ (DCIS): A modelling study.

Overdiagnosis estimates of ductal carcinoma in situ (DCIS) vary from 20-91%, which complicates screening communication and optimization. The aim was to quantify the influence of follow-up time and screening setting on overdiagnosis of DCIS. The fully validated micro-simulation Markov model for DCIS (SimDCIS) was used to estimate DCIS overdiagnosis in different screening settings with varying follow-up. DCIS overdiagnosis was defined as the number of diagnosed DCIS (screen-, clinically detected, or progressed to invasive breast cancer) in screening that would not have been diagnosed without scr

Cancer screening activity results in overdiagnosis and overtreatment of papillary thyroid cancer: A 10-year experience at a single institution.

BACKGROUND: It is estimated that one of the potential cause of the increasing prevalence of thyroid cancer (TC) is the easier and widespread access to diagnostic tools. If an individual evaluates the thyroid gland due to a mentioned mechanism without considering TC risk factors or symptoms, we can describe this phenomenon as cancer screening activity (CSA). AIM OF THE STUDY: We 1) estimated what types of TC were diagnosed due to CSA, 2) analyzed what clinicopathological features were characteristic of TCs diagnosed by CSA, 3) determined if these features were characteristic of indolent cases,

Women's Awareness of and Responses to Messages About Breast Cancer Overdiagnosis and Overtreatment: Results From a 2016 National Survey.

BACKGROUND: Scientists, clinicians, and other experts aim to maximize the benefits of cancer screening while minimizing its harms. Chief among these harms are overdiagnosis and overtreatment. Although available data suggest that patient awareness of these harms is low, we know little about how patients respond to information about these phenomena. OBJECTIVES: Using the case of breast cancer screening, this study assesses women's awareness of and reactions to statements about overdiagnosis and overtreatment. METHODS: We draw on data from a 2016 population-based survey of US women aged 35-55 y

Long-acting porcine ACTH stimulated salivary cortisol reduces the overdiagnosis of adrenal insufficiency compared to serum cortisol in cirrhosis liver.

BACKGROUND: There are no reliable methods in clinical practice to diagnose adrenal insufficiency (AI) in patients with cirrhosis owing to variable cortisol-binding protein levels. This leads to unreliable results in ACTH stimulated serum cortisol test. We aimed to estimate the long-acting porcine (LA)ACTH-stimulated serum and salivary cortisol levels of patients at different stages of cirrhosis using second generation electrochemiluminescence and to determine the prevalence of true adrenal insufficiency in these patients. DESIGN, PATIENTS AND MEASUREMENTS: We included 135 noncritical patients

How can we predict the active surveillance candidates meeting all Epstein criteria prior to prostate biopsy to avoid overdiagnosis?

OBJECTIVE: To investigate the effectiveness of PSA, prostate volume (PV) and free-to-total PSA ratio (fPSA%) in predicting patients meeting all active surveillance criteria, including Epstein criteria. METHOD: Retrospective analysis was made of the data of 1901 men who underwent transrectal ultrasound (TRUS)-guided prostate biopsy in our clinic between January 2015 and December 2019. The active surveillance criteria were determined as Gleason score ≤6, when specified ≤2 positive cores with <50% cancer involvement in every positive core, a clinical T1c, a PSA <10ng/mL and a PSA density <0.15 n

Preventing Overdiagnosis of Acetabular Labral "Tears" in 40-Plus-year-old Patients: Shouldn't these be called Labral "Fissures" Instead?

Acetabular labral tears represent a common finding on magnetic resonance imaging of the hip. Labral tears can arise from a multitude of underlying pathological processes or they may be an asymptomatic incidental finding. The prevalence of labral tears and their lack of specificity make this an area vulnerable to potential overdiagnosis. The overdiagnosis of labral tears leads to overtreatment by exposing patients to unnecessary surgeries as well as complications ranging from unsatisfying outcomes to deep venous thrombosis. This risk is compounded by the tabloid popularization of labral surgeri

Imaging Pulmonary Embolism in the Era of Subsegmental Clots: What the Internist Needs to Know About Overdiagnosis, Anticoagulation, and Follow-Up.

The widespread adoption of multidetector computed tomography pulmonary angiography has dramatically increased detection of subsegmental pulmonary embolism (SSPE), raising critical questions about overdiagnosis, optimal anticoagulation strategies, and appropriate follow-up. Despite increased detection, observational data demonstrate remarkably low short-term recurrence rates that do not differ significantly from more proximal pulmonary embolism. However, anticoagulation carries major bleeding risks, creating clinical equipoise about net benefit in low-risk isolated SSPE. Radiologist interpretat
